In accordance with FAR Part 12, the FAR 52.212-3, Offeror Representations and Certifications-Commercial Items should be used. Block 27A of the SF 1449 is checked which incorporates by reference FAR 52.212-3. Assumption is these are outside of any page limits. Please include this as part of the Volume IV - Model Task Order. A section has been added as part of the Instructions to Offerors for your reference. Page limits do not apply.
(2) It is our assumption that provision of a copy of the teaming/partnership agreement does not apply to each entity within the nationwide provider network. Provider agreements are considered confidential and are not provided with proposal submissions. In lieu of actual agreements, provision of the standard language of the agreement is typically submitted. Is this approach acceptable? If not, please clarify.Correct. This is an acceptable approach.

The Table I-1 outlines 5 components which may or may not be separate files or separate sections of a single file. Section 3 instructions identify this volume as only the OCI component. All five (5) componets are required and it does not matter how they are received (separate or altogother). Revised Section 3 to be more specific.

Sections 1.3.2 and 2.2.63, 6Section 1.3.2 states that cost or pricing information is to only be addressed in price (Volume III) and task order documentation (Volume IV) volumes. Section 2.2.6 instructs that the small business subcontracting plan is provided IAW FAR Part 19.704 which requires inclusion of the total dollars planned to be subcontracted and a statement of the total dollars planned to be subcontracted to small business concerns. If not, please clarify.Updated to include small business subcontracting percentages only. Please only include pricing information in Volume III - Price.
Table I-1 and Section 53, 9-10Please clarify instructions on provision of the model task order documentation. The Table I-1 shows it as part of Volume II and Section 5 identifies it as its own Volume IV, including the KMPL and ARQ.Updated Table I-1 to include revised Volume Organization.

Is this assumption correct? If not, for these purposes recommend that the network be listed as a single entity.Network can be listed as a single entity, however, offeror's should include a list of all probable Subcontractors.

If not, for these purposes recommend that the network be listed as a single entity. The Government will need the specific locations of provider locations. Removed percentage by location requirement.
Section 6.1.310Please clarify the instructions for completion of Attachment 7, KMPL. The project manager is the only position designated as key personnel. It is our assumption that only the direct project team staff is to be listed and not personnel in corporate departments or others who indirectly support the contract, and not the nationwide provider network. Evaluation Factors for Award16-OctIt is our assumption that Volume II and Volume IV are not evaluated. Is this assumption correct? If not, please clarify.Volume II - OCI, Volume III - Price, and Volume IV - Model Task Order will be evaluated for acceptability but not will not be scored for a color rating.
Generaln/aThe Attachment 2 instructions state to “include a signed copy of the Model Task Order and Sections A through J” and to complete and sign the SF 1449. Attachment 3 does not contain a Section A, signature block, or a SF 1449. Request clarification and/or a revised Attachment 3 in order to comply.Section A is the SF-1449. No need to sign the Model Task Order if the SF-1449 is signed. Will update Instructions to Offerors.
Section B.15-MarRequest any revisions made to Attachment 6, Pricing Model and the priced items be reflected in the CLINs in the tables. All changes have been identified in Attachment 6 and highlighted in Orange.
Section B.15-MarThe section contains CLIN X009, Hepatitis B Vaccinations – At Risk Employees and CLIN X012, A La Carte Services (see pricing model). Attachment 6, Pricing Model has the Hepatitis B Vaccinations – At Risk Employees listed as an A La Carte Service.
(1) Please clarify if the “Hepatitis B Vaccinations – At Risk Employees” is a separate CLIN or if it is a part of the A La Carte Services.Removed "Hepatitis B Vaccinations - At Risk Employee". There is now only one line for Hepatitis B Vaccine for a La Carte proposal.
(2) If it is a separate CLIN, it is our assumption that the price entered in the Model Task Order for CLIN X012 should not contain the price for this vaccination. Is this assumption correct? If not, please clarify how to determine the firm fixed price for both CLINs X009 and X012Model Task Order CLINs have been updated to remove the "Hepatitis B Vaccinations for At Risk Employees". Hepatitis B is now an a La Carter item under CLIN X011.
Section B.15-MarThe section contains CLIN X012, A La Carte Services (see pricing model) where the specific items are listed in Attachment 6, Pricing Model. Within Attachment 6 under A La Carte Services there are a total of 29 items and 8 have volume estimate per year values and 21 have unknown volume estimates per year. It is our assumption that the firm fixed price entry in the Model Task Order for CLIN X012 only uses the 8 items that have a volume estimate to calculate a total for the entryPlease propose a price for all items under A La Carte whether they have an estimate or not. We are expecting that all services will be utilized but could not provide estimated volumes for all services. This CLIN will likely have a set of Sub CLINs to identify these individual items.
Section F.27The section identifies the primary place of performance as FBI locations and the work will be performed at contractor/subcontractor/provider locations nationwide in support of the FBI locations. Request revision to the section.Revised section F.2 to include updated place of performance language.

