In addition, the offer must contain a statement that it is firm for a period of 180 days from the solicitation closing date.” It is our assumption that this content is to be provided in the signed Cover Letter (i.e. not in the Executive Summary). 23C.10.516In reference to report submission within 5 calendar days, will FOH’s vendor(s) be able to deliver specimen and provide lab results within 24 hours? At the current time, it may take a day for the courier to deliver the specimen and that the lab facility generally provides results 2-3 business days after the lab draw and only tests specimens on work days. If specimen is delivered on a Saturday, specimens are tested the following work day, which often times result in the degradation of cell morphology. The shipping courier currently does not deliver Saturday or Sunday, unless the envelope as a “Saturday” delivery. Additionally, the examining provider is required to review/discuss the results with the employee, who is then required to sign the FOH 5 form. For some agencies, the contractor is only allowed to contact the examinee through the POC. 43C.10.516It is our understanding that a standard referral for services has 15 calendar days total to complete and submit report as Services need to be completed within 10 days of referral receipt and report submission is to be completed within five days from final appointment. Is this correct? If not please clarify.See updated C.10

Is this assumption correct? If not, please clarify.L.7 Specifically states that offerors are required to idendity actual and potential OCI's, therefore, if there are no OCI's then this would not be applicable.

L.916, 52, 62, 98Section I and L.9 contain FAR 52.237-10 -- Identification of Uncompensated Overtime (Mar 2015). This procurement is for a firm fixed price contract and this clause applies to labor hour rates and therefore is not applicable. Request removal of this clause.Removed, reference Amendment 1
L.5.358, 94Section: Cover/Cover Letter/Contact Page. RFP states to “Provide a cover letter which introduces the company, the quote, and the quote volume.” It is our assumption that the Cover Letter is separate from the Cover/Contact Page. Is this assumption correct? If not, please clarify. The contract page if the front page of the SF1449.
C.10.215Services need to be completed within 10 calendar days but the disincentive is for each business day. This creates a disadvantage for the contractor if a service request is made on a Friday where the contractor may immediately lose at least one day due to the upcoming weekend where a contractor’s PPN may not be available to schedule an appointment. Request requirement be defined as “completed within 10 business days.”See updated C.10.2
C.10.315Services need to be completed within 5 calendar days but the disincentive is for each business day. This creates a disadvantage for the contractor if an expedited service request is made on a Friday where the contractor may immediately lose at least one day due to the upcoming weekend where a contractor’s PPN may not be available to schedule an appointment. Request requirement be defined as “completed within 5 business days.”See updated C.10.3

