MARKET RESEARCH QUESTIONNAIRE

AMBULANCE SERVICES

FEDERAL MEDICAL CENTER (FMC) LEXINGTON, KY

The Federal Bureau of Prisons (FBOP) is conducting market research in compliance with Federal Acquisition Regulation, Part 10. The service will encompass the provision for Ambulance and Medical Transport Services.

The services will be provided at Federal Medical Center (FMC) and/or Satellite Prison Camp (SPC), Lexington located at 3301 Leestown Road, Lexington, Kentucky 40511.

Most services are expected to occur but are not limited to the following Kentucky counties: Bourbon, Clark, Fayette, Franklin, Jessamine, Madison, Scott and Woodford. Most services will have either their origination or destination as the Federal Medical Center and/or Satellite Camp Prison, 3301 Leestown Road, Lexington, Kentucky, 40511. The contractor will agree to transport inmates to the destination requested by FMC regardless if another hospital is a shorter distance from the FMC. The Contractor must be available for emergency ambulance and/or routine ambulance transport services 7 days a week, 24 hours a day, including Federal holidays. For emergent transport, the Contractor must be on-site at the FMC Lexington within 15 minutes of request.

These services are listed in the North American Industry Classification System (NAICS) Code Category 621910 – Ambulance Services. The size standard number for NAICS 621910 is represented in millions of dollars. (below $22.5 million is considered small, above $22.5 million dollars is considered large).

Employer’s liability not less than $100,000, General Liability no less than $500,000 per occurrence, and Automobile Liability of at least $200,000 person, $500,000 per occurrence for bodily injury, and $20,000 per occurrence for property damage. • DOJ-01, Whistleblower Information Distribution will be incorporated into the solicitation and resulting contract.

• DOJ-08, Continuing Contract Performance During a Pandemic Influenza Emergency – when supplies or services are considered critical to mission accomplishment will be incorporated into the solicitation and resulting contract.

• FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Products and Commercial Services will be incorporated into the solicitation and resulting contract.

• FAR 52.252-1, Provisions Incorporated by Reference will be incorporated into the solicitation.

• FAR 52.204-7, System for Award Management will be incorporated into the solicitation.

• FAR 52.212-1, Instructions to Offerors Commercial Products and Commercial Services and to identify any tailoring.

• FAR 52.204-24, Representation Regarding Certain Telecommunications and Video Surveillance Services and Equipment will be incorporated into the solicitation.

• JAR 2852.233-70, Protest Filed Directly with the Department of Justice, Service of Protest, will be incorporated to notify offerors of the location whereby the Contracting Officer shall be served will be incorporated into the solicitation.

• FAR 52.212-3, Offeror Representations and Certifications – Commercial. Product and Commercial Services if not current in sam.gov will be incorporated into the solicitation.