Generaln/aLanguage Specialists Audiogram Only are included in Attachment 6 as a priced item and are not included in the SOW. Please clarify any additional requirements for these audiograms and provide associated form.SOW has been updated to include this service.
Generaln/aDuring the COVID-19 national emergency many providers are not performing certain services/procedures due to safety concerns (e.g. audiogram and PFT). During this time, would the Government accept a Whisper Test in lieu of the audio exam?Yes, with the exception of Language Specialists. These employees are monitored for Standard Threshold Shifts which cannot be determined by a Whisper Test. This is subject to change.
Sections C.4 and C.62,3The RFP states the second appointment is used for an overview of lab results and a PPD read. Can these items be completed virtually in lieu of an in-person appointment (e.g. telephonic, telehealth, etc.)?All PPD must be read in person whether that is a scheduled appointment or "drop-in" is fine. All normal/abnormal lab results can be communicated electronically or telephonically and must be documented on a completed report to the FBI.
Section C.84It is our assumption that the time for the background investigation process to be completed by the Government for the provider and at least one designated employee is excluded from the 60 days and is considered included in the “time delays.” Is this assumption correct? If not, request a much longer duration for establishing new provider location, such as 120 days.Yes, any delays caused by background investigations would be an excusable delay. This transition period has been updated to 90 days.
Section C.84It is our assumption that a new location would be outside of the Attachment 9, Locations. Is this assumption correct? If not, please clarify.Yes, this assumption is correct.
Section C.114The RFP states that outreach services may be required at FBI facilities which include flu vaccinations and other services on the contract. Typically an outreach fee is used for these types of outreach services in addition to the prices for the individual services. Request addition of an outreach fee to the Attachment 6, Pricing Model.See Attachment 6 for vaccination administration only proposal spot. An outreach fee is not allowable for this contract. This scenario would only be for rare situations in which a field office could reach out to providers on this contract to assist with administering flu or other vaccines at a field office location.
Section C.125Please provide detail on what information is required on the monthly quality check report. Is it the same as the Monthly Self - Assessment Report (MSAR)?The Contractor provided monthly quality check is the MSAR. The FBI will also perform a quality check for accuracy, completeness, and legibility.
Section C.125The last line of Section C.12 states “Additional requirements and / or instructions for each line item may be included in the individual referral service / work order requirements, and / or dictated by the FBI.” It is our assumption that any “additional requirements and/or instructions” are further clarification of the contract and not new services outside of scope or priced items. Is this assumption correct? If not, please clarify. This sentence has been deleted from the SOW.
Section C.135The RFP language infers that service / work order requirements for Specialized Services could be outside of the scope and requirements in the SOW. Please clarify the intent of Specialized Services. It is our assumption if there are material changes and/or new services that a contract modification will be executed. Is this assumption correct? If not, please clarify.Yes, this is correct. If there are any material changes, a bi-lateral contract modification would be issued.
Section C.177-JunRequest clarification on the scheduling process and the role of the contractor, provider, and FBI POC. Clear roles and responsibilities and lines of communication are required to ensure proper execution of scheduling process and contractor capture of required information in order to manage performance. It is our assumption that the term “Provider” is intended to be “Contractor.” Is this assumption correct? If not, please clarify.Yes, this is correct. The Contractor is responsible for reaching out to the Employee/Applicant once a work order is received to schedule them with the individual Provider. Please refer to QASP for quality standards regarding timeliness.
Section C.177Request additional information on rescheduled appointments such as how many are permitted by the applicant or employee and how are the scheduling timelines impacted (e.g. restart with a reschedule process).Contractor has three attempts to contact employee/applicant per section C.17. When the limit is reached, the FBI is notified.

