Dear Prospective Contractor, This Request for Proposal (RFP) package (13-247-SOL-00108) enclosed explains the Indian Health Services (IHS) requirement for a vendor who can provide materials and services, to Provide, Assemble and Install and Commission New Primary Bulk Oxygen Tank, Vaporizer, Reserve Oxygen Tank, Oxygen Manifold and new Emergency Connection at the Phoenix Indian Medical Center (PIMC) located in Phoenix, Arizona in accordance with the Scope of Work (SOW).at the Phoenix Indian Medical Center (PIMC) located in Phoenix, Arizona in accordance with the Scope of Work (SOW). SCHEDULE OF SUPPLIES/SERVICES (Continued)
A. SCOPE OF WORK:
General
The Phoenix Indian Medical Center is seeking a vendor who can provide materials and services, to Provide, Assemble and Install and Commission New Primary Bulk Oxygen Tank, Vaporizer, Reserve Oxygen Tank, Oxygen Manifold and new Emergency Connection at the Phoenix Indian Medical Center (PIMC) located in Phoenix, Arizona in accordance with the Scope of Work (SOW).
Background
The scope of this project is to provide supplies and services to make the designated replacements to the Bulk Oxygen System in this SOW. Proposed equipment shall meet the specifications outlined in Part 4. See Attachment A for Site and Location Map.
PART 1 GENERAL REQUIREMENTS
A. Each phase of work shall be complete and fully operational within five (5) days of start of work and thirty (30) days of delivery of the Purchase Order.
B. Work can be performed Monday – Sunday anytime at no additional expense to the Government.
C. The contractor shall provide a project schedule to the Project Officer before work commences.
D. The contractor shall inform the Project Officer of any interruption of utility service 3 business days in advance of such interruption.
E. Contractor shall ensure proper advance notice, at least five working days in advance, of commencement of work.
F. Contractor shall inform all personnel working on this contract of basic PIMC Hospital emergency procedures.
G. All work shall be accomplished utilizing the following codes for reference;
OSHA 1910.104 (Occupational Health and Safety) NFPA 99 (1999) (National Fire Protection Association) NFPA 101 (2000) (National Fire Protection Association) NFPA 50 (2001) (National Fire Protection Association) CGA (Compressed Gas Association)
H. Utilities Services: Maintain existing medical oxygen utility services for Medical Center at all times. Provide temporary labor, materials, equipment, connections, and backup medical oxygen supply to assure uninterrupted services. Where necessary to

cut existing medical gases, or conduits, wires, cables, utility services, or of master alarm monitoring and signaling systems shall be re-installed at suitable places where required by NFPA 99 1999; recommended by the manufacturer; or where directed by the Project Officer.
I. Medical oxygen or Medical Gas Master Alarm monitoring and signaling systems shall not be interrupted without prior approval of Project Officer.
J. Electrical: When an electrical outage cannot be accomplished, work on any energized circuits or equipment shall not commence without the Project Officer prior knowledge and written approval.
K. Contractor will be notified (in writing) of approval of request, or of which other date and/or time such interruption will cause least inconvenience to operations of Medical Center. Interruption time approved by Medical Center may occur at other than Contractor's normal working hours.
L. Contractor shall:
1. Keep roads, walks and entrances to grounds, to parking and to occupied areas of buildings clear of construction materials, debris and standing construction equipment and vehicles. Wherever excavation for new utility lines cross existing roads, at least one lane must be open to traffic at all times.
2. Method and scheduling of required cutting, altering and removal of existing roads, walks and entrances must be approved by the Project Officer.
M. The Contractor shall protect from damage all existing improvements and utilities at or near the work site and on adjacent property of a third party, the locations of which are made known to or should be known by the Contractor. The Contractor shall repair any damage to those facilities, including those that are the property of a third party, resulting from failure to comply with the requirements of this contract or failure to exercise reasonable care in performing the work. If the Contractor fails or refuses to repair the damage promptly, the Contracting Officer may have the necessary work performed and charge the cost to the Contractor. (FAR 52.236-9)
PART 2 QUALIFICATIONS:
2.1 Contractor shall document experience installing similar make and model equipment/systems in a Healthcare environment over the past 3 years. Such information shall be submitted with bid. This requirement shall be fulfilled by the provision of the following items:
A. Names of three firms for which the contractor has installed the type of equipment outlined in this contract.
B. Include the name of each firm along with a name and a telephone number of a management official that can be contacted to verify the contractor’s past experience in installing, servicing and repairing similar equipment and/or systems.