C.9

J

M.2.3.212, 65, 105The original RFP did not include an Attachment F – Service List Supplemental Information. Please provide Attachment F.The RFP has been updated to include Attachment F.
L.861-62, 98The RFP contains “If subcontractors are proposed, Offerors shall include a commitment letter from the subcontractor detailing: (list of 7 items).” Item 6 is “A complete price proposal in the same format as the Offeror’s price proposal.” A complete Volume III includes many items which are not required for a proposed subcontractor such as completed SF 1449, completed Section K, and Small Business subcontracting plan. Also, per L.5.4.2, other than certified cost or pricing data is not required as part of the submission. Subcontractor prices are cost items for the Offeror’s price proposal and therefore not required. Request removal of this instruction. There appears to be some misunderstanding regarding the definition of a subcontractor. In the context of a PPN, a health care facility providing services within the Contractor's PPN should not be considered a subcontractor. A subcontractor would be defined as a Contractor responsible for managing part of the PPN network, not providing health care services within the network. Refernce the definition of "subcontract" under FAR 19.7.
L.861-62, 98The RFP contains “If subcontractors are proposed, Offerors shall include a commitment letter from the subcontractor detailing: (list of 7 items).” This instruction does not appear to align with the scope of services for FOHNet. Request removal of this instruction.Reference above
L.861-62, 98The RFP contains “If subcontractors are proposed, Offerors shall include a commitment letter from the subcontractor detailing: (list of 7 items).” This instruction does not appear to align with the scope of services for FOHNet. The vast majority of subcontractors are medical professionals and many of the 7 items do not apply. Request removal of this instruction as it is not applicable to medical professionals.Refernce above
M.2.2.266, 102The RFP contains a list of 6 items which will be used in evaluating each reference. Item 6 states “Demonstrating any combination of a minimum of 50 line item services from Attachment A per month.” It is our assumption that this is a minimum quantity of 50 per month of a combination of FOH services. Is this assumption correct? If not, please clarify.Yes.
C.32, 6The RFP states “all required forms will be specified in the service/work orders and made available to the Contractor in advance of service requests.” Please clarify if each referral will have all the required forms available with the referral in FedHealth.Yes, forms will be available within FedHealth.
L.5.4.161, 97The RFP states “Emergency/surge line items will not be evaluated, and are not required to be priced as part of an Offeror’s proposal.” Attachment A – Service Price List does not contain emergency/surge line items. Are these line items to be included in the price schedule and not priced at this time? Please clarify.Emergency/surge line items will not be priced at this time.
C.10.516The RFP states “Following the submission of a report, FOH reviewing physicians may have a need to contact PPN practitioners directly for clarification regarding the services provided. The Contractor shall support and facilitate this direct communication within all PPN agreements.” Since providers will want to be compensated for their time in speaking with the FOH reviewing physicians, request that a line item be added in Attachment A for this service.See updated C.10.5, Attachment A, and Attachment F.
L.5.460-61The RFP states “Offerors shall complete Attachment A with their proposed FFP for each service for either or both Regions they wish to be considered for. Each Regions Attachment A worksheet will need to be included in the respective East or West Business/Price Proposal.” It is our assumption that if an Offeror is submitting a proposal for both the East and the West Region that only the completed Attachment A for the corresponding Region is completed (i.e. only Attachment A FOHNet East worksheet is completed and submitted with the East Region Volume III and only Attachment A FOHNet West worksheet is completed and submitted with the West Region Volume III). Is this assumption correct? If not, please clarify.Yes.
L.5.460, 96The RFP states “Offerors shall include a complete copy of this solicitation and a signed copy of the front page of any amendments, as part of its Business/Price Volume.” Is the Offeror to include all solicitation files from the original release and Amendment(s) in the submission? Are these to be submitted as separate files? Please clarify instructions on including “a complete copy of this solicitation” with the proposal submission. Please thoroughly read the instructions. Not sure what needs to be clarified, as stated a complet copy of the solicitation and a copy of the amendments need to be included as part of the Business/Price Volume.
L.5.460, 96The RFP states “Offerors shall include a complete copy of this solicitation and a signed copy of the front page of any amendments, as part of its Business/Price Volume.” Please provide further instruction for submission of SF 30(s) from Amendment(s). Are these to be attachments to the Cover Letter? Please thoroghly read the sintructions, these are part of the Business/Price Volume. The cover letter is also part of the Business/Price Volume.
C.9.513-14The RFP states “Uploaded results will be paired with the individual record/referral prior to submitting to FOH for final acceptance.” Please clarify how the “pairing” activity is performed by the Contractor in FedHealth.Uploaded results will be located in a document repository within FedHealth. Users will utilize unique pairing to combine results with respective referrals.