Section C.197Request clarification and revision of this Section. Content contains language on scheduling requirements and report submission requirements for expedited orders.

Per Section C.21, Expedited Referral Report Submission the order shall be completed within 5 business days from the final appointment date. This also aligns with Attachment 5, QASP and its Attachment 1, Performance Requirements Summary (PRS). Updated this section. Please see revised SOW and QASP.

Attachment 5, QASP and its Attachment 1, Performance Requirements Summary (PRS) states that expedited referrals are to be scheduled within 48 hours of order receipt for an appointment within 5 business days of the notification (i.e. appointment to occur within 5 business days of order receipt). Updated this section. Please see revised SOW and QASP.

Section C.19 states that “shall schedule and complete” within 5 business days which is not aligned with the other content.Updated this section. Please see revised SOW and QASP.
Sections C.19 and C.218-JulThe sections reference a Section C.15.4, Expedited Service Fee and the SOW does not contain this section. Please clarify and provide the referenced content. Please provide more information on the Expedited Service Fee including if the fee value is set by FBI and its value or should the fee be included in the Pricing Model as an Offeror’s bid amount.Updated SOW to remove refernce to C.15.4. Attachment 6 has been updated for Offeror's to propose an expedited service fee.
Section C.197The section references a Section C.10.2, Standard Referral Service Completion and the SOW does not contain this section. C.20 is the correct reference.
Section C.208Request a limit to the disincentive of $25 per business day such as 50% of the price of the item. The relative value or the priced item and the $25 varies by the service.Concur, with an up to 50% disincentive. Updated SOW.
Section C.218The section references a Section C.10.5, Standard Referral Report Submission and the SOW does not contain this section. It is our assumption that Section C.20, Standard Referral Report Submission is the correct reference. Is this assumption correct? If not, please clarify.Updated SOW to remove refernce to C.10.5. C.20 is the correct reference.
Section C.228The section states that a disincentive is assessed per missing item. It is our assumption that this disincentive is applied only once per exam form (e.g. missing signature and section of form not completed incurs $10 disincentive). Is this assumption correct? If not, request a limit such as 50% of the price of the item.Disincentive is based on each error found within each exam. Agree to limit disinceintive to no more than 50% of service cost.
Section C.238The RFP states that all results/required information will be digitally submitted. Please clarify if there are any special requirements regarding original hardcopy forms (i.e. consent form).Hard copies are not to be submitted to the FBI. (See section C.53) Hardcopy forms shall be destroyed 60 days after the acceptance of the exam packet by the FBI. Updated section.
Sections C.23 and C.269Sections C.23 and C.26 state “The Provider shall not address abnormal values, with the Employee/Applicant, except for critical values or findings deemed by the examining Practitioner as life threatening or emergent.”This should only apply to applicants. We'd want the examining provider to address abnormal labs with employees and refer them to PCP.
Section C.4 identifies the use of a second appointment to allow for overview of lab results. Please clarify what results are to be discussed. The second appointment should only be utilized for PPD reads when required.
Section C.259RFP states “The Contractor shall include the original test or submit an electronic copy of the EKG with interpretation by the evaluating Provider along with the Provider’s signature.” Will the agency accept the provider’s initials in lieu of a signature? Do the PFT tracings and lab results also require provider’s signature/initials?The items mentioned should be included as part of the completed report to the FBI. A provider's signature will be required.
Section C.2510Section identifies additional tests and “Blood group and type” is not included in the A La Carte items in Attachment 6. Request this item be added to Attachment 6 to support pricing.Blood Group and Type has been added to the A La Carte section of Attachment 6 - Pricing.

