Attachment No. 2
Statement of Work
For
Installation of Athletic Equipment
Turtle Mountain Middle School Weight Room
1. INTRODUCTION
The Bureau of Indian Education (BIE) Turtle Mountain Middle School has a requirement for delivery and installation of Athletic Equipment.
2. PLACE OF PERFORMANCE
Services shall be performed at BIE Turtle Mountain Community Middle School, 1330 Braves Boulevard, Belcourt, ND 58316. Location of equipment is the
School’s Weight Room located downstairs.
3. SCHEDULE FOR SERVICES
Installation of equipment shall be coordinated with School’s principal. Services shall be performed Monday through Friday, excluding Federal Holidays. If work must be scheduled after hours or during weekends, prior coordination and approval is required.
All work shall be scheduled through the Contracting Officer’s Representative (COR).
4. SCOPE OF WORK
The Contractor shall furnish sufficient technical and supervisory personnel to ensure the accomplishment of the work. Additionally, the contractor shall furnish all services, materials, equipment, tools, and travel required in connection with this work.
On-Site Assessment: Contractor shall verify all items of work and notify the COR of any discrepancies or concerns prior to performing any work. Prior approval is necessary for such a visit.
All areas that are used for access shall be kept clean and any damage to these areas shall be repaired to match the original condition. When the work is completed, all surfaces in the work area shall be restored to the original condition.
4.1. DUTIES AND RESPONSIBILITIES:
Contractor shall assemble all athletic equipment and place in weight room. Coordinate with School’s principal of placement of equipment.
Contractor is responsible for unpacking and disposing of packaging boxes, plastic, etc.
Contractor shall ensure flooring is clean and ready for installation of athletic equipment.

Contractor shall coordinate the removal of any items that might obstruct the location of equipment to be installed.
4.2.TESTING AND INSPECTIONS:
Contractor shall inspect and test equipment to ensure equipment is working properly.
Contractor shall provide a minimum of three (3) years of preventative maintenance, support services for equipment specified. Services shall be performed by an experience qualified technician.
4.3. DELIVERABLES/REPORTS
The Contractor shall provide the following deliverables/reports to the COR after service:
A. Product manuals, warranty information, service maintenance logs.
5. UTILITIES
The Government will furnish to the Contractor all electricity for maintenance purposes at no charge to the Contractor. All electricity shall be carefully conserved.
6. CLEARANCE REQUIREMENTS
A. Personnel: All Contractor personnel working onsite shall initiate and successful pass a
Department of Interior (DOI) Access/e-Qip background check. Employee (s) will require an escort until background clearances have been approved. The Contractor shall provide nametag/identification badges for all employees working onsite. All employees shall sign-in/out at the front desk prior to beginning service and at the end of service.
B. Vehicles: All Contractor vehicles requiring entry onto school property shall be marked with the company logo. All vehicles must have current registration and proof of insurance.
7. WORK PLACE SAFETY
The Contractor shall be responsible for ensuring a safe working environment while onsite. The
Contractor shall comply with all applicable Federal laws and with such additional measures as the
Contracting Officer may find necessary. The Contractor is required to implement a ZERO
INJURY PROGRAM for this project.
Contractor is responsible for 3% TERO Fees with the Turtle Mountain Band of
Chippewa Indians
