• A complete building inspection to be conducted monthly.
When inspecting a "portion" of the building, the inspection should include at the minimum one room of each type, such as: rest room, office, corridor, lobby, etc.
Inspections by the contractor shall be conducted after completion of work and before use by the occupants of the area.
5.2 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
This contract is a performance-based service contract wherein monthly payment is dependent on consistent, satisfactory completion of all work requirements. If performance is unsatisfactory or work is not completed per specifications in this Quality Assurance Plan, a percentage of the Contractor’s monthly payment may be withheld. A QASP Evaluation Worksheet has been developed for use by the COR for the cleaning schedules addressed in this contract. The plan is divided into two parts:
1. An informational section that identifies the work requirements; a frequency schedule in which each required service is to be performed, and a percentage-of-pay value.
2. An evaluation section that serves as the form used for each inspection to record satisfactory or unsatisfactory completion of key requirements. This form will also be used to summarize the results of inspections on a monthly basis and recommend if there are to be any monthly deductions.
5.3 PERFORMANCE EVALUATION PROCESS
Inspections of the Contractor’s work for compliance with this SOW will be conducted throughout the performance period of this contract. If contractor performance is deficient, the course of action is dependent on the severity, impact, and/or frequency of the non-performance. Rework is the first action to resolve deficiencies. When rework has been completed by the Contractor, the COR will inspect to verify the work has been completed and accepted. The Contractor shall not consider the task complete until all of the deficiencies have been corrected. Deficiencies as well as acceptable and high quality results, and unusual conditions or equipment failures will be documented by the COR.
5.3.1 Weekly visual inspections will be performed to ensure compliance with the performance standards. A QASP
Evaluation Worksheet will be used by the COR for each inspection that identifies deficiencies. The
Government may exercise its authority to perform inspections at any time, including immediately following the Contractor’s nightly cleaning. Three unsatisfactory performance ratings in any category in consecutive months or five written unsatisfactory performance ratings in any category within one year shall be considered evidence of noncompliance with the contract and grounds for removal of the Contractor Personnel or termination of the contract.
5.3.2 At the end of the billing period (typically one calendar month), the COR will summarize the results of the month’s inspections, calculate any recommended adjustments to the Contractor’s scheduled payment for that month, and submit the calculations to the CO with the Contractor’s invoice and supporting QASP Evaluation
Worksheet.
5.4 DEDUCTION PROCESS
The Government will record and communicate to the Contractor utilizing the QASP Evaluation Worksheet all instances of substandard performance, whether through inspections, direct observation, customer complaint or any other mechanism.
5.4.1 If one or more complaints are received regarding the same defect during one service cycle, the complaint will be considered one complaint. Upon notice of a defect, utilizing the Quality Assurance Surveillance Plan
(QASP) Evaluation Worksheet, the Contractor will be given the specified time to correct the defect. If the defect cannot be corrected, the defect stands. If the corrective action is not completed within the specified time (7 working days), a deduction will be applied each service cycle until the corrective action is completed.
5.4.2 Each major element has a corresponding item in the QASP Evaluation Sheet. Each element is assigned a percentage value based on the government’s determination of the relative value of that particular service item. Each element is rated based on the criteria stated in the contract. The COR will evaluate the services provided, and calculate unsatisfactory or missed performance. Each item rated unsatisfactory will receive the stated deduction element for that week.
5.5 INVOICING SCHEDULE
Invoices shall be submitted the first part of the month for services rendered during the previous month.
Vendor is to submit an attachment to the IPP invoice. Attachment shall reference the dates of service, and a description of services provided. Invoice shall be prorated accordingly for service not provided as scheduled.
5.6 REDUCTION OF INVOICE FOR CONSTRUCTION ACTIVITY
Invoices shall be reduced for construction activity in any area that is included in this contract. During construction activity, the portion of the building undergoing construction is under control of the Contractor performing the work and the Contractor will be responsible for cleaning the construction area. The invoice shall be reduced by the average cleaning cost per square foot for the contract times the square footage of the area under construction times the fractional portion of the month that area was under construction. For
Example, if the average cleaning cost per month is $0.80 per square foot, and 3500 square feet of a building is under construction for 24 days when no cleaning is required, the monthly invoice would be reduced by:
($0.80/SF x 3500SF x 24days/30days) = $2,240. In this example, for that month with construction activity, the monthly invoice would be reduced by $2,240. The COR will determine the invoice reduction for construction activity in a building.