Request that Hepatitis A Series and Hepatitis B Series be added to Attachment 6 to support pricing.Updated Attachment 6 - Pricing Model to include Hepatitis A and B as seperately priced services under "A La Carte"
Section C.2812-NovRequest clarification of content in Section C.28, OSHA Medical Surveillance Examinations and priced items in Attachment 6.
Section C.28 lists four components to the OSHA Medical Surveillance Examinations and these exams are not included in Attachment 6. Attachment 6 does include Hazardous Materials Response Examinations with FD-900 review and FD-899 completion and PFT and Respiratory Protection Examinations with FD 900 and FD 899 forms.Blood Lead and ZPP are listed under A La Carte since they may not be included with every exam. Removed PFT from HAZMAT pricing and included RBC and Plasma Cholinesterase as part of the exam.
Section C.3413Per the CDC in January 2020, Sanofi Pasteur, the manufacturer of the only yellow fever vaccine (YF-Vax) licensed in the United States, has announced a total depletion of YF-Vax as they transition to a new production facility. Sanofi Pasteur applied and received approval from the US Food and Drug Administration (FDA) to make another yellow fever vaccine available in the United States under an investigational new drug (IND) program.* Manufactured by Sanofi Pasteur in France, this vaccine, Stamaril, is registered and distributed in more than 70 countries. It is comparable in safety and efficacy to YF-Vax. Due to the current situation and unavailability of the Yellow Fever vaccine, it is requested that Stamaril may also be used. Is this acceptable?Stamaril may be used until such time yellow fever or alternate becomes available.
Section C.3513The RFP states that local FBI FOs will be directly responsible for the administration fee, which shall not be billed under this contract. Request clarification on the process to bill the FOs. Is the administration fee established per vaccination event based on number of attendees or should it be priced on the contract for use by the FOs?Updated this section. Please see revised SOW. Request a copy of the table be provided. There isn't an attached weight table. The FD-300 was retired when the FD-1093 was created. Provider should go by standard AMA height and weight tables.
FD109314#26 – Is there a separate FD1093 form for applicants? Providers are not to determine “fitness for duty” for applicants as this is determined by the FBIHQ.Updated Attachment 10 to include the form for Applicant (1093a)
Generaln/aIt is our assumption that the PDF file format of the Attachment can be converted to MS Word file format for ease of completion and submitted in this file format. Is this assumption correct? If not, please clarify. This is acceptable as long as it is convnerted back to PDF prior to submitting to FBI.
Attachment 10 – Forms9Attachment 10 includes form FD-967 - New Agent Physical Activities, which includes references to "Personal Weapon Attacks," "Survival Techniques," "Handcuffing," "Firearms," and a number of other non-medial assessments that would not be typical knowledge for a nationwide provider network. Would the contractor be expected to facilitate this type of evaluation?Based on the medical assessment the provider would need to determine if the applicant can safely perform these tasks. A list of essential duties will be provided at time of award.
Attachment 2 - Instructions to Offerors 1.3 ORGANIZATION/NUMBER OF COPIES/PAGE LIMITS3Table I-1: Proposal Organization indicates that the Model Task Order should be submitted with Volume II and Section 5.0 Volume IV – Task Order Documentation indicates it is a separate volume. Can the Government clarify the submission requirements for the Model Task Order?Revised Table I-1. Volume IV should be for Model Task Order Documentation
Attachment 2 - Instructions to Offerors 1.3 ORGANIZATION/NUMBER OF COPIES/PAGE LIMITS3Given that offerors are required to address the 21 page SOW and 12 page QASP, will the Government consider raising the page limits for the Technical Capability and Management Approach sections to 25 pages each? These have been updated to 17 each.
Attachment 2 - Instructions to Offerors 5.1.1. Section A – Solicitation/Task Order Form9Are offerors required to submit all 11 pages of the SF1449 or just the first two pages? Please submit all pages of the SF-1449 as part of your submission.

