STATEMENT OF WORK

Physical Therapist

1.0 BACKGROUND /PURPOSE

The Cheyenne-Eagle Butte School is a Bureau of Indian Education (BIE) operated school located in Eagle Butte, South Dakota. As a BIE operated school, Cheyenne- Eagle Butte School is responsible for providing a high quality general education and Special Education programming. The Cheyenne-Eagle Butte School-Special Education Department is seeking a State of South Dakota Physical Therapist that are able to provide long term service for the Cheyenne-Eagle Butte School. The Physical Therapist will assist with the completion of Physical therapy evaluations and prepare written reports. Based upon the evaluations the Physical Therapist will prescribe therapy goals to meet individual student needs. The purpose is to provide continuing uninterrupted therapy to students already identified as needing physical therapy.

1.1 SCOPE OF WORK

The service provider shall provide all personnel, supervision, materials and other items and services necessary to effectively evaluate and provide therapy to the students at the Cheyenne-Eagle Butte School-Special Education Department In addition the service providers will have the necessary medical licensure as required by the State of South Dakota. In addition the Physical Therapist must hold National Certification from the Osteopathic Medical Examiners. It is imperative that the Physical Therapist i s well versed in State of South Dakota and Federal Special Education laws, rules, regulations and eligibility requirements.

1.2 SERVICE PROVIDER PERSONNEL

The purpose of this contract is to acquire the services of a South Dakota certified Physical Therapist. It is imperative that the Physical Therapist possesses state certification at time of application. The Physical Therapist be licensed through the South Dakota Board of Medical and Osteopathic Examiners. In addition the Physical Therapist must also be licensed by the State of South Dakota Board of Osteopathic Medical Examiners. The therapists must have extensive experience working on a reservation or in reservation schools. Experience should consist of experiences that provide a minimum of four years experience working with Native Americans or Native American Students on a reservation o r i n a r e s e r v a t i o n schools in South Dakota.

The service providers must be able to provide 2 days per week on site when school is in session. It is imperative the contractors be able to work collaboratively with the Special Education Director/School Supervisor, school staff, students and parents. Upon notification from school required to be available for consultation when not on-site. The
