14. DESCRIPTION OF AMENDMENT/MODIFICATION (Organized by UCF section headings, including solicitation/contract subject matter where feasible.)
The purpose of this amendment is to answer questions posed by Offerors.
1) Question: Please provide the rest of the sentence on page 27 K-8(c) of the REVISED RFP
(01/23/2013).
Answer: Any identification of limited rights data or restricted computer software in the offeror's response is not determinative of the status of the data should a contract be awarded to the offeror.
2) Question: In reviewing the Business Proposal Instructions (Page 31) under L.5.1 it refers to Table 15-2 of FAR Part 15. We noted that SF1411 Form captured all of the required

information however, SF1411 is obsolete (see attachment). Is there another approved form that captures the required information?
Answer: No. Please provide the required information under L.5.1.
3) Question: Page 35 L.13 - Our proposed contract total will exceed $650,000 for the five year period. We are unclear if the small business subcontracting plan applies and how it may apply.
Page 73 - Attachment H - appears to be a recommendation only?
Answer: Not a recommendation, it is a requirement of the RFP.
4) Question: Page 52, Task 9-D: in the current contract with the County of San Diego, the language was amended from "The contractor shall" to "The contract may"Will this still apply to the next contract if awarded?
Answer: No. The word "shall" will be retained.
5) We have an issue now that I need your help in navigating. The COR at NHDP stated that they cannot be used as a reference. My impression from you was that they could. I did not send the forms to her at the NHDC because of this. I was told that other organizations or even patients should be contacted and that the forms in question dont exactly apply to patient care situations and that a letter of reference may be sent instead.
Therefore when I contacted my references I gave them the option of writing a letter of reference or filling out the form writing not applicable where necessary.
Answer: The Past Performance Survey must be used.
The Contracting Officer Representative (COR) can submit a Past Performance Survey for any Offeror.
6) Question: Request clarification for Section
L.5.2:
c. It is HHS policy that contractors provide all equipment and facilities necessary for performance of contracts. Should equipment and facilities be listed including that of our sub recipients?

Answer: No
7) Question: The Offeror must identify all Government-owned property in its possession and all property acquired from Federal funds, to which the Offeror has title that is proposed to be used in the performance of this contract.
Should we distinguish between HD purchased items/ state-purchased, state-leased/sub recipient purchased and/or lease equipment?
Answer: No.
8) Question: Is the budget format used in prior years a good method for this proposal?
Response: Not sure of the budget format used in prior years. Budget format shall contain the cost information requested in the RFP on page 31.
9) Question: Are we allowed to add a 3% inflation rate?
Answer: The Offeror may propose an escalation rate in accordance with the Consumer Price Index
(CPI).
10) Question: Should the budget reflect proposed costs for one year or for all the years combined?
Answer: The proposed budget should reflect the cost for the base year and each option year separately.
11) Question: Do we base the costs on the clients they are projecting to serve or do we continue to use the actual $ spent as a base since it would be hard to break down clients served with the products purchased for them?
Answer: The Offeror shall determine and base costs on the tasks required.
12) Question: After patients complete medication, how long is the follow-up period for lepromatous and tuberculoid patients. Is there a criteria to determine eligibility for follow-up services? If so, what are they?

after Completion of Treatment, and pg. 6, "Schedule of Follow-Up Visits" provides guidelines for services post treatment.
13) Question: There are patients that continue to experience disability due to Hansen's disease such as reactions, wound care, orthotics, podiatry, occupational and physical therapy services. Will funds support these services throughout that patient's disability and need for ongoing care?
Answer: These are all aspects of care which the
Offeror must factor into their proposed budget.
These contracts are a payer of last resort, so the Contractor shall have insured patients utilize their medical insurance as applicable.
14) Question: I m following up on the email sent below requesting a response to the attached questions relating to Ambulatory Care RFP
13-258-SOL-00002. It would be helpful to receive a response so we are able to review and submit any follow-up questions before your deadline.
Answer: An amendment will be posted at www.fbo.gov to respond to all questions received.
15) Question: The SOW lists 3 essential personnel, namely a Project Director, MD
(half-time) and RN (half-time).
Does it mean the Project Director is a full time position? This is so critical for us to know because we envision getting a Marshallese person who would also do health education/communication, translation, health literacy, outreach etc. For the MD and RN, can we split the FTE among 2 individuals? How about including an FTE effort beside those 3 - budget or epidemiologist for example?
Answer: The HD contract does not fund full-time personnel; therefore, the Offeror must assess the type of personnel and the amount of time necessary to provide HD services as defined in the RFP.

option in our proposal to keep sending skin biopsies to Baton Rouge or we must seek a private entity?
Answer: It's the Offeror's discretion.
17) Question: If we contracted a private lab, do we still have to send a specimen to Baton Rouge as well?
Answer: No. If the result from a private lab is inconclusive, or a second opinion is needed, you may send it to NHDP Lab at no cost. We do not charge for biopsies.
18) Question: If we are allowed to send the skin biopsies to Baton Rouge, what will the cost be?
Answer: The Offeror shall pay for the mailing.
