Coast Guard Station Milwaukee, P/N: 19855042
1.0 GENERAL
The United States Coast Guard (USCG) is seeking a qualified contractor to perform an asbestos abatement and flooring replacement project at USCG Station Milwaukee located at 2420 South Lincoln Memorial Drive, Milwaukee, Wisconsin. Station Milwaukee is a working Coast Guard Station and will remain in operation during the project.
Bidders are responsible for visiting the site to field verify existing conditions and determine actual dimensions and the nature of the work required. Failure to visit the site does not relinquish the bidder from determining the extent and scope of the work required and estimating the difficulty and cost to complete the project.
The USCG Sector Lake Michigan Facility Engineering Officer will be the USCG’s Onsite Representative for this project. Arrange pre-bid site visits to verify existing conditions by sending an email to: Jason.M.Smith@uscg.mil
2.0 BACKGROUND
The USCG Station Milwaukee building (RPUID 47094) was built in 1969 with an addition added in 1981. There is asbestos containing floor tile and mastic in various spaces and corridors on both the first and second floor of the building, and many of the floor tiles are damaged or missing.
3.0 OBJECTIVE
Abate asbestos floor tile and mastic, prepare sub-floor, and install replacement finished flooring at USCG Station Milwaukee.

Spaces where asbestos abatement and new floor installation are to take place are shown by the highlighted areas on the building floor plans in enclosure (1). The approximate area is 2,300 square feet (this is a rough estimate and shall be field verified by Contractor). The following spaces are included in this project:
• First Floor: Lobby, PWAY 01, PWAY 02, Stairwell ST03, Stairwell ST04, Storage closet 1J1. This project does include the carpeted area and stairs in the lobby.
• Second Floor: PWAY 06, PWAY 07, Room 205 (Laundry), PWAY 08, PWAY 09, Stairwell ST03, Stairwell ST04, Storage closet 2J1.
5.1 PROJECT PHASING
USCG Station Milwaukee is an active Coast Guard Small Boat Station that is manned 24-hours a day. The station will remain in operation for the duration of this project; therefore, portions of the project will need to be coordinated with USCG personnel to minimize interference with unit operations. Coordinate timing between successive phases with Coast Guard personnel to allow for necessary relocations.
The station crew conducts daily operations in passageway 04 and the adjacent office spaces, which are not part of this project. There are multiple entrances to the building, and at least one entrance to this area must always remain available.
The south wing of the building is used as barracks and sleeping quarters for on-duty crew members. One floor of the barracks must always remain available for use by the USCG i.e., if the first floor is secured due to abatement activities, the crew shall still have access to and be able to use the rooms on the second floor, and vice-versa.
5.2 HAZARDOUS MATERIAL ABATEMENT
Remove the Asbestos Containing Materials (ACM) identified in the Asbestos Inspection Reports (See enclosures 2 and 3) in the areas listed in section 5.0 and indicated by the highlighted areas in enclosure (1). Materials identified as ACM within the structure include: 12” VCT/VAT floor tile and associated mastics in corridors and other selected areas. The stair treads were not tested for asbestos, but they are assumed to be positive, and shall be abated as well. The carpet in the lobby was not tested for asbestos, but it is to be replaced with new LVP/LVT flooring as part of this project and there may be asbestos tile beneath the carpet.
Proper containment of the abatement areas shall be installed as required to prevent any asbestos from entering areas within the station building that do not have ACM.
Mastic removing solvent must be nonflammable and must not contain methylene chloride, glycol ether, or halogenated hydrocarbons. Solvents used on site must have a flash point greater than 140 degrees F.
Unacceptable Practices: The following work practices shall not be used:
a. High-speed abrasive disc saws that are not equipped with point of cut ventilator or enclosures with HEPA filtered exhaust air.
PAGE 3/8
b. Compressed air used to remove asbestos containing materials, unless the compressed air is used in conjunction with an enclosed ventilation system designed to capture the dust cloud created by the compressed air.
c. Dry sweeping, shoveling, or other dry clean up.
d. Employee rotation as a means of reducing employee exposure to asbestos.
Contractor shall ensure that all work is executed in accordance with all required local, State, and Federal codes. Prior to the start of work, obtain necessary permits and licenses in conjunction with asbestos removal, encapsulation, hauling, and disposition, and furnish notification of such actions required by Federal, State, regional, and local authorities. Notify the Regional Office of the United States Environmental Protection Agency (USEPA) Wisconsin environmental protection agency/Department of Natural Resources, and the Contracting Officer in writing 10 working days prior to commencement of work in accordance with 40 CFR 61-SUBPART M.
Submit copies of all notifications to the Contracting Officer.
All asbestos workers must have a current State of Wisconsin asbestos worker's license. Submit copies of all asbestos workers license/certification to the Contracting Officer.
The Contractor shall ensure that all hazardous waste generated by this project is disposed of in accordance with all local, state, and federal laws and regulations. The Contractor is responsible for preparing all disposal and shipping documentation/manifests, and container labeling, and shall provide copies of this documentation to the Contracting Officer.
5.3 SUB-FLOOR PREPARATION
Contractor assumes all responsibility for preparation of flooring, including any inspections, skim coat layers, bead blasts, scarification, etc. of the exposed subfloor, and any preparations required as a result of the use of solvent mastic removers; on concrete porous surfaces, mastic removing products can leave an oily surface that tends to prevent new mastics from adhering properly, and Contractor shall be responsible for ensuring appropriate floor preparation and condition of the subfloor prior to installation of new flooring products.
Do not install new flooring on surfaces that are unsuitable and will prevent a proper installation.
All substrate to receive new flooring shall be clean and dry and free of any compromising conditions that would prevent or hinder the installation of the flooring. Prepare subfloor in accordance with flooring manufacturer's, mastic manufacturer’s, and adhesive manufacturer’s recommended instructions to ensure the proper installation of the new finished flooring. Repair holes, cracks, depressions, or rough areas using material recommended by the flooring or adhesive manufacturer. Free floor of any foreign materials and clean. Submit one copy of the manufacturer's printed Installation instructions for the flooring, including surface preparation, and recommended adhesives to the Contracting Officer.

