About this file
This document is a Past Performance Questionnaire for the National Oceanic and Atmospheric Administration (NOAA) Southeast Fisheries Observer Programs solicitation (1305M325R0054). The questionnaire is designed for vendors to submit references for past performance evaluation, with references required to complete the form by January 10, 2026, either through an attached form or an online questionnaire link. The solicitation covers an Indefinite Delivery Indefinite Quantity (IDIQ) contract for Southeast Fisheries Science Center Observer Programs and Platform Removal Observer Program.
The questionnaire requires detailed information about previous contract performance, including an assessment of seven key performance elements: quality of product/service, schedule, cost control, business relations, management of key personnel, and utilization of small business subcontractors. References are asked to rate performance from "Exceptional" to "Unsatisfactory" across these categories, with space for comments. The document is to be submitted to Jamie Abu Shakrah and Carina Topasna at NOAA's Western Acquisition Division, with specific email submission instructions and formatting requirements.
