All items are to be provided and all services are to be performed in strict accordance with this solicitation and the attached Performance Work Statement.
INSTRUCTIONS TO QUOTERS
FAR provision 52.212-1, Instructions to Offerors – Commercial Items (Jul 2021), applies to this acquisition. As prescribed in FAR 12.301(b)(1), the following addendum is provided for this solicitation and hereby amends any language therein:

QUOTATION PREPARATION INSTRUCTIONS
To assure timely and equitable evaluation of quotations, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and the identified evaluations factors. Failure to meet a requirement may result in a quotation being ineligible for award. By submission of its quotation the offeror agrees to hold the prices firm for 60 calendar days from the solicitation closing date.
In order to be determined responsive an offeror must submit all documentation to demonstrate the following:
a) Acknowledgement of compliance with all solicitation terms and conditions.
b) Acknowledgement of all solicitations amendments
c) Completion of representations and certifications (FAR 52.212-3 Feb 2021)
d) Return a copy of the file named “14. Prohibition Covered Telecommunication Equipment –
English” duly filled in and signed
e) Proof of System for Award Management (SAM) registration
f) Proof of possessing the qualifications and requisites provided for by art. 38 paragraph 1 of
Legislative Decree no. 81 of 9 April 2008 and subsequent amendments [to include current copy of the self-certification, as communicated to the Italian Ministry of Health in accordance with Italian Ministerial Decree 4 March 2009 (G.U. Serie Generale n. 146, 26 June 2009)] to perform Competent Physician services.
g) One copy of Chamber of Commerce certificate proving the authority of the firm/professional to operate in the specific business sector of the instant solicitation. This document shall not be not older than one hundred and twenty (120) days from the present solicitation posting date.
h) Copy of Documento Unico di Regolarita’ Contributiva (DURC) or equivalent, certifying the payment of host nation security benefits not older than one hundred and twenty (120) days from the present solicitation posting date.
Issuance of purchase order. Quotations should contain the quoter's best technical and price terms.
After the evaluation of quotations, the Contracting Officer may conduct interchanges with one or more quoters of the Government's choice before issuing any purchase order. The Contracting Officer will not negotiate with any quoters other than those of the Government's choice and will not use the formal source selection procedures described in FAR part 15. Quoters may request information on purchase order resulting from this solicitation from the contracting officer.

Questions shall annotate a reference to the specific RFQ element to which they pertain. In addition to the requirements outlined above required to determine responsiveness of prospective quoters, the quote shall consist of two additional separate parts;

PART I – PRIOR EXPERIENCE
The offeror shall provide prior experience information on at least one (1) but no more than three (3) contracts. Such information shall be provided on previous and/or current contracts within the last three
(3) years (from the date set for receipt of quotes) covering a period of at least one year. The prior experience information provided, shall be comprehensive, detailed, to clearly demonstrate the contractor capability to perform the required service.
The quoter shall provide evidence of prior experience in the following format:
a. The title of the contract/project
b. Type of working activities handled
c. Description of activity accomplished: medical surveillance, meetings, trainings, site visits…
d. Length of contract/project (year/month/date – year/month/date)
e. Value of contract/project in Euro (total amount)

PART II – PRICE
Fixed Priced Quotes: the resultant contract will be Firm-Fixed Priced. Multiple awards will not be considered. Accordingly, quoted prices shall be fixed, and not subject to revision and/or negotiation if selected for contract award.
The quote must be submitted for all items, for Base Year plus four (4) Option Periods. The CLINs of this RFQ should be filled in with UNIT and TOTAL amounts. The quote must be submitted in EURO (€).

EVALUATION CRITERIA
FAR Provision 52.212-2, Evaluation -- Commercial Items (Oct 2014) and its addenda does apply to this acquisition. As prescribed in FAR 12.301(c), the following addendum is provided for this solicitation and hereby amends any language therein:
Addendum to 52.212-2 Evaluation - Commercial Items (Oct 2014)
BASIS FOR CONTRACT AWARD: This is a commercial service acquisition under the authority of FAR Part 13.5, Simplified Procedures for Certain Commercial Items. Government evaluation will be conducted on received quotations considering the evaluation factors listed herein. Consistent with Simplified Acquisition Procedures the quotations will be evaluated in accordance with FAR 13.106-2(b).
Comparative evaluations will be utilized whereby the government will compare all quotes that meet the minimum requirements as defined in Addendum to 52.212-1 and 52.212-2. The government will select an awardee whose quote is considered the most advantageous to the government. Therefore, the Government reserves the right to select an awardee that provides benefit to the government exceeding the minimum standards, but is not required to do so.
This selection will be based on an integrated evaluation of the factors as described below.
Part I – Prior Experience in providing certified and credentialed Occupational Health Physician(s) (Medico Competente) services.
Part II – Price
I. Prior Experience evaluation:
Under this part, the Government will evaluate the offeror’s submitted information on at least one (1) but no more than three (3) contracts/projects within the last 3 years from the issue date of subject solicitation, demonstrating that the offeror provided services relevant to those required in the PWS.
Relevant performance includes effort involving similar scope or greater in scope, magnitude and complexity than the effort described in this solicitation.
II Price Evaluation:
The reasonableness of the quotation will be determined on the total price provided for this solicitation.
All prices quoted will be determined by multiplying the quantities by the proposed unit price for each Contract Line Item Number (CLIN) to confirm the extended amount and the total amount. The total evaluated price (TEP) will include all line items (CLINs) plus the 50% of the final option period price to reflect the price of a maximum six month extension.
Quoters should give the best available pricing to the Government for each CLIN in arriving at the most competitive price. Additionally, as this requirement is commercial and will result in a "firm-fixed-price" a comparison of quoters' total price will satisfy the requirement for price analysis, in determining the price fair and reasonable. The Government may determine that an offer is unacceptable if the prices are significantly unbalanced. As referenced in clause FAR 52.217-8, evaluation of options shall not obligate the Government to exercise the option(s).

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)
(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items.
