About this file

This document is a Request for Information (RFI) issued by Multnomah County Health Department's Behavioral Health Division (BHD) in Oregon. The RFI seeks potential service providers to operate a new 24/7 Sobering & Crisis Stabilization Center, which aims to expand the county's behavioral health and substance use disorder continuum of care. The center, planned for completion in calendar year 2027, will divert intoxicated individuals from local jails and emergency departments by offering a low-barrier first responder drop-off alternative. The RFI is open from 4/9/2025 8:00 AM PDT to 5/2/2025 4:00 PM PDT, with an optional virtual pre-proposal conference scheduled for 4/17/2025 at 12:00 pm. The county is specifically seeking providers who can operate the center, provide withdrawal management services, and offer sobering services (both voluntary and involuntary), potentially through direct service or formal partnerships.

The RFI is a non-competitive information-gathering process to help shape future solicitations, with no contract award resulting from this document. The center will accommodate 50 beds and provide services including 24/7 access to sobering, withdrawal management, and medication-assisted treatment (MAT). The county anticipates a mix of reimbursable and non-reimbursable services, and is interested in understanding potential bed allocation strategies, service integration approaches, and cost structures. County staff will be responsible for intake and triage of deflection participants, while both county and contracted staff are expected to occupy the facility. The RFI includes detailed questions about clinical workflows, service capabilities, potential barriers, coordination mechanisms, and funding considerations for prospective service providers.

Instructions to Proposers

All Questions to be answered in the text boxes provided after each question, except for Question 1.13 which requires a file upload in that question. See Question 1.13 for formatting and other information or requirements.

INTRODUCTION

The Multnomah County Health Department, Behavioral Health Division (BHD) is expanding the behavioral health and substance use disorder (SUD) continuum of care by opening a 24/7 Sobering & Crisis Stabilization Center (Center). The Center, with a goal of project completion in calendar year 2027, will divert intoxicated individuals from local jails and emergency departments by offering a low-barrier first responder drop-off alternative that does not leave individuals in crisis on the streets.

The Center will offer triage and services including 24/7 access to sobering, withdrawal management, and Medication Assisted Treatment (MAT). The Center will also be able to accommodate involuntary sobering holds for less than 24 hours, and will be the hub for the Multnomah County deflection program, which will be managed and staffed by Multnomah County. It is anticipated that both County and contracted staff will occupy the facility, with County staff responsible for the intake and triage of deflection participants only.

The Multnomah County Health Department, Behavioral Health Division is seeking to procure services to:

• Operate, on behalf of the County, the 24/7 Sobering and Crisis Stabilization Center

Description

• Directly provide Withdrawal Management Services

• Either directly, or through formal partnership with another identified provider, provide Sobering

(both voluntary and involuntary)

• Either directly, or through formal partnership with another identified provider, provide MAT

Through this RFI, the County seeks information that will help define what the right level of services would be at the Center, and assist the County to develop future solicitation(s) for those services.

There shall be no monetary exchange or contract resulting from this RFI.

The County, at its sole discretion, may request product demonstrations from any or all qualified respondents.

BACKGROUND

BHD has been tasked by the Board of County Commissioners to open a 24/7 Sobering & Crisis Stabilization Center. BHD is also responsible for operating the County’s deflection program, pursuant to the requirements set forth in HB 4002. The County will purchase a facility out of which both the Sobering & Crisis Stabilization Center and deflection programming will run.

As of mid-October 2024, center-based deflection has been available to eligible individuals between the hours of 7am - 11pm, housed within a short-term leased facility located at 900 SE Sandy Blvd, Portland, OR. In Spring 2025, services will expand to include voluntary sobering and facility operations will become 24/7.

It is expected for existing services, including deflection and voluntary sobering, to transition to the Sobering & Crisis Stabilization Center in the calendar year 2027. In addition to moving existing services to the new Center, services will be expanded to include involuntary sobering, withdrawal management (WM), and medication assisted treatment (MAT), all while retaining 24/7 operations.

Taking into account the expectations above, the County seeks answers to the following questions: (All questions will be answered in the text boxes provided in the Questions section of this RFI).

Program Implementation

1.Would you be able to provide all three services that the county is seeking to procure (MAT, WM, Sobering)? If not, what are the barriers to doing so?

2.Is there any combination of the services you would be able to provide? If so, could you partner with other organizations to provide the remaining service? Please describe.

3.What are some concerns you anticipate that could create clinical or medical barriers to care? Are there any recommendations to mitigate these concerns?

4.How would you collaborate with the county and other partners if you could not perform all of the services (WM, MAT, sobering)?

5. Involuntary Sobering: How would you screen/assess to determine if you could accept an individual on an involuntary hold? How would you determine that the person would likely be stable during a 24-hour hold?

6.If it is determined that the person would not be stable enough to be involuntarily held because they would not be able to be stabilized within a max of 24 hours, what procedures would you have in place to transfer the person to the appropriate level of care?

7.Withdrawal Management (WM): How would you determine when someone comes to the Center for sobering and is in need of withdrawal management? What would a workflow that integrates these services entail?

8.Would your WM program support clients regardless of what substance(s) they are using, or are there any substances that your program would not provide WM for?

9. Medication Assisted Treatment (MAT): How would MAT services be integrated into this continuum of care, which includes WM and Sobering? What type of supportive services would be needed to support clients receiving MAT at the Center?

10. Funding: It is anticipated that there may be some combination of reimbursable and non-reimbursable services provided at the Center. Assuming a total of 50 beds available within the Center, what would be the optimal breakdown of beds between reimbursable services (WM, MAT) and non-reimbursable services (sobering, including involuntary) to sustainably operate all of the services?

11.Taking into account your answer to the question above, in your own format, please provide your cost breakdown between the different types of services? For the non-reimbursable services, what would be the funding needed to provide those services taking into account staff-, operational-, and facility-related costs? Label the cost breakdown as Attachment A and upload here. Attachment A will be limited to three (3) pages.

12. Coordination with MultCo’s Deflection Program: How would you coordinate your service delivery with the County, knowing that coordination with the County’s deflection program is necessary?

Multnomah County is required to disclose non-exempt public documents pursuant to Oregon public records law. Information provided to this RFI should not include material determined to be confidential.