2.2 Only Medical Gas Certified individuals will be authorized to braze medical gas piping.
Contractor shall be required to document the background experience that qualifies his/her employees to perform the work of this contract.
PART 3 GENERAL INTENTIONS
3.1 Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, furnish supervision, labor and materials, and perform work for installation and third party commissioning of a new 1500 gallon Bulk Liquid Oxygen Storage Tank, disconnection, refurbishment, and re-installation of the exiting 525 gallon Bulk Liquid Oxygen Storage Tank for use as the reserve, replacement of alarm initiating devices and level controls, provide for the installation of new atmospheric tank mounted vaporizer of equal service size and installation of a common oxygen service fill connection sized for the main storage tank.
3.2 Contractor shall provide and install a new emergency oxygen connection at location indicated (Attachment A). This installation will be located at the North West corner of the Hospital facility in a protected enclosure. Additional system isolation valves will be necessary at termination points. In addition a 300 Sq Ft. concrete pad will need constructed to ensure the safe operation of a temporary oxygen supply
3.3 Contractor shall provide all supervision, labor and materials for the electrical installation of the project including, but not limited to, electrical conduit, wiring, control/initiating circuits, power, etc. to install new bulk oxygen storage tank, and to re-install existing bulk storage tank (as the oxygen reserve storage tank) and required electrical power/alarm initiating circuits. All electrical work shall be in accordance with the National Electrical Code (NEC) latest edition, and with NFPA 99; Health Care Facilities Code, 1999.
3.4 Contractor shall provide any new concrete pads including those to support bulk storage or bulk deliveries as required as to manufacturer specification and or NFPA and OSHA. Contractor shall also provide replacement protection equivalent to that which will be removed for construction and additional as necessary to protect any new bulk storage tanks.
3.5 Project shall include a 3rd party testing agency certified in medical gas system inspections.
PART 4 SUBMITTALS
4.1 Contractor shall submit samples or documentation of the materials before work commences. The contractor shall receive written approval from the Project Officer or designee before installation.
4.2 Bulk Oxygen Tank
A. Pedestal mounted 1500 Gallon. Factory tested and certified bulk medical oxygen tank.
B. Piping:
a. Copper Tubing: Type "K", ASTM B819, seamless copper tube, hard drawn temper, with wrought copper fittings conforming to ASME B16.22 or brazing fittings complying with MSS SP-73. Size designated reflecting nominal inside diameter. All tubing and fittings shall be labeled "ACR/OXY", "OXY", "OXY/MED", "ACR/MED", or "MED".
b. Brazing Alloy: AWS A5.8, Classification BCuP, greater than 537 °C (1000°F) melting temperature. Flux is strictly prohibited for copper-to-copper connections.
c. Screw Joints: Polytetrafluoroethylene (teflon) tape.
d. Apply piping identification labels at the time of installation in accordance with current NFPA. Apply supplementary color identification in accordance with CGA Pamphlet C-9.
Labels shall be in accordance with NFPA 99 1999, and installed at intervals specified.
e. Multiple (Trapeze) Hangers: Stainless Steel, cold formed, lipped steel channel horizontal member, not less than 41 mm by 41 mm (1-5/8 inches by 1-5/8 inches), 2.7 mm (No. 12 gage. All unistrut, clamps, fasteners, and piping support hardware shall be stainless steel.
f. Medical gas piping clamps shall comply with NFPA 99 Health Care Facilities Code,1999. Pipe supports shall be plastic coated whenever in contact with the tube.
4.3 Refurbishment plan for existing 525 Gallon Bulk Tank for Reserve
A. Refurbish to Factory Specifications.
4.4 Concrete
A. Tank Pad (s) – Sized to meet the requirement of the proposed tank. Including mounting provisions and grounding requirements.
B. Emergency Connection Pad - 300 Sq Ft. concrete pad will need constructed to ensure the safe operation of a temporary oxygen supply
4.5 Emergency Connection
A. At location indicated (Attachment A). This installation will be located at the North West corner of the Hospital facility in a protected enclosure. Additional system isolation valves will be necessary at termination points.