These instructions seem to apply to the SF33, not the SF1449. Can the Government please clarify which boxes are required?Correct. This has been updated to reflect blocks 17a and 30a, 30b, and 30c instead.
Attachment 5 QASP, Desired Outcome 58The Performance Standard, AQL, and Monitoring Method are missing for this measure. Please advise. This has been updated. See revised QASP.
Attachment 6 Pricing ModelEstimated Volume TabThe estimated volume tab indicates 225 Federal Aviation Administration Exams are needed, but no location provided. What locations should offerors associate with these exams? Please expect FAA exams at every Field Office location.
Attachment 6 Pricing ModelEstimated Volume TabTotal Travel Medicine and/or Immunizations. Attachment 4 Statement of Work C.34
Statement of Work C.74-MarDriving distances. Would the Government consider extending the distance requirements to reflect other similar Government programs? For example, in similar programs the VA and DoD allow up to 50 miles and the CDC allows 30 miles for urban and 75 miles for rural. Extending the distance would also better allow offerors to meet board certification, specialty, and JCAHO requirements.Exceptions to distance requirements must be approved on a case-by-case basis. SOW requirements apply.
Statement of Work C.94New Provider Locations.Yes, a start-up call will be provided at the start of the contract with follow-ups as necessary. If it is an established location, offeror can provide training documentation to new providers. If it is a new location, the FBI and Contractor would hold a joint startup/telecon.
Is it the intent of the Government to have a startup/telecom with all providers at the start of contract performance, or only as new locations are added? Alternatively, can an offeror provide documentation to show that each individual provider received training?
Statement of Work C.125“The Contractor will provide a monthly, Quality check report to the FBI COR with every invoice.”See section C.17, C.41 and C.42 for requirements.
Can the Government provide the report specifications document which details what specific information/data is to be included in the monthly Quality Check Report?
Statement of Work C.146“The Contractor shall maintain a PPN and provide FBI with a comprehensive list of all active Provider locations with associated Practitioners and service capabilities identified. The initial report will be provided to the COR at the time of award and any time upon request.”This list must be provided prior to the start of contract performance from the awardee in order to complete security screening and then as needed as new locations are added.
In order to allow offerors the time needed to incorporate required flow downs from the signed prime contract to provider network contracts, would the Government change the due date for the provider locations to coincide with the start of contract performance?
Statement of Work C.22The RFP notes outreach services may be provided using a mobile van or traveling medical staff. What volume of outreach services should offerors use? Or would these methods only be needed in the event services are requested and a provider location is unavailable? These services are expected to be an outlier and only needed in the event local FBI locations specifically need certain services. Examples may include annual flu vaccine administration (FBI provided), COVID-testing, or COVID vaccine immunization (when available).
Statement of Work C.228Would the Government consider removing this disincentive? If no, would the Government consider limiting to total liability of the disincentive to the total billable charge for the service? Also, would the Government be willing to consider an incentive for meeting a performance requirement for completeness of exam components? The FBI has revised this to including limiting disincentive to 50% of the total cost of services.
Statement of Work C.305“No paper originals or copies are to be permanently stored with the Contractor or at any provider locations. The use of electronic medical records at a Contractor’s PPN location must be approved by FBI.”Medical records are the property of the federal government and are not subject to state guidelines. Official record keeping of completed exams will be the responsibility of the government.
Will the Government update this section to reflect that contractors may store medical records in compliance with Federal and State guidelines?
Statement of Work C.3012FAA Exams. Can the Government provide an estimate of annual volume and locations of these exams?255 estimated volumes and are to be expected at all 56 field offices.
Statement of Work C.3613Board certification of physicians. Since the Government is willing to allow mid-level clinicians to perform services under this contract, would the Government also consider allowing physicians (MDs, DOs) that are not board certified, but that are properly licensed and credentialed, to perform services? Having this flexibility would also provide additional coverage in rural and OCONUS locations. The Government will allow physicians to be board-eligible.
Statement of Work C.3613The Government lists the following specialties as eligible to perform services: cardiology, emergency medicine, internal medicine, family medicine, or preventative medicine. Would the Government consider expanding that list to include other qualified specialties, such as Occupational Health for example, and include sub-specialties as well?Yes, occupational medicine is listed a sub-section of preventative medicine.
Statement of Work C.3614Credentialing verification. “All Providers shall be re - verified by the Contractor using the process above on an annual basis.” Would the Government consider changing this to ensure offerors align with National Committee for Quality Assurance (NCQA) credentialing standards, which is the industry standard for provider credentialing? The Government requires these re-verifications every year. Please use the standards written in section C.40 only.
Statement of Work C.3814“A separate, itemized invoice shall be submitted for each service location…”The FBI requires a single invoice with seperate sections for each ordering FBI Field Office (offices with have multipe service locations). Sections should be formated such that sections can be sent to ordering FBI Field Offices to validate that services were ordered, completed, and accepted.
