About this file

This is a guidance document for hoisting and pile driving equipment under RSHS Section 19, specifically addressing overhead and gantry crane operations at the Northern California Area Office (NCAO) of the Bureau of Reclamation, issued July 2014.

The document clarifies that crane usage at NCAO falls under the maintenance category and does not require compliance with OSHA 1926, Subpart CC. Cranes are classified as stand-by equipment not in regular use. Three inspection types are required: Daily inspections performed by crane operators prior to carrying loads, including visual audible inspections, functional testing of all systems, limit switch verification, hydraulic brake inspection, hook and latch examination, and wire rope inspection; Frequent inspections conducted approximately every six months coinciding with annual preventive maintenance, with detailed criteria for hook wear (no greater than 10% of original cross section, throat opening changes not exceeding 5% or 0.25 inch) and wire rope removal standards; and Annual inspections by third-party contractors for cranes rated 5 tons or greater, with qualified Reclamation employees or third-party inspectors conducting inspections for equipment below 5 tons. Load testing must be between 100-110% of crane rating, with critical lifts defined as 90% of rated capacity. Crane operator certification is valid for five years, physical exams for three years, and employees operating equipment rated 2,000 lbs or less may be designated by supervisors. The document references ASME B30.2 Chapter 2-2 for inspection and testing standards.

RSHS Section 19 Hoisting and Pile Driving Equipment Guidance and Clarifications Overhead and Gantry Cranes Jascoli, July 2014

Note: Italics are excerpted from RSHS Section 19 issued May 2014

Crane Usage Defined - Construction or Maintenance Until further notice, the usage of cranes at NCAO falls under the MAINTENANCE category of work and do not need to comply with OSHA 1926, Subpart CC for cranes used in construction.

Crane Frequency of Operation Defined - NCAO Cranes are Stand-by Cranes that are not in regular use. Even during rewinds and runner replacements, crane usage never rises to regular use category.

19.3 Requirements for Cranes and Hoisting Devices, Inspections and Testing

19.3.1 Inspection Forms – This paragraph says: Sample forms are located in FIST 2-11. (This FIST does not exist).

19.3.2 Inspections - A designated person will conduct all inspections. A designated person is a person selected by the employer as qualified to perform specific duties.

Translation – Plant mechanics will perform daily inspections and frequent inspections in addition to the CARMA preventative maintenance. A third party contractor will be performing the annual inspections.

19.3.2(a) Frequency There are three types of inspection, DAILY, FREQUENT and ANNUAL under the Stand-by Crane usage.

NOTE: See ASME B30.2 Chapter 2-2 Inspection and Testing as a reference.

Daily Inspection – Crane Operator to perform the following prior to carrying load:

1. Perform visual and audible inspection of the crane;

2. Functionally test all systems;

3. Test and verify limit switches;

4. Visually inspect hydraulic brake systems;

5. Visually inspect hook(s) and latches;

6. Visual inspection of the wire rope;

7. Crane Operator to sign and date log.

Frequent Inspection – A frequent inspection is to be performed approximately six months from the last annual inspection. This will be scheduled to coincide with the crane annual preventative maintenance cycle in CARMA.

1. Perform visual and audible inspection of the crane;

2. Functionally test all systems;

3. Test and verify limit switches;

4. Visually inspect hydraulic brake systems;

5. Visually inspect hook(s) and latches:

a) Wear no greater than 10% of the original cross section;

b) Change in throat opening no greater than 5%, not to exceed 0.25 inch;

c) No visible bend or twist from plane of unbent hook.

6. Hook attachment and securing means

7. Visual inspection of the wire rope and removal from service criteria:

a) Significant distortion of the wire rope structure such as kinking, crushing, unstranding, birdcaging, signs of core failure or steel core protrusion between the outer strands.

b) Significant corrosion;

c) Electric arc damage or other heat related damage;

d) Improperly applied end connections;

e) Significantly corroded, cracked, bent or worn end connections;

f) In running ropes, 6 randomly distributed broken wires in one rope lay or 3 broken wires in one strand in one rope lay;

g) In idler or equalizing (standing) rope, more than 2 broken wires in one rope lay located in the rope beyond the end connection or more than one broken wire in a rope lay at an end connection;

h) A diameter reduction of more than 5 % from nominal diameter.

8. Rope for proper spooling onto drums and sheaves;

9. Warning devices;

10. Inspection to be documented and recorded.

19.3.2(b) Annual Inspection - Cranes and hoists with a rating of 5 tons or greater will have a third party inspection annually.

Cranes and Hoists below 5 tons are to be inspected by a qualified Reclamation employee or third party.

19.3.3 Load Tests

Load test to be no less than 100% of crane rating and no more than 110%.

A load test for critical lift is valid for four years.

19.5 Qualifications for Operating Hoisting Equipment

19.5.2(b) Crane Operator Certification valid for 5 years

19.5.2(e) Hoisting operations of equipment rated at 2,000 lbs or less may be performed by an employee designated by the immediate supervisor.

19.5.3 Crane Operator physical exam valid for 3 years

19.7 Requirements for Critical Lifts

A critical lift for overhead and gantry cranes is 90% of crane rated capacity.

Test weight for critical lift may be actual load for some cranes.
