The questionnaire also addresses whether cure notices, show cause letters, letters of reprimand, payment suspensions, or terminations were issued; whether the evaluator would award another contract; customer satisfaction with final products and services; and whether the firm was provided opportunity to respond to negative comments. An overall rating and comments section conclude the evaluation, which must be signed and dated by the evaluator.
