STATEMENT OF WORK
Yellow Cliffs Water System Decommissioning and Toilet Building Demolition
Tonto National Forest Cave Creek Ranger District
• SCOPE OF WORK
This project has two objectives. First, to decommission and demolish the water supply system at the Yellow Cliffs recreational site on Bartlett Lake. The water system consists of a control room with a solar pump, one 5,000 gallon fiberglass storage tank, Industrial filter, water line connections, PVC waterline and fencing surrounding the control room.
The well head is located 130 feet from the control room as detailed in the attachments included. The existing waterline shall be drained and abandoned in-place by capping water lines below grade at hydrants. Electrical lines below grade shall be abandoned in-place. All detritus associated with the demolition of the site shall be disposed of outside of Forest Service land. Access to the site is provided by an unmarked, unpaved road off of Upper Bartlett Lake Road.
The second objective is to remove three toilet buildings located at the Yellow Cliffs recreation site along with their concrete footings. These toilets will be replaced by double pre-cast concrete vault toilets. The new toilets are being purchased by the government and are not included with this contract. This objective consists of three phases: 1) Demolition 2) Site preparation for new toilets 3) Site work after toilets are placed. Each of the toilet buildings is paired with a 3,000 gallon concrete septic tank that is to be filled with sand furnished by the contractor after it is pumped of its contents.
• LOCATION
The project is located at Bartlett Lake on FR 459 (Upper Bartlett Lake Road) approximately 20 miles east of Cave Creek, Arizona on the Cave Creek Ranger District of the Tonto National Forest. Driving time is approximately 1.5 hours from Phoenix, Arizona.
• SUBMITTALS
Provide the following:
NESHAP Demolition Permits The Contractor shall provide to the Forest Service copies of all Maricopa County/ADEQ and NESHAP demolition permits to the Engineer for review. The Contractor is responsible for air quality testing to ensure compliance as well as any fees required for submittal.
2 | P a g e
Well Abandonment Authorization The Contractor shall provide to the Forest Service a copy of the Notice of Intent to Abandon a Well (NOIA) form and written authorization from the State of Arizona Department of Water Resources granting the abandonment authority. Only a well driller licensed in the State of Arizona, or a single well licensee (a person licensed to abandon one exempt well on his or her own property), may abandon a well in Arizona. Abandonment may not begin until the well driller or licensee has possession of a well abandonment card at the well site, issued by ADWR in the name of the well driller or licensee, authorizing the abandonment of that specific well in that specific location
Well Abandonment Completion:
The Contractor shall provide to the Forest Service a copy of the Well Abandonment Completion Report on file with ADWR within 30 days after completion of well abandonment well that describes the actual methods and materials used to abandon the wells.
• REQUIRED WORK
Objective I
1. Demolition Removal of CMU Control Building (8’x12’) includes concrete floor slab with turndowns and shall be completely excavated and demolished on site prior to disposal off of National Forest Land. If testing identifies the presence of asbestos containing materials (ACM), the contractor shall remove and dispose of the ACM by an asbestos removal contractor.
However, the building was constructed in 2000 and based on this historic knowledge; it is assumed that there is no presence of ACM. Disposal shall meet all State and local laws and regulations and Arizona Department of Environmental Quality. Demolition shall not begin until (10) business days after the contractor has notified Maricopa County or Arizona Department of Environmental Quality of the toilet demolition through a NESHAP. The contractor shall complete and submit the NESHAP after receiving copies of the demolition permit. Once all of the permits and notifications are issued, the contractor shall begin demolition.
Removal of Chain Link Fence (180 LF) Removal of 5,000 gallon Fiberglass Water Storage Tank.
Draining and Capping of PVC Water Line. The existing waterline shall be drained and abandoned in-place by capping water lines below grade at hydrants. Electrical lines below grade shall be abandoned in-place
Decommissioning of Potable Water Well. The wellhead is located approximately 150 feet from the CMU control building. Construction debris resulting from the demolition of the CMU building shall be removed first and in accordance with the project specifications. A Notice of Intent to Abandon a Well NOIA form must be filed with ADWR prior
3 | P a g e to abandoning the wells. The contractor shall file an NOIA form (DWR 55-38) with the ADWR office and pay the appropriate filing fee according to ADWR Instructions. Prior to any well abandonment activity, the Contractor shall submit to the Engineer, written authorization from the State of Arizona Department of Water Resources, granting the abandonment authority. The wells shall be abandoned through filling with grouted cement concrete, bentonite drilling mud, or clean sand with bentonite. The wells shall be sealed with cement grout or a column of bentonite drilling mud of sufficient volume, density, and viscosity to prevent fluid contamination. Materials containing organic or toxic matter shall not be used. The filling and sealing of the wells must prohibit the vertical movement of water both inside the wells and surrounding annular space outside of the casing.
2. Site Work
All voids resulting from removal of components shall be filled with excavated native material from the site or engineered fill provided by the Contractor. Backfilling of the excavated areas shall proceed in eight inch lifts. Each lift shall be compacted by two passes with a whacker compactor. Contractor shall furnish water for compaction that is free of substances detrimental to the work. All disturbed areas shall be flush to existing ground and raked smooth
Objective II

They are based upon the original design data available prior to advertising the project. Original design data include the preliminary survey information, design assumptions, calculations, drawings, and the presentation in the contract. Changes in the number of units of the originally contemplated work will not be made unless authorized under one or more of the following conditions:
(1) As a result of changes authorized by the Contracting Officer in the originally contemplated work. Minor changes to adjust balance points, etc., will not be considered changes in quantities.
(2) As a result of the Contracting Officer determining that errors exist in the original design data used to determine designed quantities, that cause a pay item to change by 15 percent.
(3) As a result of the Contractor submitting to the Contracting Officer a written request showing evidence of errors in the original design data used to determine design quantities that cause a pay item total to change by 15 percent or more. The evidence must be verifiable and consist of calculations, drawings, or other data that show how the designed quantity is believed to be in error.
(b) Actual Quantities (AQ): These quantities are determined from measurements of completed work.
Final payment will be made for the actual quantities at the unit price shown in the SCHEDULE OF ITEMS, subject to the Variation in Quantities Provision.

Note: Executive Order (EO) 13706, Establishing Paid Sick Leave for Federal Contractors applies to all contracts subject to the Davis-Bacon Act for which the contract is awarded (and any solicitation was issued) on or after January 1, 2017. If this contract is covered by the EO, the contractor must provide employees with 1 hour of paid sick leave for every 30 hours they work, up to 56 hours of paid sick leave each year.
Employees must be permitted to use paid sick leave for their own illness, injury or other health-related needs, including preventive care; to assist a family member (or person who is like family to the employee) who is ill, injured, or has other health-related needs, including preventive care; or for reasons resulting from, or to assist a family member (or person who is like family to the employee) who is a victim of, domestic violence, sexual assault, or stalking. Additional information on contractor requirements and worker protections under the EO is available at www.dol.gov/whd/govcontracts.
Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).
The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of ""identifiers"" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).
Union Rate Identifiers
Page 11 of 15beta.SAM.gov
A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers
Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification.
