3.1 GENERAL REQUIREMENTS

A. DESCRIPTION OF DUTIES

The Federal Bureau of Prisons (FBOP) intends to make a single award to a responsible entity that will provide all necessary equipment, labor, materials, and incidentals to construct the work specified in these documents at the Federal Medical Center (FMC) located in Fort Worth, Texas. FMC Fort Worth is located at 3150 Horton Road, Fort Worth Texas 76119.

B. SCOPE OF WORK

The FBOP seeks eligible business firms capable of designing and constructing a special operations response team obstacle course obstacle at Federal Medical Center, Fort Worth, Texas. This project will consist of a base bid. The base bid shall include the construction of one tough man pre-engineered obstacle, earthwork and leveling, and metal-frame enclosure erection.

C. QUALITY ASSURANCE

All construction activities shall be monitored by a Contracting Officer Representative (COR) employed by the Federal Bureau of Prisons. The COR will monitor/observe the day to day operations of the project and serve as liaison between the FBOP and contractor. The COR shall be the main contact for scheduling, badging, site access, product submittals, quality control, testing, final inspection, etc. The Contractor shall immediately contact the COR if discrepancies are discovered that may impact project schedule or cost. The Contractor shall be responsible for acquiring and maintaining any required permits.

The Contractor installing the pre-engineered/pre-fabricated obstacle shall be certified by the manufacturer. The Contractor must be eligible to represent the manufacturer and possess the ability to convey a full one year warranty for all components of the construction work to FMC Fort Worth. In the event there is a conflict between these document, code(s), and any documentation provided by the manufacturer, the contractor shall follow the most stringent specification at no additional cost to the Government.

The contractor shall perform all work in accordance with applicable codes, standards, regulations, and law. The contractor shall repair all damage done by his/her workers to any existing-to-remain equipment, utility, building system, or other property at no cost to the Government.

Any site conditions that will require the contractor to perform work outside of what is required on the contract documents shall be considered a changed condition and authorized only by Modification of Contract. The contractor shall notify the COR of any such conditions immediately after they are uncovered and shall not begin work on these areas prior to receiving a completed Modification of Contract document from the CO.

D. CONSTRUCTION SUBMITTALS

The contractor is required to provide submittals for all products to be supplied/installed ten

(10) days after commencement of work. Additionally, the Contractor shall process submittals within fifteen (15) calendar days after receipt. The Contractor shall promptly notify the Contracting Officer (CO) and the COR when submittal action is delayed for any reason, while citing the cause for delay.

In addition to the FAO-0005 Submittals clause in the solicitation document:

Preparation of Submittals: Provide permanent marking on each submittal to identify project, date, Contractor, Subcontractor, submittal name, Specification section number, and similar information to distinguish it from other submittals. A minimum of four (4) marked copies will be supplied for each submittal.

Coordination and Sequencing: Coordinate preparation and processing of submittals with performance of the work so that work will not be delayed by submittals. Coordinate and sequence different categories of submittals for the same Work, and for interfacing units of Work, so that one will not be delayed for coordination of review with another.

Upon completion of the project, the Contractor shall provide a complete and detailed certification that all aspects of the project were installed operate as designed, to include all safety protective functions.

Should it appear that the work intended to be described is not sufficiently detailed or explained by the Construction Specification, the Contractor shall issue a Request for

Information (RFI) to the FBOP CO. RFI’s shall be used by the Contractor to clarify information in the Construction Specification only. Each RFI shall be numbered sequentially, dated and indicate a reasonable date for the CO’s written reply. All RFI questions shall be thoroughly re- searched by the Contractor, prior to presenting them as RFI’s. In no case shall any work proceed in uncertainty.

A. TESTING AND ACCEPTANCE:

The Contractor shall notify the COR ten (10) days prior to the completion so that arrangements can be made to have BOP required staff available to inspect the site and provide a punch list if necessary.

B. UTILITIES

The Contractor shall have access to limited water (hose spigot) and electrical power (120 V receptacles) adjacent to the project site. All further utility needs required to accomplish contract work shall be provided by the Contractor at his or her expense.

E. SAFETY AND HEALTH

The Contractor is wholly responsible for safety and health on the project. In performing the work under this contract the Contractor shall:

1. Conform to the specific safety requirements established by this contract;

2. Comply with the safety rules of the FBOP institution that concern related activities not directly addressed in this contract;

3. Take all reasonable steps and precautions to prevent accidents and preserve the life and health of Contractor and Government personnel performing or in any way coming in contact with the performance of this contract;

4. Take such additional immediate precautions as the CO may reasonably require for safety and accident prevention purposes; and

5. The Contractor must remain compliant with all OSHA regulations and is subject to their jurisdiction throughout the life of the contract. FMC Fort Worth personnel are un-der the Jurisdiction of OSHA.