Please clarify that a separate invoice is needed for every service location. This could result in hundreds/thousands of invoices at a time. Does the Government instead intend that the contractor submit a single invoice monthly that includes the itemized details listed in this section?
Statement of Work C.4015Given that this is a new contract, considering the background check and fingerprinting requirements, and the current state of availability of providers due to the COVID-19 crisis, would the Government consider extending the transition to 90 days? Alternatively, would the Government consider a phased transition?Yes, the transition period has been extended to 90 days.
Statement of Work C.4818An NDA form does not appear to be included in the solicitation. Can the Government provide this form? This section and the requirement for a physical NDA has been removed. Please adhere to updated section C.47 of the SOW as it will be the prevailing standard.
Statement of Work C.5620Fingerprinting. Given the COVID-19 crisis, we understand that availability of fingerprinting services nationwide is limited and may continue to be for some time. Would the Government consider a offeror process to instead complete a Criminal Background Check, which would include both Federal and County background checks on all credentialed healthcare practitioners with a lookback period of 7 years from date of completion. Records of compliance could be reported or audited at the Government’s request.Unfortunately, a Limited Access Request (LAR) with fingerprints will be the security requirement for this contract.
Statement of Work. GeneralGeneralWill the Government authorize appointment texts or emails to agents (for appointment reminders or other appropriate communications), and then include this in the SOW?Agree. Can add employees FBI email address to FD-1126.
Attachment 2 Instructions to Offerors, 6.1.29Identifying Codes and Applicable DesignationsRemoved the percentage requirement but request the Offeror still list all locations where work is to be performed.
The solicitation requires: “List all locations where work is to be performed and indicate whether such facility is a division, affiliate, or subcontractor, and the percentage of work to be performed at each location.”
Considering this is a new contract and the uncertainty of where the applicants might be, would the Government consider removing this requirement for Providers?
Where the RFP requires final proposal submission via email, we would need either send the file encrypted with a password or via a secure data transfer software like MoveIt. Can you advise if it’s acceptable for us to password encrypt the final submission email file, send the file via MoveIt, or advise another method for these parts of the proposalPassword Protected submission is acceptable.
Attachment 2 Instructions to Offerors, 6.1.310Security DocumentationPlease include the corporate Key Personnel as well as the Proposed Project Manager
Requires the completion of Attachment 7 “Key Management Personal Listing”. Can the Government advise which Contractor personnel should appear on this Attachment?
Instructions to Offerors2Can the 38% of the Small Business Subcontracting plan be combined by a small that meets more than one Social economic set-aside? i.e. if such company “A” is a WOSB, HubZone, SDVOSB, which would account for 12%. Meet the minimum requirements for WOSB, Hub, SDVOSB? Or is the government looking to have more than one company meet each social economic set aside? No, the subcontracting goal is to try and subcontract with multiple small businesses.
SOW4If services are to be approved before being performed at different locations...what is the anticipated wait time for the government's approval of the new location? The NAC (security check) and credentialing would need to be submitted and approved before a new provider could see an Employee/Applicant. This is not expected to be more than 30 days.
SOW2Can the government please list the usual required immunizations to be provided for travel?Yes. Required for all travel and FFD requirement: Tdap, MMR x2, Hepatitis A series, Hepatitis B series (or Twinrix-A/B combined series), Polio, Varicella series or proof of chicken pox, Flu and others recommended by CDC by location.
SOW7Does the Government have an anticipated time frame to approve the use of EMR systems at PPN Clinics? Not at this time.
SOWC.56SOW states “PPN clinics shall have at least one (1) designated employee cleared (LAR with fingerprints) to access FBI employee medical records.”Background and fingerprinting will occur annually. This process is expected to take around 30-45 days.
a. Does the fingerprinting and background check need to be reperformed periodically?
b. What is the expected time frame to get a provider approved to see FBI patients?
SOWC.1Does the government have specific location requirements for CDL and FAA medical exams?FAA and CDL exams are expected at all 56 Field Office Locations.
SOWC.16Does the government have any past data to estimate emergency/surge levels? No official data. Mission dependent.
SOWC.17The SOW states “The Contractor shall ensure that the Provider works directly with Employees / Applicants to schedule appointments ...” and then states “The Contractor shall make at least three (3) attempts via phone to contact the Employee / Applicant or defined FBI POCs to schedule the appointment ...” Can the government clarify who will contact the applicant to schedule appointments? The FBI POC will send an Employee/Applicant's package to the Contractor to call the Employee/Applicant to schedule an appointment. Section has been revised.
Attachment 2. Instruction to OfferorsSection 1.3. Table I-1Table I-1 lists 3 volumes and the narrative asks for 4 volumes. Can the government confirm that the proposal layout should follow the table I-1 or the narrative section 2.2 through 6.0?Table I-3 which has been revised.
Attachment 9 – LocationsSheet 1Please provide the zip codes for each office identified so that we may ensure the provider locations fall within the appropriate distances from these locations.Attachment 9 has been updated to include zip codes.