4.7 Protection
A. Fencing - Match and extend existing.
4.8 Valves
A. Ball: In-line, other than zone valves in cabinets:
1. Seventy five millimeter (2 1/2 inches) and smaller: Bronze/ brass body, Fed. Spec.
MSS SP72 & SP 110 , Type II, Class 150, Style 1, with tubing extensions for brazed connections, quarter-turn, full port, three-piece or double union end connections, teflon seat seals, full flow, 4125 kPa ( 600 psi) WOG minimum working pressure, with locking type handle indicating open or closed, cleaned for oxygen use and labeled for intended service
2. Eighty to one hundred millimeter (3-4 inches): Bronze/ brass body, Fed. Spec. MSS SP72 & SP 110, Type II, Class 150, Style 1 with tubing extensions brazed to flanges, quarter turn, full ported, three piece, double seal, teflon seals, full flow, 4125 kPa (600 psi) WOG minimum working pressure, with locking type handle indicating open or closed, cleaned for oxygen use and labeled for intended service.
3. Shutoff valves shall be identified in accordance with NFPA 99, 1999.
A. Check:
1. Eighty millimeter (3 inches) and smaller: Bronze/brass body, straight through design for minimum pressure drop, spring loaded, self-aligning with teflon cone seat, vibration free, silent operation, supplied NPT female threads at each end with flow direction arrow permanently cast into, cleaned for oxygen use and labeled for intended service, 2750 kPa (400 psi) WOG minimum working pressure.
2. One hundred millimeter (4 inches) and larger: Iron body, bronze trim, swing type, vertical or horizontal installation, flange connection, with flow direction arrow permanently cast into, cleaned for oxygen use and labeled for intended service, 1025 kPa (150 psi) WSP.
4.9 Gages
A. Pressure Gages: Includes gages temporarily supplied for testing purposes.
1. For line pressure use adjacent to source equipment: ASME B40.1, pressure gage, single, size 115 mm (4-1/2 inches), for compressed air, nitrogen and oxygen, accurate to within two percent, with metal case. Range shall be two times operating pressure. Dial graduations and figures shall be black on a white background, or white on a black background. Gage shall be cleaned for oxygen use, labeled for appropriate service, and marked "USE NO OIL". Install with gage cock.
2. For all services downstream of main shutoff valve: Manufactured for oxygen use, labeled for the appropriate service and marked "USE NO OIL", 40 mm (1-1/2 inch) diameter gage with dial range 1-690 kPa (1-100 psi) for air.
4.10 Alarms
A. Provide all low voltage control wiring required for complete, proper functioning system, in conformance with NFPA 99 Health Care Facilities Code, 1999 and these specifications.
B. Master Alarm Functions: Provide the following individual alarms at the master alarm panel.
1. Oxygen Alarms:
a. Liquid oxygen low level alarm: Functions when stored liquid oxygen reaches a predetermined minimum level.
b. Reserve switchover alarm: Functions when, or just before, reserve oxygen supply goes in operation.
c. Reserve low supply alarm: Functions when contents of cylinder reserve oxygen supply are reduced to one day's average supply; switch and contacts at the bulk tank control panel.
d. Reserve low pressure alarm: Functions when the gas pressure available in the liquid reserve oxygen supply is reduced below the pressure required to function properly.
e. Low pressure alarm: Functions when system pressure downstream of the main shutoff valve drops below 275 kPa (40 psi), plus/minus 14 kPa (2 psi); operated by pressure switch or transmitters.
f. High pressure alarm: functions when system pressure downstream of main shutoff valve increases above 415 kPa (60 psi), plus/minus 14 kPa (2 psi) set points; operated by pressure switches or transmitters.
g. Cylinder reserve pressure low: Functions when the content of a cylinder reserve header is reduced below one day’s average supply.
4.11 Pressure Regulators
A. For 690 kPa (100 psi) regulator, balanced design, provide duplex in parallel, valve for maintenance shut-down without service interruption. For additional pressures, locate regulators remote from compressor near point of use, and provide with isolation valves and valve bypass.
1. For systems 5 L/s (10 scfm) and below: Brass or bronze body and trim, reduced pressure range 170 – 850 kPa (25 – 125 psi) adjustable, spring type, diaphragm operated diaphragm operated, relieving. Delivered pressure shall vary not more than one kPa (0.15psi) for each 10 kPa (1.5psi) variation in inlet pressure.