The Contractor will provide FMC Fort Worth with a written Safety Plan addressing all items listed above, to include actions necessary in case of an emergency and directions to the nearest health care provider to be used.

All contractors and employees shall have and wear the required personal protective equipment (PPE) prescribed by industry standards at all times while on the job site. It is the responsibility of the contractor to provide all PPE equipment.

Should a system require Lock-out/Tag-out, the system will be required to have both Con-tractor and FBOP locks on the isolation point to prevent shock. All lockout/tagout measures to be implemented, and/or removed, shall require visual verification by the on-site superintendent and COR prior to doing so.

F. HOUSEKEEPING AND MATERIAL STORAGE/DISPOSAL

A laydown yard for equipment and supply will be provided to the contractor, and will be designated after award. The contractor awarded the contract will be responsible to ensure the premises are kept clean and free from accumulations of waste materials and rubbish at all times. The contractor will remove all debris, scrap, and rubbish from the work area daily.

Surplus materials and all equipment shall be promptly removed from the site upon completion of the work. The Contractor shall be responsible to remove all debris and waste materials from the job site at FMC Fort Worth upon conclusion and acceptance of the project.

These items must be disposed of in accordance with all Federal, State, and local laws.

The Contractor shall provide protection of the work against weather, rain, wind, storms, frost, cold, or heat so as to maintain all work, materials, apparatus and fixtures free from in-jury or damage.

The Contractor shall review his or her site layout plan for staging, stockpiles, etc., with the COR. The Contractor shall maintain all staging and construction areas in a neat and orderly fashion.

The Contractor will ensure proper containment for any chemicals and ensure all materials are stored in an attitude most conducive to safe and proper storage prior to installation. All flammable materials shall be stored in a cool, dry area away from open flames and sparks.

Follow precautions outlined on containers or supplied by the material manufacturer/supplier.

All contractor tools shall be inventoried prior to entry and exit from the institution each day.

Any contractor materials, tools, and/or equipment that will be required on-site for an extended period of time may be permitted to be stored in an area designated by the COR.

The contractor shall store these tools in a securable, single point access storage container(s), such as a job or gang box for the duration of the project.

All contractor tool containers shall be secured using a padlock protocol with two locks to secure it. One shall be a contractor lock and the other lock provided by the institution. If necessary and at the discretion of the FBOP, a heavy duty steel chain, provided by the contractor, may be used to facilitate the locking requirement. All materials and equipment stored within the container shall be organized to permit ease of inventory.

Contractor tools stored in an on-site container will require a daily inventory check in/out accountability protocol. At the start of each work day all tools stored within a container(s) shall be verified as all present and accounted for by an on-site contract employee and con-firmed as such by an escorting FBOP staff member. A tool inventory form, provided by the contractor, shall be used to facilitate this process.

At the end of the work day, all tools will be inventoried back in their respective storage container(s). The end-of-the-day tool inventory procedure shall require the presence of; a) Escorting FBOP staff member; b) Contract employee overseeing tool accountability; and c) Institution Lieutenant who will oversee, and validate the tool inventory procedure.

All contractor owned extension cords that will be used during the project, regardless of length, shall be in good condition with no broken outer insulation. Extension cords are considered a “high risk” tool having a FBOP tool classification of Double-A. Due to this policy, all contractor owned extension cords used on the project will not be permitted to be stored in the on-site container and will be required to exit the institution at the end of every work day for the duration of the project.

If ladders are required, all ladders regardless of the size shall be stored, and secured to a stationary fixture with a chain and lock in a predetermined area, in a way that they cannot be removed. If ladders are required, immediately consult with the COR to ensure the COR can make timely notification to appropriate institution staff.

3.2 CONTRACTOR SECURITY/WORKING CONDITIONS

A. WORKING CONDITIONS

1. Working Hours - Normal business hours for the facility is 7:30a.m. - 4:00p.m. Work hours and work days can be vary from the normal business by submitting a schedule to the CO for advance approval prior two weeks prior to the commencement of work.

Thereafter, the contractor will be required to provide a written request for extension of hours or change in work days to the CO.

2. Parking – Parking is available for contractor vehicles. Parking areas, either on or off-site will be furnished for workmen by the Contractor at a location approved by the COR.

