The purpose of this amendment is to 1) provide responses to the questions, 2) incoporate attachment A, revised statement of work dated August 13, 2014, 3) incorporate the revised dates under section B.2 titled Compensation Cost/Price, 4) incorporate the revised Schedule of Deliverables under section F.6 and 5) to incorporate attachment D titled Rural Counties.

Is it intended that since contractor is allowed
14 weeks to submit ranked list of hospitals (3.4, 3.5, 3.6), that the in 4.2 means that contractor actually only has about 7 months?
Answer 2:
The requirement that all on-site consultations be completed by ten (10) months from the EDOC is no longer applicable. Each of the eight on-site consultations must be completed before the end date of contract, allowing time for writing of consultant's report for inclusion in the
Quarterly Report of the quarter in which the TA was provided and in the toolkit.

Is the COR committed to prompt review and feedback on draft toolkit to allow changes to be incorporated within a week?

FY2014-15 are eligible to apply for TA?
Answer 6:
Per the Background section of the Statement of
Work, eligibility is open to small rural hospitals with fewer than 50 staffed beds and located in persistent poverty areas. Current and former Section 330A grantees are also among those eligible to receive assistance.
Question 7:
Will the government please clarify whether the contractor will be developing a formal Request for Application (RFA) such as those for grant awards or developing less formal version of the application?
Answer 7:
The offeror is not required to develop a RFA.
Question 8:
(Task 3.6) Who is responsible for selecting the final 8 hospitals to receive TA? Will it be the
COR or the contractor?
Answer 8:
The offeror will provide to the COR a rank-order list of hospitals based on stated criteria. The
COR will make final selection based on hospital ranking.
Question 9:
If the COR is to make the selection, then is the contractor providing a short-list based on our screening and scoring process of the applications?
Answer 9:
Per section 3.6 of the SOW, the contractor will submit a ranked-order list of hospitals selected to receive technical assistance based on program eligibility, readiness, selection criteria, and preference statement to the COR for review and approval. The COR will make final selection based on hospital ranking.
Question 10:
If the answer to 7 is "yes" there is some clarification needed on the timing specified in the SOW. If the application process and form are due within 6 weeks from EDOC and finalized 3 days

later and then the selection of the eight hospitals is due 8 weeks later, does the government anticipate that the hospitals will have sufficient time to complete the application and be evaluated for eligibility?
Answer 10:
N/A. The answer to 7, above, is no.
Question 11:
As a follow up to our question 10. above, selection of the hospitals and the TA Plan to support the 8 selected hospitals are due at the same time. Will the government re-evaluate the timeline and consider adding time for analysis and development of the TA Plan after the hospitals are chosen?

Yes (see attachment). The timeline for
Deliverable 4.1 is revised to indicate submission of TA Plan to COR within three (3) weeks of the
Ranked Order List (Task 3.6).
Question 12:
The title of Task 5 is "Toolkit" however section
5.1 refers to a toolkit "report" in the first sentence then later references a "toolkit".
Please clarify whether the deliverable is an actual toolkit or a toolkit report.

The deliverable is a toolkit.

Since the travel ODCs cannot be accurately proposed until the 8 hospitals are selected, will the government provide some parameters for all offerors to use for site visit logistics to ensure consistency across the Cost Proposals?

other product that require 508 compliance and should be part of "passing" or accepting the corresponding deliverable. While this requirement may apply to previously developed
(off-the-shelf) materials, we would like to know how we can complete GPAT before the material
(toolkit, video, etc) are developed during the execution of this contract.
Answer 25:
The GPAT applies to any EIT deliverable the contractor or potential contractor is going to develop and deliver to the Federal Government
(HRSA). The GPAT applies to more than web applications.
When filling out the GPAT, the vendor should focus on the deliverables. So for example, if
HRSA is asking for a website the vendor should explain in column 5 under 1194.22 a-p how they are going to meet that specific provision. In column 4 they should state whether they can
Fully, Partially or Not meet the provision.
