The contractor is responsible for travel to Eureka, California for local service delivery, with Mt. Shasta available as a backup location for employee travel. Deliverables include documentation of tests provided to employees at the time of service and itemized daily cost and accounting summaries submitted to the Contracting Officer or Contracting Officer's Representative. Data submitted to the government must exclude Personally Identifiable Information (PII), though patients may receive their own PII information. No security requirements apply to this contract.