3. Telecommunications – Cell phones, cameras or two way radios/pagers are not al-lowed within the secure perimeter of the institution, however if required, must be se-cured in a locked vehicle and only utilized when necessary for the project.

A laptop computer specifically configured to perform programming work, should it be required by the contract, would be the only exception. Such technology would require operational inspection by the institution’s Computer Services Manager and approval for institution access by the Warden.

The Facilities Department maintains a digital camera that will be utilized by BOP staff to document the project work. The Contractor may request photographs or copies of these documentation photos for record keeping.

4. Work Location - The work location is OUTSIDE the perimeter fence, check-in will be required at the rear gate to keep the institution aware of contractor status on Federal property.

5. Controlled Items/Prohibited Items/Worker Dress - All construction materials/tools shall be controlled and stored in a designated area mutually agreed upon between the COR and the contractor. The storage area will be a secure lockable gang box or similar that includes a contractor lock and a FBOP lock for security purposes, size to be deter-mined by the contractor and the COR.

Contractor will be permitted to wear jeans in the institution, however, shorts, green, camouflage, or khaki-color clothing will not be allowed. The clothes will be proper and suitable for the services that are being provided.

6. Contraband: The Contractor shall inform his/her employees that intoxicating beverages, drugs, weapons, etc.., will not be allowed on the site under any circumstances. Introduction of contraband shall be subject to the penalties prescribed by Title 18 USC 1791.

7. Explosives: The use of explosives is absolutely forbidden without written authorization from the FBOP CO upon no less than thirty (30) days advance written request.

8. Smoking: Smoking on federal property is permitted only in designated outdoor areas.

This policy will be strictly enforced.

9. Professional Behavior: All work shall be carried out in such a way so as to cause minimal interference with the normal operations of the institution. Contract employees shall comply with all institution regulations regarding safety and security.

The CO or COR may, if circumstances dictate, instruct the Contractor to permanently remove a particular employee from the project if he or she engages in unsafe, careless, or unprofessional actions.

B. SECURITY CLEARANCE/ CONTRACTOR BADGES

1. All Contractors/subcontractors and their construction staff will be required to attend a four (4) hour orientation and security procedures training session. Each individual is required to attend. This training must be accomplished prior to being granted access to the institution to start work. Multiple security forms must be filled out prior to the training.

2. All Contractor/subcontractor personnel working on site will be issued and required to wear a photo ID/contractor badge provided by the institution. For security reasons, the badge shall be worn in a visible manner as directed by the COR.

3. Each day, each Contractor and subcontractor shall provide a checklist to the COR, showing all employees on site, verifying the number of workers on site, and that all workers are approved to be on site.

4. Contractors should plan for a minimal amount of time accrued for security considerations, i.e., check-in screening, delay due to inmate movements, institution lock-down, institution emergencies, etc. In the event of an institution emergency, the Contractor will be instructed to secure all work, secure and stow all tools, equipment and materials, and the Contractor will be escorted OFF of the institution property. Work will resume once the emergency passes.

C. DELIVERY OF MATERIALS:

All construction materials/equipment required for work on this project is required to be processed by the Contractor and coordinated with the institution COR for temporary storage at the Contractors designated laydown yard, or the work site. Any construction related deliveries which are directly shipped to the contractor can be received between the hours of 8:00 a.m. to 2:00 p.m., Monday through Friday, excluding Federal Holidays. At no point will a FBOP staff member receive or sign for a delivery if the contractor or his representative is not present to verify and accept the delivery.

Under no circumstances will FBOP or its staff be responsible for the acceptance of any Con-tractor or company deliveries. The contractor shall be responsible for providing their own means of loading and unloading contractor material or equipment. The contractor shall not use the institution entry/driveway for the loading, unloading, or storage of any materials and equipment. The contractor will not impede/block the driveway at any time during or after the work shift.

All products delivered to the job site shall be in their original unopened containers or wrappings and clearly labeled with the manufacturer’s name, product identification an date of manufacture.

Protect all materials from damage during transit, storage and delivery to the job site. Place all materials on pallets and protected from moisture. Materials damaged in handling or storage shall not be used. All materials determined to have been damaged are to be replace and removed from the site immediately.

SECTION 01 33 20 – CONSTRUCTION SUBMITTAL PROCEDURES PART

1 - GENERAL

1.1 SUMMARY

A. Section Includes:

1. Definitions.

2. General Submittal requirements.

3. Specific category submittals.

1.2 DEFINITIONS

A. The types of submittal requirements specified in these Requirements include shop drawings, product data, samples, certificates of conformance or compliance, certified test or inspection reports, and miscellaneous work-related submittals for construction.

