SECTION 2 – DESCRIPTION/SPECIFICATIONS/WORK STATEMENT

2.1 OBJECTIVES AND BACKGROUND

This purchase order would develop a model standard continuum of care for public and private systems serving adults with SMI. SAMHSA is seeking guidance on the adoption of a continuum of care to be used in its contracting, data requirements and technical assistance to state and local behavioral health authorities. SAMHSA has a long history of leadership in the promulgation of evidence based practices, operating principles, and recovery oriented outcomes. Given the significant changes over the past several years due to the Affordable Care Act, changes in funding methodologies, ongoing development of new evidence based practices, and the proliferation of new models and methods of services delivery (e.g.

telehealth, peer services and supports, further integration of behavioral health with overall health), SAMHSA has determined that the current continuum of care approach must be critically examined and updated.

In the summer of 2017, SAMHSA convened an expert panel charged with making recommendations on the need for and the process of developing a continuum of care for SMI. The outcome of this expert panel is a document that provides high level and summary recommendations. SAMHSA’s overall objective is for this contract to develop a comprehensive strategy for the adoption, dissemination and implementation of a model continuum of care for SMI.

2.2 SCOPE AND PURPOSE:

The contractor shall develop a white paper that presents a model standard continuum of care for the prevention, treatment and recovery for Serious Mental Illness (SMI). The contractor shall initially draw from the results of an expert panel, conducted by SAMHSA/CMHS during the summer of 2017. In addition, the white paper shall include an assessment of current literature on the topic of SMI continuum of care; best practices on the identification, implementation and analysis of such approaches; and rationale for the selection of specific service models. The contractor shall conduct, via electronic communication, a set of interviews with continuum of care end users such as state and local providers, peer organizations and advocacy groups. The purpose of these interviews is to inform the contractor about the feasibility of adopting the recommended continuum of care, the personnel, clinical, and funding challenges of adoption, and any refinements. The white paper should identify challenges, needs, lessons learned and recommendations regarding the adoption of standard continuum of care across a diverse set of providers, payers and governmental authorities such as SAMHSA, local state mental health authorities and related authorities such as state Medicaid authority, local MCO or ASO entities. Lastly, the contractor shall develop and present a set of PowerPoint slides via SAMHSA’s web platform that summarizes the continuum of care project.

2.3 STATEMENT OF WORK:

A. General Requirements

1. Independently, and not as an agent of the Government, the contractor shall exert its best efforts and furnish the necessary personnel, facilities, equipment, materials, and supplies, and perform the work set forth below.

Task 3: Develop and submit a summary to the COR of Task 2, including, at minimum, summaries of the expert panel/literature review; directions in thinking about continuum of care, challenges, needs, and lessons learned throughout this review; and recommendations continued discussion of these issues with a group of subject matter experts (SMEs).

Task 4: Arrange and undertake in-depth interviews with a variety of SMEs (up to 9 individuals) that are and could be directly impacted by the adoption of a model continuum of care. The interviews may be conducted individually or as a convened group(s), via teleconference, a number of small groups, etc. The approach(s) will be determined in consultation with the COR. The SMEs may include state mental health commissioners, representatives of peer, family and advocacy groups, professionals working in the field of adult SMI services, relevant related governmental entities such as CMS, DOL, and USDOJ as well as various non-governmental organizations such as NASMHPD, MHA, and NAMI. The SMEs will be identified in consultation with SAMHSA and final SMEs selected must have COR approval prior to conducting the interview.

Task 5: Develop and submit to the COR for review and approval a summary of Task 4, including, at a minimum, brief highlights of each interview and a draft outline for the white paper submission. Receive and incorporate the comments and revisions on the summary and draft outline from the COR and resubmit. If additional feedback is given, this process will be repeated until COR approval is obtained.

Task 6: Develop and submit a draft white paper that includes, at a minimum, a summary of Tasks 2 and

4 with a review of the literature on a Continuum of Care, current thinking on the direction/goals/challenges regarding Continuum of Care dissemination, adoption and monitoring; a fairly in-depth presentation of a recommended set of Behavioral Health Adult Standards of Care. If additional feedback is given, this process will be repeated until COR approval is obtained.

The contractor will make the final white paper 508-compliant for posting on the SAMHSA web site. For all documents developed through this contract, the format and content will follow the HHS Guide to