I. INTRODUCTION
Wing Three Inc. was retained by USCG to conduct an inspection for asbestos containing materials located in limited areas at:
2nd Floor Living Quarters Hallway 2420 S. Lincoln Memorial Drive
Milwaukee, WI 53207
The sampling inspection included all suspect building materials to determine if asbestos containing materials were present within the space as required by US EPA NESHAPS regulations 40 CFR 61 Subpart M.
II. BUILDING SURVEY
On date of test, Wing Three Inc. conducted sampling of all visible suspect building materials present located in proposed renovation area at:
2nd Floor Living Quarters Hallway 2420 S. Lincoln Memorial Drive
Milwaukee, WI 53207 The visible inspection was comprised of two elements:
1. A visual determination as to the extent of suspect materials within the building.
2. Sampling and documentation of observable suspect materials.
The results of the survey integrated with the Polarized Light Microscopy with Dispersion Staining (PLM/DS) analysis of bulk samples taken are outlined in this document. If you have any questions, please contact Wing Three Inc. @ 262-832-1194.
III. THE LABORATORY
Haye Microbial Consulting 3005 E. Boundary Terrace, Suite F.
Midlothian, VA 23112 804-562-3435 contact@hayesmicrobial.com
IV. METHOD OF ANALYSIS
Analysis is performed by using the bulk samples for visual observation and slide preparation(s) for microscopical examination and identification. The slides are analyzed for asbestos (chrysotile, amosite, crocidolite, anthophyllite, and actinolite/tremolite), fibrous non-asbestos constituents (mineral wool, paper, etc.), and nonfibrous constituents. Asbestos is identified by refractive indices (obtained by using dispersion staining), morphology, color, pleochroism, birefringence, extinction characteristics, and signs of elongation. The same characteristics are used to identify the non-asbestos constituents.
The microscopist visually estimates relative amounts of each constituent using a stereoscope if necessary.
The test results are based on a visual determination of relative volume of the bulk sample components.
The results are valid only for the item tested. Current US EPA NESHAP regulations state asbestos materials means material containing more than 1% asbestos as determined using the method specified in Appendix E, Subpart E, 40 CFR Part 763 Section I, Polarized Light Microscopy. Refer to 29 CFR 1926.1101 Construction) and 29 CFR 1910.1001 (General Industry) for specific OSHA requirements.
V. SUMMARY
The following materials were found to contain asbestos:

Analysis Details All samples were received in acceptable condition unless otherwise noted on the report. This report must not be used by the client to claim product certification, approval, or endorsement by AIHA, NIST, NVLAP, NY ELAP, or any agency. The results relate only to the items tested. Hayes Microbial Consulting reserves the right to dispose of all samples after a period of 60 days in compliance with state and federal guidelines.
PLM Analysis All Polarized Light Microscopy (PLM) results include an inherent uncertainty of measurement associated with estimating percentages by PLM.
Materials with interfering matrix, low asbestos content, or small fiber size may require additional analysis via TEM Analysis.
TEM Analysis Analysis by TEM is capable of providing positive identification of asbestos type(s) and semi-quantitation of asbestos content.
Definitions 'None Detected' - Below the detected reporting limit of 1% unless point counting is performed, then the detected reporting limit is .25%.
New York ELAP Per NY ELAP198.6 (NOB), TEM is the only reliable method to declare an NOB material as Non-Asbestos Containing.
Any NY ELAP samples that are subcontracted to another laboratory will display the name and ELAP Lab Identification number in the report page heading of those samples. The original report provided to Hayes Microbial Consulting is available upon request.
