About this file

This is Amendment #1 to a Request for Bid (RFB) issued by the University of Wisconsin-Madison Purchasing Services for Test and Measurement Equipment (Contract 18-5806). The original RFB was issued on October 13, 2017, with an initial due date of November 2, 2017 at 2:00 PM CDT, which was extended by this amendment to the same date. Written questions regarding the bid must be submitted by October 20, 2017, and should be directed to Jeff Whitman, the designated contact at UW-Madison Purchasing Services. The contract term will run from November 1, 2017, through October 31, 2020 (three years), with automatic renewal for two additional one-year terms unless either party provides written notice 60 calendar days prior to expiration. Bidders must submit a complete response including the signed Request for Bid Form, Bidder Response Sheet, specifications and descriptive brochures, itemized bid list, vendor information sheet, and client reference list.

Bid prices and terms shall remain firm for 60 days from the date of bid opening. All bidders must certify compliance with competition and conflict of interest requirements and confirm they are not debarred from federal contracts. Proprietary information within the bid response must be designated separately using form DOA-3027, as the University cannot guarantee confidentiality beyond what is permitted under Wisconsin Public Disclosure Law. Any significant ambiguities, errors, or discrepancies in the RFB document must be reported in writing to the Agent within five business days of the issue date. Supplemental information and revisions will be posted on vendornet.state.wi.us and publicnotices.wi.gov.
