Replace Vinyl Flooring at VC Breezeway

DATED: 02/22/2024

Stones River National Battlefield Statement of Work

1. INTRODUCTION

This Statement of Work (SOW) outlines the scope for replacing the vinyl floor tile and base molding with new commercial grade vinyl flooring in the Visitor Center building.

2. OBJECTIVE:

This contract will consist of removing the 469 SF of 12”x12” Vinyl Floor tile and 169 LF of 6” base molding and replace it with new commercial grade Vinyl flooring glued to the concrete floor of the visitor center building. The previous date of replacement was in 2004. The vinyl tile is very near the end of its’s lifecycle and needs replacement.

3. PLACE OF PERFORMANCE:

This project will take place at the Visitor Center breezeway located at 3501 Old Nashville Highway. Murfreesboro, TN 37129.

4. PERIOD OF PERFORMANCE:

The period of performance shall be within 60 days after the start date on the notice to proceed.

5. SCOPE:

· Contractor shall remove and dispose of existing 12”x12” vinyl floor tile and base molding.

· Contractor shall prepare the subfloor for the installation of new vinyl floor tile.

· Contractor shall install 469 SF of new vinyl floor tile in the specified areas according to manufacturer’s guidelines and industry standards.

· Contractor shall install 169 LF of 6” base molding to compliment the new vinyl floor tile.

· Contractor shall ensure that all installations meet building codes and regulations.

· Contractor is responsible for suppling all materials and labor for this contract.

· Contractor shall dispose of all the debris properly in approved landfill.

· Site visit is highly encouraged.

6. SUBMITTALS:

· The Contractor shall submit Vinyl flooring sample to show how it matches up with existing color and design or close as possible.

· Contractor shall submit base bolding sample to show how it matches up with existing color and design or close as possible.

· Contractor shall submit a copy of all warrantee paperwork.

· All submittals shall be on the Government CM-16 form and approved by the project COR.

7.GOVERNMENT-FURNISHED PROPERTY, MATERIAL, EQUIPMENT, OR INFORMATION (GFP, GFM, GFE, OR GFI):

There will be no Government-furnished property or services for the purposes of this contract.

8. CLEANING:

· Interior Cleaning: Contractor shall sweep surfaces; Clean building surfaces with recommended products.

· Final Cleaning: Contractor shall remove all tools, equipment, surplus materials, and rubbish. Refinish surfaces of existing facilities that are marred, scratched, or damaged to match original condition. At time of final inspection, site shall be free of nails and wood debris and project shall be thoroughly clean and ready for use.

9. SECURITY:

· The contractor is responsible for all materials and equipment on the job site at all times.

· All employees working on the job site shall be legal to work in the United States of America and not be a prior convicted felon.

· Park hours are from 8:00 AM until 4:30 PM. The Contractor may plan to work on site from 8 AM to 4:00 PM Monday thru Friday; except for Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Contractor shall not work outside of these hours, without prior approval from STRI Facility Manager.

10. QUALITY ASSURANCE REQUIREMENT:

The contractor shall develop and maintain a quality program to ensure services are performed in accordance with commonly accepted commercial practices. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. As a minimum the contractor shall develop quality control procedures addressing the critical areas identified in this statement of work. The government will periodically evaluate the contractor’s performance in accordance with the statement of work.

11. SUBSTANTIAL COMPLETION AND FINAL INSPECTION: Contractor shall submit a written request that the project is complete and request a final inspection. The COR will complete an inspection within 3 days of receipt of request.

· If the COR determines that the work is not complete, he will immediately notify the Contractor, in writing, stating the reasons. After completing work, the Contractor shall resubmit a request for another final inspection.

12. POST-CONSTRUCTION INSPECTION: The COR and the contractors project manager shall conduct the walk around to do a final inspection. Any, and all deficiencies shall be corrected before the National Park Service will accept the project.

13. ACCEPTANCE OF THE WORK: After all deficiencies have been corrected, and work has been accepted by the Government, payment will be made in accordance with the Schedule of Values.

14. MEASURE AND PAYMENT: Payment will be included under the lump sum price for this project

CONTRACTED SERVICES STATEMENT:

Contractor’s employees, performing services under this award, shall be controlled, directed, and supervised at all times by the assigned project manager. Contractor’s management staff shall ensure that employees properly comply with the performance work standards outlined in the scope of work, specifications, and contract documents. The tasks, duties, and responsibilities set forth in the Statement of Work, and Specifications may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal policy, obligating the appropriated funds of the United States Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee, or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR).
