CHECK & FILL IN ALL APPLICABLE BLANKS BELOW
__ The requested material or service represents the minimum requirements of the government.
n/a
__ A patent, copyright, or proprietary data and limits competition.
n/a
X Specific characteristics of the material or service that limit the availability to a sole source (unique features, function of the item, etc.). Describe in detail why only this suggested source can furnish the requirements to the exclusion of other sources.
The unit must be compatible in all aspects (form, fit, and function) with existing NIDEK RT-5100 systems presently installed across Navy Medicine Operational Training Command. The NIDEK
RT-series refraction system was previously selected for mass acquisition across Navy Medical spaces to ensure standardization across Navy Optometry/Ophthalmology. All Navy optometry providers are trained on this equipment, fostering efficiency and decreasing likelihood for errors.
This is also the preferred model for training AMOs as this is what they will utilize in MTFs and onboard Navy vessels. Marco Ophthalmic is the sole authorized distributor in the United States of
NIDEK products.

X Other information to support a sole source buy:
The existing RT-5100 units were purchased in 2008 and are beginning to fail intermittently. The
RT-6100 is the direct replacement system for existing RT-5100 equipment and will also seamlessly integrate with our medical processing and with existing Marco Tonoref II Auto
Ref/Kerato/Tono/Pachymeters.
I CERTIFY THAT STATEMENTS CHECKED, AND INFORMATION PROVIDED ABOVE, ARE
COMPLETE AND CORRECT TO THE BEST OF MY KNOWLEDGE. I UNDERSTAND THAT THE
PROCESSING OF THIS SOLE SOURCE JUSTIFICATION PRECLUDES THE USE OF FULL AND
OPEN COMPETITION.