4.12 Pressure Switches
General purpose, contact or mercury type, allowing both high and low pressure set points, with contact type provided with a protective dust cover; adjustable range set by inside or outside adjustment; switches activate when indicated by alarm requirements. Use one orifice nipple (or DISS demand check valve) for each sensor or pressure switch.
4.13 Electrical
A. Conduit Size: In accordance with the NEC, but not less than 0.5 in [13mm] unless otherwise shown. Where permitted by the NEC, 0.5 in [13 mm] flexible conduit may be used for tap connections to recessed lighting fixtures.
B. Conduit:
1. Rigid steel: Shall conform to UL 6 and ANSI C80.1.
C. Conduit Fittings:
1. Rigid steel and IMC conduit fittings:
a. Fittings shall meet the requirements of UL 514B and NEMA FB1.
b. Standard threaded couplings, locknuts, bushings, conduit bodies, and elbows:
Only steel or malleable iron materials are acceptable. Integral retractable type IMC couplings are also acceptable.
c. Locknuts: Bonding type with sharp edges for digging into the metal wall of an enclosure.
d. Bushings: Metallic insulating type, consisting of an insulating insert, molded or locked into the metallic body of the fitting. Bushings made entirely of metal or nonmetallic material are not permitted.
e. Erickson (union-type) and set screw type couplings: Approved for use in concrete are permitted for use to complete a conduit run where conduit is installed in concrete. Use set screws of casehardened steel with hex head and cup point to firmly seat in conduit wall for positive ground. Tightening of set screws with pliers is prohibited.
f. Sealing fittings: Threaded cast iron type. Use continuous drain type sealing fittings to prevent passage of water vapor. In concealed work, install fittings in flush steel boxes with blank cover plates having the same finishes as that of other electrical plates in the room.
2. Liquid-tight flexible metal conduit fittings:
a. Fittings shall meet the requirements of UL 514B and NEMA FB1.
b. Only steel or malleable iron materials are acceptable.
c. Fittings must incorporate a threaded grounding cone, a steel or plastic compression ring, and a gland for tightening. Connectors shall have insulated throats.
D. Conduit Supports:
1. Parts and hardware: Stainless Steel.
2. Individual Conduit Hangers: Designed for the purpose, having a pre-assembled closure bolt and nut, and provisions for receiving a hanger rod.
3. Multiple conduit (trapeze) hangers: Not less than 1.5 x 1.5 in [38 mm x 38 mm], 12-gauge steel, cold-formed, lipped channels; with not less than 0.375 in [9 mm] diameter steel hanger rods.
4. Solid Masonry and Concrete Anchors: Self-drilling expansion shields, or machine bolt expansion.
PART 5 INSTALLATION/ PHASING
5.1 Phasing
Phasing: To insure such executions, Contractor shall furnish the Project Officer with a schedule of approximate dates on which the Contractor intends to accomplish work in each specific area of site, building or portion thereof. Arrange such dates to insure accomplishment of this work in successive phases mutually agreeable to PIMC Chief Engineer, Project Officer and Contractor, as follows:
Phase I: Install, inspect and commission new Emergency Oxygen supply including connection, valves, purge ports and concrete necessary in order to maintain facility during construction on balance of the system. The Emergency Oxygen to be supplied by owner.
Phase II: Install, inspect and commission New Bulk Liquid Oxygen Storage Tank to include vaporizers, oxygen system interconnecting piping, manifolds, electrical, alarm initiating devices, anchoring system components, etc., for a complete and commissioned Medical Oxygen Supply system per NFPA 99 Health Care Facilities Code, 1999.
Phase III: Refurbish existing Bulk Liquid Oxygen Storage Tank, reinstall and anchor, provide all utility interconnections, and commission Medical Oxygen Supply systems.
5.2 Installation
A. Keep open ends of tube capped or plugged at all times or otherwise sealed until final assembly.
B. Cut piping square and accurately with a tube cutter (sawing not permitted) to measurements determined at place of installation. Ream tube to remove burrs, being careful not to expand tube, and so no chips of copper remain in the tube. Work into place without springing or forcing. Bottom tube in socket so there are no gaps between tube and fitting. Prevent oil or grease being introduced into tubing. Where contamination has occurred, material is no longer suitable for oxygen service.
