A. Phamatech Inc.

Q. What does your agency currently pay for the cups in the bid?

A. $2.33 per unit.

Q. Will your agency allow for multiple, alternative bids (for example: primary bid, alternative bid 1, alternative bid 2, etc…)?

A. No.

Q. Will your agency allow bid responses that include devices that deviate slightly from the requested configuration and/or drug cutoff levels?

A. No.

QUESTIONS AND ANSWERS (09/20/16)

Q. What do you guys pay for shipping?

A. .25 per unit

Q. Shall I bid both cups and you can review both and make a determination? Would they need to be 2 separate bids?

A. Page 3 of the solicitation indicates: "Samples are not required to include the exact panel or cut offs, but should be similar to the product that would be provided if the vendor is awarded." Therefore, (samples) with exact cut off levels as prescribed in the RFQ are not required, but the cup should comply with the physical requirements as specified in page 2 of the RFQ. If a vendor is proposing a selection of different cups for this solicitation, separate "bids" for each cup is required.
