About this file

This document contains five attachments from the Virginia Department of Transportation (VDOT) for IFB #159471, including forms for small business subcontracting plans, State Corporation Commission registration, normal and emergency contacts, subcontractor approval requests, and quarry source materials. The documents are standardized forms that bidders must complete and return with their bid responses to be considered responsive.

The forms emphasize Virginia's goal that over 42% of purchases be made from small businesses certified by the Virginia Department of Small Business and Supplier Diversity (DSBSD). Bidders must either be DSBSD-certified small businesses themselves or detail their plans to subcontract portions of the work to DSBSD-certified small businesses, including women-owned, minority-owned, and service-disabled veteran-owned businesses. The subcontracting plan requires detailed information about proposed subcontractors including certification numbers, contact information, and percentage of work to be performed. All subcontractors must receive prior written approval from VDOT, and the primary contractor remains fully liable for subcontractors' work.