B. Work-related submittals are categorized for convenience as follows:

1. Shop drawings include specially-prepared technical data for this project, including drawings, diagrams, performance curves, data sheets, schedules, templates, patterns, reports, calculations, instructions, measurements, and similar information not in standard printed form for general application to a range of similar projects.

2. Product data is standard printed literature which includes illustrations, schedules, performance criteria, instructions, brochures, color charts, diagrams, test data and other information illustrating a material, product or system for a portion of the Work.

3. Samples are physical examples illustrating size, kind, pattern, texture and color, material, equipment, system or workmanship and establishing standards by which the Work will be judged. Certificates of conformance or compliance are documents attesting that a product complies with a specified standard including Flame Spread Ratings.

4. Certified test (or inspection) reports are documents attesting that a product meets a specified level of performance or quality when a prototype specimen is tested or inspected in accordance with a specified procedure, and consist of a certified statement by the product supplier or Contractor accompanied by a complete report of the inspection or test.

Miscellaneous submittals related directly to the work (non-administrative) include warranties, maintenance agreements, workmanship bonds, project photographs, survey data and reports, physical work records, copies of industry standards, record drawings, field measurement data, operating and maintenance materials, overrun stock, and similar information, devices, and materials applicable to the work and not processed as shop drawings, product data samples, certificates of conformance or compliance, or certified test reports.

C. The submittal schedule and register shall minimally include:

1. Submittal description.

2. Specification reference.

3. Type and quantity of submittal.

4. Date of final submittal and approval by the Contractor.

5. Date of submittal to the Government.

6. Review action by the Government, where applicable.

7. Date of return by the Government, where applicable.

1.3 GENERAL SUBMITTAL REQUIREMENTS

A. Coordination and Sequencing: Coordinate preparation and processing of submittals with performance of the work so that work will not be delayed by submittals.

1. Telephone contacts, personal contacts, fax transmissions and other means of communications shall be employed to assure prompt processing of submittals and prevent delay in the construction work.

2. The Contractor shall process submittals within fifteen (15) calendar days after receipt.

3. The Contractor shall promptly notify the Contracting Officer or Construction Representative when submittal action is delayed for any reason, citing the cause for delay.

B. Preparation of Submittals: Provide permanent marking on each submittal to identify project, date, Contractor, Subcontractor, submittal name, Specification section number, and similar information to distinguish it from other submittals.

C. Show Contractor’s executed review and approval marking and provide space for action marking.

D. Transmittal Form: Provide standard transmittal form with each submittal.

1.4 SPECIFIC-CATEGORY SUBMITTAL REQUIREMENTS

Shop Drawings: Refer to FAR 52.236-21, "Specifications and Drawings for Construction." Provide newly prepared information, with graphic information at accurate scale (except as otherwise indicated), with name of preparer indicated (firm name). Identify materials and products in the work shown. Indicate compliance with standards, and special coordination.

1. Equipment and Systems: Shop drawings for equipment and systems shall show ratings (where applicable), and how components are assembled, function together, and how they will be installed. Shop drawings along with product data, samples, certificate of conformance or compliance, certified test or inspection reports, and other submittals for equipment, systems, and their component parts shall be coordinated and submitted as a unit. Multiple or piecemeal submissions are not acceptable except where prior approval is obtained from the Construction Representative, in which case a list of data to be submitted later shall be included with the first submission.

B. Product Data and Samples: Except for job site mockups and sample construction provided in the quantity specified in other Sections, each submittal shall include at least four (4) copies; two (2) copies will be retained by the Federal Correctional Institution for record and at least one (1) copy will be returned to the Contractor.

SECTION 03 30 00 – CAST-IN-PLACE CONCRETE

PART 1 - GENERAL

1.1 RELATED SECTIONS

A. Section 01 00 03 − GENERAL STRUCTURAL REQUIREMENTS.

1.2 SUBMITTALS

A. Product Data: For each type of product used.

PART 2 - PRODUCTS

2.1 CONCRETE MATERIALS

A. Portland cement shall conform to ASTM C150, Type I, II, or III, low alkali.

B. Aggregate: Normal-weight aggregate for concrete to meet ASTM C33.

C. Fly Ash: Coal fly ash for use as a mineral admixture in concrete shall conform to either Class C or Class F requirements, as specified in ASTM C618.

