PROPOSAL SUBMISSION INSTRUCTIONS
NOTE: Proposals that fail to follow the quotation submission requirements may be determined unacceptable and may be rejected from further consideration.
Government intends to make award without discussions with offerors. Therefore, the Offerors’ initial proposal should contain the offeror’s best terms from a price and technical standpoint.
The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary.
CONTENT OF THE TECHNICAL QUOTATION
All quotes shall consist of sections addressing the following:
1. TECHNICAL CAPABILITY
The Offeror’s technical proposal to the Government must demonstrate the Offeror’s technical understanding of the Performance Work Statement (PWS). The proposal should demonstrate the company’s understanding and capability in a concise, logical manner and should not contain superfluous material not directly related to the solicitation. All information must be presented in sufficient depth to allow the Government to make a comprehensive evaluation of the Offeror’s understanding of the Performance Work Statement and the Offeror’s capability for successful performance to meet these requirements. The Technical Capability shall include the following:
Element 1. The proposal will provide a detailed methodology to be used to achieve the objectives described in the PWS Section 3.0.
Element 2. The proposal shall identify the solutions to any potential problems that could be encountered in implementing its approach. Examples of specific potential problems and solutions must be identified.
Element 3. The proposal shall identify the offeror’s internal procedures for monitoring the progress of assigned tasks and ensuring the quality of services.
Element 4. The proposal shall provide information on the offeror’s plan to build capability in quality improvement science skills and knowledge to develop high-performing, innovative health care teams, integrating improvement products and tools using a change package that provides a vehicle for the dissemination of improvement principles, practices and strategies, providing technical assistance and training for the IPC program and to provide the expertise needed by IHS ambulatory care facilities to achieve PCMH recognition and designation.
Technical Capability is limited to twelve (12) pages.
2. KEY PERSONNEL
The Offeror shall submit resumes for all Key Personnel to be utilized in the performance of these requirements. The contractor is to determine and provide a list of 2 to 5 key personnel and their proposed role leading significant areas of the operation. Offeror shall clearly note which requirements section that the individual being proposed would work in. Also, the Offeror shall certify that the information on each key personnel submitted is true and complete.
Element 1. At minimum, resumes for the required key personnel shall reflect experience of the principal participants with emphasis on knowledge, skills and abilities with the IPC Program to support the requirement. At minimum, the key personnel team shall be qualified to perform the work defined under PWS Section 3.0. Each resume shall be limited to three (3) pages and shall provide the following information:
a. Name, the currency, educational qualification, quality and depth of relevant experience of the proposed key personnel.
Element 2. Relative experience and capability of the project manager to function in an environment with the complexity, diversity and inter-dependent communication and collaboration and to successfully supervise and coordinate the workforce.
Individuals in the role of project manager must possess:
a. A current Project Management Professional (PMP) certification.
b. A minimum of 3 years’ experience in use of Electronic Health Record, technology, and quality improvement processes and training, data, and clinical quality, value based payment and technical assistance to teams pursuing PCMH primary care transformation and designation.
Element 3. Management Approach/Enhanced support – Ability of the contractor to provide enhanced support using key methods, techniques, processes, procedures, etc. to provide additional support to monitor and measure complex processes, identify and mitigate potential risks, ensure the accuracy and timeliness of deliverables and communicate effectively to meet the needs of the IHS customers and Programs.
3. RELEVANT PAST PERFORMANCE
The Offeror will be evaluated to assess the demonstrated quality of performance on similar work, to include the ability to control the quality and cost of work, timeliness of performance, and effectiveness at accomplishing the goals of previous work.
Element 1. Offerors are required to provide relevant experiences for no less than three (3) and at most five (5) contracts performed during the past seven years or currently in process for the relevant services as are being solicited under this solicitation. The Offeror’s relevant past performance information must include the following; Contract Number(s);
type of contract; period of performance; total original, present, or final contract dollar value; and specify whether the Vendor was the prime Contractor or the subcontractor.
Relevant contracts are those contracts that are of similar scope, magnitude, nature, and work. A representative of IHS may contact the offeror’s previous customer(s) for a phone interview to help rate the vendor’s performance.
The Government may supplement the information offerors provide with performance information it may obtain from any source including its own experience with the vendor.
Element 2. All offerors shall submit a short narrative that describes their experience in working with communities and organizations that represent a broad range of American Indian and Alaska Native communities. The offeror will be evaluated based on the number of years and depth of experience/knowledge working with American Indian and Alaskan Native communities. Also, offeror shall demonstrate their understanding of IHS mission, goals and customers.
Element 3. The Agency will conduct Relevant Past Performance evaluation via. Past Performance Information Retrieval System (PPIRS). PPIRS records for the identified contract under element 1 will be assessed and other records within PPIRS may be considered. No submission is required for this element.
This submission shall be limited to 8 pages
PROPOSAL SECTION TEMPLATE
The Section Template below shall be used for the formation of the Offeror’s proposal. Offerors are allowed to add additional information - before and after the mandated sections below - to address the other aspects such as introduction, company background, closing statement, etc.
However, the following format must be followed for the information addressing the technical elements in the Section L.1.

EVALUATION FACTORS FOR AWARD
(a) The Government will award a Firm Fixed Price contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
(1) Technical Capability
(2) Key Personnel
(3) Relevant Past Performance
(4) Price
(b) The Government will conduct a best value evaluation to determine a single Indian Small Business Economic Enterprise (ISBEE) offeror for this award. In doing so the Government may award to other than the lowest priced proposal or other than the highest technically rated proposal. It is anticipated that one award will be made. All proposals shall be evaluated based on the following:
PAST PERFORMANCE
The Federal Government uses the Contractor Performance Assessment System (CPARS) to document Contractor performance information that is required by Federal Regulations. Past performance information entered to CPARS is available to Federal agencies through a separate system called the Past Performance Information Retrieval System (PPIRS) for source selection purposes. To determine how well the Offeror has performed in the past, the Government is using past performance rating information available in PPIRS as well as information provided through questionnaires and direct communication with references.
Relevancy Relevancy will be evaluated based on the size, scope and complexity of the Offeror’s past performance and how it compares to the services specified in Section C of this RFP. As the relevancy of past performance increases the significance of the reference also increases.
PRICE EVALUATION
The total price of each year will be added up to arrive at the total evaluation price.
RELATIVE IMPORTANCE OF EVALUATION CRITERIA
All evaluation factors other than cost or price, when combined, are more important than cost or price. All non-price factors are of equal importance. Selection of the firm to perform this Contract will be based on IHS’ assessment of the best value to the Government.
FAR 52.217-5 EVALUATION OF OPTIONS (JUL 1990)
Except when it is determined in accordance with FAR 17.206(b) not to be in the Government’s best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government is considering the evaluation to include extensions of services under the same terms and at the same prices as proposed by the contractor for the establishment of the award. Evaluation of options will not obligate the Government to exercise the option(s).
EXCEPTIONS TAKEN TO ANY TERMS AND CONDITIONS STATED IN THE RFP
Complete rationale, justification, and cost impact must be included. The Government advises Offerors that it intends to evaluate proposals and award a contract without discussions.