L.5.154-55, 91The RFP states that “The Technical Appendix may only include resumes of Key Personnel” and that it is not included in the page count. The RFP does not identify or stipulate “Key Personnel.” Section C.3, 5th paragraph states that “The Contractor shall designate a qualified program manager to be responsible for all Contractor operations.” Is the program manager to be designated as Key Personnel? Please clarify what Contractor positions are to be designated as Key Personnel and if there is a page limit to the resume. The contractor needs to identify those personnel they consider Key to performance of the contract/program. Therefore, it would stand to reason that the designated Program Manager would be a key position that should be identified.
L.5.154, 91The RFP states that “The Technical Volume pages will have 1-inch margins, and using not less than 11-point font, Times New Roman.” Can smaller font sizes be used in tables and graphics, e.g. not less than 8?No.
C.1926-28The RFP states that a multiple-award indefinite-delivery, indefinite-quantity (IDIQ) contract is anticipated for this requirement and that task orders issued under this IDIQ contract shall be firm fixed-priced (FFP). This FFP contract type places upon the contractor maximum risk and full responsibility for all costs and resulting profit or loss. Based on the contract type, the penalties appear large. This situation increases the risk and prices to the Government and request reconsideration in order to support more competitive pricing. The FFP IDIQ has been determined to be the best fir for the Govt. requisrement based on the market research conducted.
L.861-62, 98The subcontracting goals in Attachment H and those in Section L.8 do not match. Section L.8 states Small Business as 35% and the Attachment H states this as 33%. Also, Section L.8 contains a goal for Veteran-Owned Small Business and the Attachment H does not. Please clarify the subcontracting goals and provide a revised template as needed.Attachment I has been corrected; Attachment H is the Past Performance Questionnaire. Section L.8 has also been corrected.
C.1926-28The Surveillance Matrix lists multiple cumulative penalties which maybe incurred by the contractor. This represents a significant demand on contractor resources and a corresponding significant increase in price to obtain and maintain the necessary capabilities to meet all the standards on each and every request for services. Recommend reconsideration of the definition of the penalties associated with performance standards to support more competitive pricing.See updated C.16
L.5.155, 91Volume I, Section: Cover/Cover Letter/Contact Page/Executive Summary. It is our assumption that the Cover Letter is separate from the Cover/Contact Page and is not included in the page count as stated in L.5.1. Is this assumption correct? If not, please clarify. The Cover/Cover Letter/Contact Page/Executive Summary is a general title, please follow the instructions in the description column for each section/volume for what is needed.
L.5.155, 91Volume I, Section: Cover/Cover Letter/Contact Page/Executive Summary. It is our assumption that the Executive Summary is separate from the Cover/Contact Page and the Cover Letter, and is not included in the page count. Is this assumption correct? If not, please clarify. Please read thoroughly the descriptions for what is needed in each volume, the Executive Summary is part of the volume.
L.5.155, 91Volume I, Section: Cover/Cover Letter/Contact Page/Executive Summary. RFP states to “Provide a cover/contact page in each volume. The cover/contact page shall include the following information: (list of 5 items).” It is our assumption that the Cover and Contact Page are the same and is not included in the page count. Is this assumption correct? If not please clarify. Please read Section L.5.1, first paragraph.
L.5.358, 94Volume II, Section: Cover/Cover Letter/Contact Page. RFP states to “Provide a cover/contact page in each volume. The cover/contact page shall include the following information: (list of 5 items).” It is our assumption that the Cover and Contact Page are the same. Is this assumption correct? If not please clarify. Please read and follow the instructions and descriptions for what is needed in each section/volume. Obviously, if a contract page is applicable to the Business/Price Volume, therefore if it's not applicable to that Volume it would be the cover page. This question has been asked several times.
L.5.460, 96Volume III, Section: Cover Letter/SF 1449. The RFP identifies 5 items to include in the Cover Letter including the completed SF 1449 and completed Section K Certification and Representations. It is our assumption that the SF 1449 and Section K can be provided as attachments to the Cover Letter. Is this assumption correct? If not, please clarify. Please thoroughly read the instructions. The Business/Price Volume requires a complete copy of the solicitation with all applicable clauses filled in, that include Section K.
C.9.513-14What is the timeframe the Government expects to implement a process for all PPN providers to submit service results directly to FedHealth? The current goal is to begin implementation at the latter part of the base contract year.
C.11.117What is the timeliness requirement from date of appointment to date of delivery of report for IME considering that the physician has to review records and dictate a report which can take longer than filling out forms? Please refer to Section C.10.5
C.11.318What is the timeliness scheduling requirement from date of referral to date of appointment for Psychological Evaluations? What is the timeliness delivery requirement from date of appointment to date of delivery of report for Psychological Evaluations?See updated C.11