C. Spacing of hangers: As code dictates.
D. Rigidly support valves and other equipment to prevent strain on tube or joints.
E. While being brazed, joints shall be continuously purged with oil free NF nitrogen.
The flow of purged gas shall be maintained until joint is cool to touch.
F. Do not bend tubing. Use fittings.
G. Install pressure switches, transmitter and gauges to be easily accessed, and provide access panel where installed above plaster ceiling. Install pressure switch and sensors with orifice nipple between the pipe line and switches/sensors.
H. Apply pipe labeling during installation process and not after installation is completed.
Size of legend letters shall be in accordance with ANSI A13.1.
I. After initial leakage testing is completed, allow piping to remain pressurized with testing gas until testing agency performs final tests.
J. Valve Tags and Lists:
1. Bulk Medical Oxygen Systems: All valves shall be provided with valve tags in accordance with NFPA 99, 1999, and listed on a valve list.
2. Valve tags: In accordance with NFPA 99, 1999. Unless designated otherwise tags will be engraved black filled numbers and letters not less than 13 mm (1/2-inch) high for number designation, and not less than 6.4 mm(1/4-inch) for service designation on 19 gage, 38 mm (1-1/2 inches) round brass disc, attached with brass "S" hook or brass chain.
K. Inaccessible Equipment:
1. Where the Government determines that the Contractor has installed equipment not conveniently accessible for operation and maintenance, equipment shall be removed and reinstalled or remedial action performed as directed at no additional cost to the Government.
2. The term "conveniently accessible" is defined as capable of being reached without the use of ladders, or without climbing or crawling under or over obstacles such as electrical conduit, motors, fans, pumps, belt guards, transformers, high voltage lines, piping, and ductwork.
5.3 TESTING
A. Initial Tests: Blow down, and high and low pressure leakage tests as required by NFPA with documentation.
B. Laboratory and healthcare testing agency shall perform the following:
1. Perform and document all cross connection tests, labeling verification, supply system operation, and valve and alarm operation tests as required by, and in accordance with, NFPA and the procedures set forth in pre-qualification documentation.
2. Verify that the systems, as installed, meet or exceed the requirements of NFPA, this specification, and that the systems operate as required.
3. Piping purge test: For each positive pressure gas system, verify cleanliness of piping system. Filter a minimum of 35 cubic feet (1000 liters) of gas through a clean white 0.45 micron filter at a minimum velocity of 3.5 scfm (100 Lpm). Filter shall show no discoloration, and shall accrue no more than 0.1 mg of matter. Test each zone at the outlet most remote from the source. Perform test with the use of an inert gas as described in CGA P-9 (Compressed Gas Association).
4. Piping purity test: For each positive pressure system, verify purity of piping system. Test each zone at the most remote outlet for dew point, carbon monoxide, total hydrocarbons (as methane), and halogenated hydrocarbons, and compare with source gas. The two tests must in no case exceed variation as specified in Paragraph, Maximum Allowable Variation. Perform test with the use of an inert gas as described in CGA P-9.
5. Source Contamination Test: Analyze each pressure gas source for concentration of contaminants, by volume. Take samples for air system test at the intake and at a point immediately downstream of the final filter outlet. The compared tests must in no case exceed variation as specified in Paragraph, Maximum Allowable Variation.
5.4 COMMISSIONING
Provide commissioning documentation for all inspection, startup, and contractor testing required above and required by NFPA 99 Health Care Facilities Code, 1999.
PART 6 MISCELLANEOUS MANAGEMENT ISSUES:
6.1 Contractor will be authorized to park only in areas designated by the Project Officer. Contractor’s personnel must observe all traffic and parking signs while at the installation.
6.2 Contractor will be allowed minimal use of government space to store supplies or equipment required under this contract. Contractor must bring on site supplies in quantities limited to the work for each site visit.
6.4 Contractor shall be responsible for repair of any damage incurred to the building systems as a result of contractor workmanship.
6.5 Contractor will be responsible for erecting, maintaining, cleaning and disassembly of all necessary Infection Control Barriers required by PIMC Infection Prevention Practitioners. Adhesion to PIMC Infection Control Policy will be expected and enforced. A Pre and Post inspection by Infection Prevention, Project Officer or Designee shall occur at the beginning and end of each phase.
PART 7 PERFORMANCE REQUIREMENTS
7.1 Where there is conflict between one or more code requirements, the more stringent code requirement shall be used to perform work.
