About this file

This document is a Past Performance Questionnaire template for solicitation 1305M326Q0035 concerning annual life raft inspection services. The questionnaire is completed in two stages: the contractor first completes Blocks 1-4 on Page 1 providing contractor information, work performed details, contract specifics, and project description; the completed form is then sent to a client reference for evaluation completion.

Block 1 requires contractor identification including firm name, CAGE code, UEI number, address, phone, email, and point of contact information. Block 2 identifies whether the contractor performed as prime contractor, subcontractor, joint venture, or other entity, with the percentage of project work performed and prime contractor information if applicable. Block 3 captures comprehensive contract data including contract number, delivery/task order number, contract type (firm fixed price, cost reimbursement, or other), contract title, location, awarding agency, client reference contact details, award date, contract completion date, actual completion date, original contract price, and final contract price with explanations of any variances. Block 4 requests a project description assessing work complexity (high, medium, or routine) and explaining project relevance to the current submission, including similar equipment, requirements, and conditions. The client reference then completes the evaluation portion by assigning ratings in six performance categories: Quality (technical standards and problem resolution), Schedule (delivery compliance and resource utilization), Cost Control (price terms compliance, invoice accuracy, and default/termination status), Management (on-site effectiveness, workforce capability, personnel expertise, and requirement flexibility), Regulatory Compliance (contractual terms and reporting requirements), and Overall Customer Satisfaction (staff cooperation, satisfaction, rehire likelihood, and overall performance rating). Each rating category uses a four-point scale: Exceeds (E), Satisfactory (S), Unsatisfactory (U), and Not Applicable (N), with detailed definitions provided. The questionnaire submission email address is kirsten.keen@noaa.gov, and the government reserves verification rights over all information provided.

RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT YOUR EVALUATION OF THE

CONTRACTOR’S PERFORMANCE OF THE SCOPE IDENTIFIED ON PAGE 1

RATING DEFINITION NOTE

(E) Exceeds Performance meets contractual requirements and exceeds aspects to the Government/Owner’s benefit. The contractual performance of the element was accomplished with no problems, and needed no corrective action throughout the term of the contract.

An “Exceeds” rating is appropriate when the Contractor performed all aspects of the scope of work without issue or need for corrective action.

There should be no weaknesses identified.

(S) Satisfactory Performance meets contractual requirements. The contractual performance of the element may have encountered minor problems for which corrective actions taken by the Contractor appear or were satisfactory.

A “Satisfactory” rating is appropriate when there were only minor problems, or major problems that the Contractor recovered from without impact to the contract. There should be no significant weaknesses identified.

(U) Unsatisfactory Performance does not meet contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element contains serious problem(s) for which the Contractor’s corrective actions appear or were ineffective.

An “Unsatisfactory” rating is appropriate when significant events occurred that the Contractor had trouble overcoming, and which impacted the Government/Owner.

Significant weaknesses were identified.

(N) Not Applicable No information or did not apply to your contract.
