RFP 17-258-SOL-00020; Title: Ambulatory Care; Amendment 0001 – Q&As

Question 1: 8,000 mileage maximum for patient travel reimbursement. Can you explain what this means exactly? See P12 of 58 in Statement of Work

As an example of my concern, Texas HD program has approximately 20 new cases per year on average.

More, if we take new cases from other states. So, if we have 20 new cases and we are assuming 50 miles distance from the HD clinic to their residence (100 miles round trip) at four times a year with no other visits including a NHDP visit in Louisiana, then just with new cases the mileage incurred would be 8,000 miles. This does not include the 110 other visits for existing cases. Even if the 110 patients were within 80 miles roundtrip for a one-time visit, this would add other 8,800 miles. With Texas HD patients, we anticipate a mileage need of 19,000 to 20,000 miles minimum.

Response to Question 1: Section B.2.1 of the Solicitation indicates travel/mileage is a cost reimbursable line item. Therefore, the Contractor may request a contract modification for increased funding once 75% of the allocated resources have been utilized. NHDP will provide a response based on their ability to provide additional funding. FAR Clause 52.232-20 -- Limitation of Cost (APR 1984) is hereby added to the solicitation.

Question 2: The HUB Subcontract plan needs some clarification for us. I have talked to HHS staffers who signed this last time for the Branch but when I asked them this time. They said that the state could not sign for the federal SBA program. I have tried for over a week to get the federal program to respond to my inquiry. See Subcontracting Plan under Ambulatory Care, page 43 of 51

As I understand it, the Small Business Subcontracting Plan only pertains to business or for profit organizations. The TB and Hansen’s Disease Branch and Health Services Region 11 which are a part of Department of State Health Services (DSHS) is a non-profit health department, as is the Center for Infectious Diseases which is a part of the DSHS. Further, the local health departments that we are contracting with are also non-profit and funded per program by DSHS. We are contracting with local health departments, specialized physicians with HD experience.

a. So, do we simply state on the plan: does not apply or NA?

b. If we do need to apply, who will approve? After a year of seeking someone’s help on this, no one has stepped up to answer my questions. Who can you recommend? What is their contact information? Can they respond in a timely manner?

Response to Question 2: A Subcontracting plan is required for any entity that is other than small business; therefore, it includes non-profit organizations. Anita Allen is the Small Business Representative; email Anita.Allen1@cms.hhs.gov, phone 410-786-9319.

Question 3: Pagination and/or appendices or attachments

When the HD Program provides certifications, resumes or budgets, can those be placed in as an attachment or appendices. My understanding is that these would not be included in the 100-page limit for the main part of the proposal.

Response to Question 3: As stated in Section L.1.3 of the Solicitation, Proposals shall not exceed 100 pages, excluding attachments, addendums, resumes and other required appendices.

Question 4: Past Performance References, see pages 45-46 of 51 mailto:Anita.Allen1@cms.hhs.gov

The parameters are:

1) identify three (3) past performance references on same or similar work as required by this solicitation and completed within the past three (3) years.

2) Proposal may include one (1) past performance reference for a subcontractor if a portion of the proposed work is to be subcontracted. The subcontractor’s past performance reference will be included in the maximum number of three (3) past performance references.

For offerors with no past performance history, the proposal shall state “No Past Performance History Available.”

Our question is we have previously used our local health department subcontractors. As I understand it, we can only use one of these. We also have a non-subcontractor who has worked with us for many years.

Can this DSHS hospital provide one of these? They are not subcontractors. We have vendors who provide services, are these a possibility?

We have worked with NHDP/HRSA and the CDC for probably over 50 years. Can the NHDP team provide a reference? The CDC?

Response to Question 4: As stated in Section L.6 of the Solicitation, The proposal shall identify three

(3) past performance references on same or similar work as required by this solicitation and completed within the past three (3) years. A list shall be included that cites these references and explains the relevance of each contract to this solicitation. The proposal may include one (1) past performance reference for a subcontractor if a portion of the proposed work is to be subcontracted. If a past performance reference for a subcontractor is used, the proposal shall state the subcontractor’s name, project manager’s name, street address, phone number, and e-mail address. The subcontractor’s past performance reference will be included in the maximum number of three (3) past performance references.

In addition, offerors are responsible for ensuring that the provided references submit the past performance questionnaire form (Attachment C) as part of their proposal by the solicitation closing date and time.

Question 5: On the RFP under Section K certifications and statements on pages 26 through 33 of 51.

a. Do we place these in verbatim? That is what it looks like.

b. Where do these go? In an attachment? In the business proposal?

Response to Question 5:

a. Section K is a fill-in for the organization submitting a proposal to certify.

b. As stated on Page 34 of the solicitation, Representations and Certifications are to be included in the Business Proposal.

Question 6: In the Cost and Pricing Data section pages, 42 through 43 of 51; I have the following series of questions.

a. What is your definition of direct labor? Does it mean just the HD program staff? Or, does it include contracted health departments. Or, is it broader?

b. We can supply direct payroll records for the DSHS HD Program staff that are directly funded.

We can mostly likely get similar records from TCID and HSR 11 as they are part of DSHS.

To acquire payroll records for subcontracted staff at any of the non-DSHS local health departments is problematic. We do have budgets from these local health departments. Will these do? Please clarify.

Response to Question 6:

a. Direct Labor can be defined as employees or workers who are directly involved in the production of goods or services.

b. Yes.

Question 7: As I understand it, the proposal once complete is to be sent to: Lohmeier, Jennifer (HRSA) <JLohmeier@hrsa.gov>. Our grants people say that we normally submitted it in FedBiz which is on the NHDP website. As I understand the RFP language that is not correct for this RFP due on November 1, 2017.

Response to Question 7: As stated in Section L.1.3 of the Solicitation, You must submit your proposal as stated below to be considered responsive to this RFP (no exceptions).

The solicitation shall be signed by an official authorized to bind the Offerors organization and shall be submitted with the proposal by email by the date and time specified on page 1 of the solicitation to Jennifer Lohmeier at jlohmeier@hrsa.gov.

Question 8: Due to the complexity of the requirement, a two-week extension is requested on the due date for proposals.

Response to Question 8: HRSA will grant an extension through 4PM EST, November 10, 2017.