7.2 All work shall be performed and completed in accordance with the following codes and standards:
A. National Fire Protection Association (NFPA) B. National Electric Code (NEC) C. Uniform Building Code. (UBC) D. American Standards of Testing Materials (ASTM) E. Occupational Safety and Health Administration (OSHA) 1926/1910 F. American Disabilities Act (ADA) G. CGA (Compressed Gas Association)
When there is a debate between one or more requirements, the more stringent requirement shall be used.
7.3 The contractor shall notify the Project Officer or Designee of any unforeseen conditions or problematic issues regarding performance of this SOW.
7.4 All materials used shall comply with ASTM standards. Citation of ASTM standard used for work may be requested.
7.5 Installation will be inspected for Quality Assurance by the Project Officer or designee, those found to be of poor quality shall be repaired/replaced at no additional cost to the Government.
7.6 Contractor shall deliver two approved completed sets of detailed asbuilt drawings and wiring schematics on a CD in AutoCAD 2010 "dwg" format to the Project Officer within 15 calendar days after each completed phase and after the acceptance of the project by the Project Officer.
7.7 Contractor shall perform the following:
A. Pre-test mechanical and electrical equipment and systems and make corrections required for proper operation of such systems before requesting final tests. Final test will not be conducted unlesspre-tested.
B. Conduct final tests required in various sections of specifications in presence of an authorized representative of the Contracting Officer. Contractor shall furnish all labor, materials, equipment, instruments, and forms, to conduct and record such tests.
C. All related components as defined above shall be functioning when any system component is tested. Tests shall be completed within a reasonably short period of time during which operating and environmental conditions remain reasonably constant.
D. Individual test result of any component, where required, will only be accepted when submitted with the test results of related components and of the entire system.
7.8 Contractor shall perform the following:
A. Contractor shall furnish Maintenance and Operating manuals and verbal instructions when required by the various sections of the specifications and as hereinafter specified.
B. Manuals: Maintenance and operating manuals (four copies each) for each separate piece of equipment shall be delivered to the Project Officer coincidental with the delivery of the equipment to the job site. Manuals shall be complete, detailed guides for the maintenance and operation of equipment. They shall include complete information necessary for starting, adjusting, maintaining in continuous operation for long periods of time and dismantling and reassembling of the complete units and sub-assembly components. Manuals shall include an index covering all component parts clearly cross-referenced to diagrams and illustrations. Illustrations shall include "exploded" views showing and identifying each separate item. Emphasis shall be placed on the use of special tools and instruments. The function of each piece of equipment, component, accessory and control shall be clearly and thoroughly explained. All necessary precautions for the operation of the equipment and the reason for each precaution shall be clearly set forth. Manuals must reference the exact model, style and size of the piece of equipment and system being furnished. Manuals referencing equipment similar to but of a different model, style, and size than that furnished will not be accepted.
C. Instructions: Contractor shall provide qualified, factory-trained manufacturers' representatives to give detailed instructions to assigned Phoenix Indian Medical Center personnel in the operation and complete maintenance for each piece of equipment. All such training will be at the job site. All instruction periods shall be at such times as scheduled by the Project Officer and shall be considered concluded only when the Project Officer is satisfied in regard to complete and thorough coverage. Phoenix Indian Medical Center reserves the right to request the removal of, and substitution for, any instructor who, in the opinion of the Project Officer, does not demonstrate sufficient qualifications in accordance with requirements for instructors above.
B) MATERIALS:
a. All supplies and materials shall be of a type and quality that conform to applicable
Federal specifications and standards. All supplies, materials, and equipment to be used in the performance of work described herein are subject to the approval of the Project Officer. The contractor shall not use any materials, chemicals, or compounds which the Project Officer determines would be unsuitable for the intended purpose or harmful to the equipment, public or employees.
b. Unless otherwise provided in the specifications of this statement of work, materials removed and not reused under the contract shall become the property of the contractor. The contractor shall remove the materials from the work site and dispose of in a manner allowable under Federal, State, County and City laws. Storage or sale of the materials on the site will not be permitted.
