QUESTIONS AND ANSWERS (08/05/15)

Q. It is my understanding that the published cut off for AMP is 1000 ng/ml and Benzoylecgonine (COC) is 300 ng/mg. Can you please clear these cut off limits?

A. The national urinalysis contract references that all urinalysis testing must be done at the same levels of sensitivity in the national contract. Therefore, the “cut off limits” as noted in the RFQ, are consistent with the cut off levels as prescribed in the national contract.

Q. What is the price you were currently paying?

A. For FY 2015', 12,000 units (NIDT’s) were purchased for a grand total of $31,800.00 from Phamatech Inc.

Q. Please clarify the drug menu your office specifically desires with this solicitation. The RFQ specifies a 7 panel, but only 6 are listed on the specs. If you could reply with an itemized list of the drugs your office needs for this device, I would appreciate it.

A. Yes, it should only be six panel. This RFQ will be amended and posted again.

Q. I am curious as to what the specs, "AMPIMETH (500ng) means. Are you under the assumption that METH will cross‐react with AMP at the same cutoff levels?

A. The Request for Quotation has been amended to include separate test panels for Amphetamine and Methamphetamine both with cut off levels at (500 ng/ml). The test cup will include seven panels.

Q. Is that price of the Non‐Instrumented Drug Test Cup the shipping included in the total cost you provided?

Q. What are they currently charging for Shipping/Freight?

A. In FY 2015, 12,000 cups were purchased at a total cost of$31,800. This cost included shipping.

Q. Will the Award money be given before the "Testing Cups" are to be received at the designated sites?

A. Award money will be distributed after the "Testing Cups" have been received and invoiced.

Q. Referencing page 4 of the solicitation, does our response require the separate break‐down of freight cost to each location ‐ or can we respond with one total cost of the product (including freight) or each location?

A. No. Do not specify shipping COSI to each de livery point. Shipping/Freight cost should be indicated as a single unit price separate from the unit price for the NIDT's, as indicated on page 4 of the RFQ.

Q. On August 11, 2015, it was brought to my attention that you have changed again back to a 7 Panel test with the following cut‐offs: AMP500/MDMA500/COC150/THC50/OP1300/0XY100/MAMP500 is that correct for now?

A. Yes.

Q. The reason is we made 3 samples that US Probation Northern District of Georgia is requesting in the RFQ for the 6 Panel AMP/500/COC/150/MDMA500/THC/OPI300/0XYlOO already? Will you still accept those?

A. No. Sample units must comply with the current RFQ as amended on August 11, 2015.

The Response deadline has been extended to Monday, August 31, 2015 at 12:00 p.m. (EST).

Q. So if you can't provide the breakdown between how much US Probation is currently paying per unit, and what Phamatech is charging for shipping/freight, why do you have on the RFQ Quote sheet Page #4 two (2) Line Items?

A. Please follow the Quote Sheet for RFQ 16‐0 IGANP to include price per unit for NIDT and price for Shipping/Freight.

Note: Shipping/Freight cost for NIDT's ‐ FY 2015 purchased through Phamatech was $0.25 per unit.

[NIDT: $2.40 per unit + $.025 Shipping/Freight per unit (Total $2.65 per cup (unit) x 12,000 = $31,800.00]

Q. From you previous answers FY 2015, 12000 cups were purchased at a total cost of $31, 800. The cost of shipping is included. So that breaks down to $2.65 per cup. So if we put down 0 the Quote Sheet say for example, $2.55 for the cup, and write in Included for Shipping/Freight would you accept that?

A. No. The Quote Sheet requests separate itemized pricing for both the Non‐Instrumented Drug Testing Cups (NIDT) and Shipping/Freight.

Q. How many test cups are you requesting?

A. 16,000 units

Q. Do you have an award date for this RFP?

A. No. Product evaluation will begin subsequent to the response deadline date of August 31, 2015.

Q. Under requirements for the non‐instrumented drug testing cups: *Cups must be FDA 510(k) cleared. To your response on Amendment #1 Answer to #1 that you use the national urinalysis contract cut‐offs. Since AMP 500 & COC 150 cut‐off is a lower cut‐off then AMP 1000 & COC

300. All the rest of the cut‐offs with this cup are FDA cleared. Will you accept a bid to be sent in?

A. According to the RFQ, samples sent along with a quotation are not required to include the exact panel or cut‐offs, but should be similar to the product that would be provided if the vendor is awarded. If awarded, the vendor must be able to produce a cup that meets the requirements of the RFQ regarding the specified cut‐off levels, and that the cup is 510(k) cleared.

Q. Will you accept a Cup that is custom made with those lower cut‐offs but aren't FDA 510(k) yet? All the rest of the cut‐offs are FDA 510(k) approved?

A. Per the RFQ, the cup purchased must be 510(k) cleared. For evaluation purposes only, samples are not required to include the exact panel or cut‐offs, but should be similar to the product that would be provided if the vendor is awarded.

Q. Are you interested in a Cup that is much more Photo friendly since your current cup is more round?

A. The cup needs to meet the requirements of the RFQ.
