Addendum to FAR 52.212-1 Instructions to Offerors
Submission of Offers: Offers shall be submitted by the due date and time provided on the SF1449 block 8 to Mindie Dixon at mdixon@usgs.gov.
One (1) electronic copy shall be submitted as follows:
Tab A – Offer Form (Standard Form 1449) - original signature and date; Contract Administration Data; UEI; Acknowledgement of Solicitation Amendments if applicable.
Tab B – Pricing-
· On the SF 1449 insert proposed unit price in the -UNIT PRICE- section within the -ITEM- and multiply the unit price times the quantity to complete -AMOUNT- in the Supplies or Services and Prices/Cost section of the solicitation for each Contract Line-Item Number (Item).
Tab C – Completed copy of FAR 52.212-3 and 52.204-24 if not updated in SAM.gov.
Tab D – Evaluation Factor Responses:
Factor 1: Technical Capability
i. Page Limit is 3 pages.
ii. Technical documentation
a. Identify the product by brand name, manufacturer, and model number
b. Compliance with salient characteristics
iii. If an authorized distributor/reseller provide relevant documentation.