C) INDEMNITY AND INSURANCE
The Contractor shall indemnify and hold the government harmless for any and all losses, damages, or liability on account of personal injury, death, or property damage, or claims for personal injury, death, or property damage of any nature whatsoever and by whomsoever made, arising out of the activities of the Contractor, his employees, subcontractors, or agents under the contract. For the purpose of fulfilling his obligations under this paragraph, the Contractor shall procure and maintain during the term of this contract and any extension thereof liability insurance in form satisfactory to the Contracting Officer. The named insured parties under the policy shall be the Contractor, the United States of America, their agents, and their employees. The amount of the insurance shall be not less than $100,000 each person, $300,000 each occurrence and $100,000 property damage. The Contractor shall furnish to the Contracting Officer prior to commencement of contract performance and when the option to extend the term of the contract is exercised, evidence of insurance coverage described herein.

D) SUBCONTRACTING
a. None of the services furnished under the Statement of Work shall be subcontracted or transferred to another Contractor or organization without the specific prior written approval of the Contracting Officer.
b. Nothing contained in the contract shall be construed as creating any contractual relationship between any subcontractor and the Government. The divisions or sections of the specifications are not intended to control the contractor in dividing the work among subcontractors, or to limit the work performed by any trade.
c. The contractor shall be responsible to the Government for acts and omissions of his own employees, and of subcontractors and their employees. He shall also be responsible for the coordination of the work of the trades, subcontractors, and suppliers.
d. The Government will not undertake to settle any differences between or among the contractor and his subcontractors or suppliers.
e. The contractor shall, without additional expense to the Government employ specialty subcontractors where required by the specifications. "Specialty Subcontractors," when specified as a requirement, means a subcontractor regularly engaged in the manufacture or installation of the contract items. The specialty subcontractor shall select and combine the materials involved, maintain and have available for the purpose, workmen skilled in the specified work. The specialty subcontractor shall be the manufacturer, be licensed by the manufacturer as an installer, or work under direct supervision of the manufacturer.
E) SUBCONTRACTING REQUIREMENTS
a. In connection with the performance of work under this contract, the contractor shall not subcontract with any subcontractor who, at the time of subcontract award, is listed on the current GSA's Lists of Parties Excluded from Procurement and Non-procurement Programs, unless otherwise authorized by the Government in accordance with Subpart 9.4 of the Federal Acquisition Regulations.
b. The List of Parties Excluded from Procurement and Non-procurement Programs is available at http://www.epls.gov/.
c. In the event of the contractor's noncompliance with the foregoing requirements, the Government may terminate this contract for default or take other appropriate action, including, but not limited to, requiring the contractor to terminate any such subcontract and substitute an eligible subcontractor in lieu thereof, at no increase in the contract price or time for performance.
d. The term "subcontractor," as used in this clause, shall mean the individual or firm with whom the contractor proposes to enter into a subcontract for manufacturing, fabricating, installing, or otherwise performing work under this contract.
e. Within ten (10) days after contract award, the contractor shall furnish to the Contracting Officer in writing a list of all subcontractors scheduled to perform work under the contract.
f. The contractor shall include the provisions of paragraphs a., b., and d. of this clause in every subcontract hereunder.
F) SITE VISIT:
Site Visit is To Be Determined (TBD).
a. The clauses at 52.236-2, Differing Site Conditions, and 52.236-3, Site Investigations and Conditions Affecting the Work, will be included in any contract awarded as a result of this solicitation. Accordingly, offerors or quoters are urged and expected to inspect the site where the work will be performed. Site Visit is Mandatory.
b. The Contractor shall perform a preliminary inspection and assessment of facility. An organized site visit is scheduled at —

G. PERIOD OF PERFORMANCE:
Contractor shall submit with offer lead times for ordering equipment and time to install equipment. Government requests best possible date.
H. CONTRACTOR'S OBLIGATIONS
The Contractor shall:
(a) Contractor shall furnish own equipment and labor for the job;
(b) Contractor shall wear visible identification provided by the Phoenix Indian Medical
Center Security Staff at all times on the premises of the Phoenix Indian Medical Center compound. The Contractor shall turn in this identification provided by the Phoenix Indian Medical Center Security Staff upon completion of the project;
(c) Provide the services described herein independent of Government supervision.
(d) Provide all services described within the statement of work which may be required by the IHS together with personnel, facilities, equipment, supplies, and other items and expertise necessary for, or incidental to, the provision of such services.