1. At the Contractor’s option, fly ash may be substituted for the specified percentage of the portland cement by weight in a concrete mixture as follows:

a. Cast-in-place concrete – 20% maximum.

b. The substitution ratio of fly ash to portland cement shall be a minimum of 1:1, but may be increased not to exceed 1.5:1, depending on the sources of materials to be used.

2. Discoloration due to fly ash is unacceptable.

D. For all exterior exposed-to-view beams, columns, wall and wall panels, fly ash and ground granulated blast-furnace slag shall not be used.

2.2 REINFORCING

A. All reinforcing steel shall be detailed and placed in conformance with the Building Code Requirements for Reinforced Concrete (ACI 318M), and the Manual of Standard Practice for Reinforced Concrete Construction by the CRSI or as modified by the Construction Documents.

B. Reinforcing bars shall be ASTM A615M Grade 420, unless noted otherwise.

C. Weldable reinforcing bars shall be ASTM A706M Grade 420, unless noted otherwise.

D. Welding or reinforcement shall be made with low hydrogen electrodes in conformance with Recommended Practices for Welding Reinforcing Steel, American Welding Society, AWS D1.4.

E. All reinforcing bar bends shall be made cold.

F. Welded wire fabric reinforcement shall be in conformance with ASTM A1064.

G. Non-composite Wythe Connectors: Shall be either glass-fiber and vinyl-ester polymer or stainless-steel pin connectors manufactured to connect wythes of precast concrete elements without shear transfer.

2.3 CONCRETE MIX

A. Unless otherwise noted, all concrete shall develop a minimum compressive strength of 21 MPa (3500 psi) at 28 days.

B. Water absorption: Limit water absorption to 6 percent by weight, maximum, for all exterior exposed to view concrete.

C. Concrete mixing shall conform to ASTM C94.

PART 3 - EXECUTION

3.1 INSTALLATION

A. Forms for concrete shall be constructed to provide the specified cambers.

B. The Design Builder shall maintain a log of structural slab elevations based on the Architectural Drawings plus the addition of any cambers indicated on the construction documents. This log shall indicate screed elevations prior to the concrete pour, and top of concrete slab prior to removal of forms.

C. The concrete slab minimum thickness shall be maintained, unless otherwise noted.

D. Floor Surface Finish: Finish surfaces to the following tolerances, measured within 72 hours according to ASTM E1155:

1. For Slab-On-Grade: Specified overall values of flatness, F(F)35; and levelness, F(L)25; with minimum values of flatness, F(F)24; and levelness, F(L)17.

2. For Suspended Slabs (shored until after testing): Specified overall values of flatness, F(F)30; and levelness, F(L)20; with minimum values of flatness, F(F)24; and levelness, F(L) 15.

3. For Suspended Slabs (unshored): Specified overall values of flatness, F(F)30; with minimum values of flatness, F(F)24.

E. Placement of concrete shall conform to ACI 304 and the accepted Construction Issue Documents. Sandblast all concrete surfaces against which concrete is to be placed.

F. The following specified concrete cover shall be provided for reinforcement placed in cast-in-place concrete, unless noted otherwise:

G. All reinforcing bars, anchor bolts and other concrete inserts shall be well secured in position prior to placing concrete. Use templates to secure anchor bolts.

H. Provide sleeves for plumbing and electrical openings in concrete before placing.

Do not cut any reinforcing which may conflict. PVC sleeves shall not be used.

Coring in concrete is not permitted.

I. The outside diameter of conduit or pipe shall not exceed one third of slab thickness and shall be placed between the top and bottom reinforcing, unless specifically designed otherwise. Concentrations of conduits or pipes shall be avoided except where detailed openings are provided.

J. Projecting corners of beams, walls, columns, etc., shall be formed with a 20 mm (0.8") chamfer.

K. Without dislodging aggregates, saw cut control joints in all slabs on grade as soon as possible after finishing. Construction joints may be substituted for saw cut joints.

L. Do not back fill against concrete or masonry walls until walls, connecting floor slabs and footings have cured to 28-day strength.

1.3 WARRANTY

A. The subcontractor shall warrant his material and workmanship on this project for a period of one (1) year from the date of Substantial Completion. The subcontractor agrees to repair or replace any defective materials, and to correct any defective security work, when given written notice by the Contracting Officer during this warranty period. The subcontractor also agrees to respond to these notices within five (5) calendar days and make all repairs as required for proper operation during this warranty period.
