STATEMENT OF WORK

Speech and Language Services

Tonalea Day School School, 160 US Highway and Route 21, Tonalea, AZ is in need of a contractor to provide Speech and Language Evaluation and Screening through indirect teletherapy and/or direct in-person services based on the student needs beginning August 1, 2023.

Services will be conducted through indirect teletherapy and/or direct in-person for the students who have been identified and found eligible as speech and/or language impaired and for any others who may transfer in or be referred, assessed and determined to be in need of these services. Services are provided per P.L. 105-17 and 34 CFR, Part B.

THE CONTRACTOR WILL:

1. Provide Speech and language services through indirect teletherapy and/or direct in-person to Tonalea Day School for 290 hours from 7/1/19 23 through 7/31/2024. Preferably contractor will be available during school operation hours, however, due to schedule conflicts, school breaks, mandatory testing, etc. this may not always be possible and indirect teletherapy and/or direct in-person will have to be scheduled around these events.

2. Evaluate and screen students that are due to have Speech/Language re-evaluations or students that have recently been referred for individual evaluation within 30 days after the parents sign the permission to evaluate;

3. Prepare and provide a comprehensive report (3 – 5 pages) to Tonalea Day School, P.O. Box 39, Tonalea AZ 86044 within 30 45 days after the evaluation takes place;

4. Conduct a Multidisciplinary Team Meeting where the results of the evaluation are explained and discussed with the general education staff, Special Education staff and parents, appropriate placement is agreed upon and, if applicable, goals and objectives for therapy are discussed and Individual Education Plan is written.

5. Provide and use standardized testing instruments;

6. Provide indirect teletherapy and/or direct in-person services in a small group or

1:1 setting depending on the Speech Language needs of the student with the speech and language technician, modeling therapy techniques that the technician should employ during scheduled therapy sessions with the identified students weekly during scheduled indirect teletherapy and/or direct in-person visits;

7. Provide technical assistance and training to the education technician who works with the speech-impaired students during the times when the contractor is not present;

8. Provide Speech/Language in-service to the Special Education staff to explain diagnostic summary;

9. Provide training in Augmentative Language and Assistive technology if found needed by any student;

10. Provide initial Speech/Language Augmentative Communication Services according to IEP’s;

11. Provide initial Speech/Language Augmentative Communication Evaluation to determine present levels of communicative functioning;

12. Provide a laptop and headphones for indirect teletherapy sessions.

13. Be aware that once the award is made, additional service days, assessments, etc.

will not be allowed without first receiving the modification approval from the contracting office.

KEY PERSONNEL MINIMUM REQUIREMENTS

The contractor shall provide key personnel that meet or exceed the following minimum requirements. Evidence of meeting these requirements shall be provided to the Government with the Contractor’s quote and Replacement of key personnel will require prior coordination and approval of the school.

Education: Masters Degree in Speech Language Pathology

# of Years of directly Related professional experience: At least five (5) years experience with Native American students is required.

Certification(s) Certificate of Clinical Competence Speech-Language Pathology (CCC-SLP) preferred.

License(s) Arizona State License (License in state where services are to be provided.

Security Requirements New consultants shall comply with Background Investigation as required prior to commencement of any work to be performed. 25 – Chapter 34- Section 3201 (P.L. 101-630); contact with or control over Indian children are subject to a background investigation and determination of suitability for employment. Contractor personnel shall present Government PIV issued identification card to the front desk and sign-in with the School Administrative Office prior to entering the school classroom.

Contractor’s personnel are subject to background investigations and must successfully pass the background prior to performance under the contract.

Contractor must provide name of the service provider who will be assigned to Tonalea Day School.