I. MINIMUM CONTRACTOR QUALIFICATIONS
a. All work shall be performed by skilled craftsman that are regularly engaged in work to be performed and shall be at the Journeyman level or directly supervised by a Journeyman.
b. The contractor shall arrange work so as not to cause interference with normal operations at the Indian Health Services facilities located at the Phoenix Indian Medical Center compound. The facility is open from 5:00 am to 6:00 pm. In those cases where some interference is unavoidable, the Contractor must make every effort to minimize the impact of the interference and its effects.
c. None of the services furnished under the Statement of Work shall be subcontracted or transferred to another Contractor or organization without the specific prior written approval of the Contracting Officer.

J. QUALITY CONTROL
a. The Contractor shall provide services under the terms and of this contract with staff capable of providing proper and adequate services and workmanship.
b. The Contractor shall be responsible for monitoring and evaluating the safety, quality and appropriateness of services provided and shall maintain the quality control program.
K. INSPECTION AND COMPLIANCE
a. Compliance Inspection: The Project Officer will, by formal inspection, determine if the specified progress or methods of work are being carried out. Failure to comply with specified work standards may be cause for termination of the Contractor’s right to proceed.
b. Contract Personnel: If the Contractor is absent from the project area, a Contractor’s representative (designated in writing) shall be available for communication on a daily basis while work is in progress.
c. Inspection of Work: Periodic inspection of work shall be made by the Project Officer to ensure compliance with contract specifications. Work not completed to contract specifications shall be reworked at no additional cost to the Government.
d. Determination of Acceptable Work: Inspection as stated above shall be performed by the Project Officer for evaluation of contract specification compliance. The Contractor will be notified in writing of any work not meeting contract specifications.
Portions of the product and installation of work which do not meet contract specifications shall be reaccomplished by the Contractor and the identified items shall be brought up to the required standard(s) before the installation will be considered acceptable.
L. POST AWARD CONFERENCE
a. A post award conference with the successful offeror is required. It will be scheduled within 10 days after the date of contract award. The conference will be held at:
Phoenix Indian Medical Center, Engineering Department unless otherwise changed by mutual agreement.
b. Contractor designated representative or foreman (whomever the Contractor will have on the site) shall be present at the prework conference.
M. COMMENCEMENT, PROSECUTION AND COMPLETION OF WORK (FAR 52.211-
10) (APR 1984)
The Contractor shall be required to (a) commence work under this contract within 60 calendar days after the date the Contractor receives the notice to proceed, (b) prosecute the work diligently, and (c) complete the entire work ready for use not later than TBD (To Be Determined) Estimated start date is TBD (To Be Determined).
N. SCHEDULE
Contractor hours of operation at the above facility shall commence no earlier than 5:00 a.m. and continue no later than 6:00 p.m., Monday through Friday (excluding federally recognized holidays), and 5:00 a.m. and continue no later than 6:00 p.m., Saturday and Sunday or as agreed upon between the Project Officer and the Contractor. O. METHOD OF PAYMENT AND INVOICE SUBMISSION:
a) Payment shall be made in ARREARS;
b) Electronic Funds Transfer (EFT) shall be utilized as method for processing payment;
c) Upon receipt of the Contractor’s properly prepared invoice, the Project Officer is responsible for certifying the invoice for acceptance of service and contacting a UFMS receiver to request a receiving receipt.
d) Improperly prepared invoices will be returned to the Contractor resulting in delayed payment.
The following shall constitute as a “properly” prepared invoice
1. Invoice containing activity with no missed period;
2. Purchase Order number and Period of performance;
3. Fee schedule of line items, description, quantity, unit of price and extended total per line item; Total amount of invoice.
Original invoice shall be submitted to:

Final payment, constituting the entire unpaid balance of the Purchase Order price, shall be made to the vendor when work has been completed.

P. PROJECT OFFICER
Jason Lindner, PIMC Engineering Department, Telephone No. (602) 263-1200 is hereby designated as Project Officer for this service. The Project Officer’s responsibilities will be to coordinate with the contractor the technical aspects of this service and the review of performance hereunder. The Project Officer does not have the authority to change or alter the order amount, terms and conditions.

The Project Officer’s responsibilities include ensuring contractor accountability, monitoring the contractor's technical progress- including the surveillance and assessment of performance and recommending to the Contracting Officer changes in requirements, interpreting the statement of work, performing technical evaluations as required, performing technical inspection and acceptance required, assisting the contractor in the resolution of technical problems encountered during performance and receiving and processing invoices for payment.
