About this file
This statement of work outlines asbestos containing material and lead based paint surveys for facilities located within the Carson and Gila National Forests. The contractor will conduct ACM and LBP surveys at nine structures within the Canjilon Lakes Campground in Carson National Forest and one structure at the Kingston Day Use Toilet in Gila National Forest. The contractor must mobilize to each site, perform inspections and sampling by an EPA accredited inspector, analyze samples at an accredited laboratory, and document all findings in separate survey reports for each national forest due by December 31, 2023. The pay unit is lump sum to cover all costs including mobilization, labor, analysis, and reporting. The point of contact for questions is Jennifer Sublett at the provided email and phone.

STATEMENT OF WORK
ASBESTOS CONTAINING MATERIAL (ACM) AND LEAD BASED PAINT (LBP) SURVEYS
CANJILON LAKES CAMPGROUND – CARSON NATIONAL FOREST
KINGSTON TOILET – GILA NATIONAL FOREST
MAY 31, 2023
SCOPE OF WORK
INTRODUCTION
The purpose of this project is to complete a survey to sample and analyze for Asbestos
Containing Material (ACM) and Lead Based Paint (LBP) for the facilities listed below:
Facilities listed are planned for demolition, however care should be taken during the sampling process to avoid damaging and exposing any hazardous materials. Contractor shall repair all sampling holes to match existing material and background until the facilities can be demolished and to prevent any contamination to surrounding materials.
The asbestos and lead-based paint survey and inspection shall be conducted by an EPA accredited inspector. The inspector shall:
• Determine the presence, type, location, and quantities of samples to be taken for any asbestos containing material (ACM) and Lead Based Paint (LBP).
• Take the number of representative samples for each facility.
• Analyze samples at an EPA/State accredited laboratory
• Define the EPA category of that material (friable ACM or Non-Friable Category I or Category II ACM) found.
• Document all findings, photos, laboratory analysis in a report as detailed below.
SITE DESCRIPTION OF WORK:
Below is a summary of the sites to be surveyed for both Lead Based Paint and Asbestos surveys.
Photos are attached in the Appendix for each site.

TASKS
The Contractor shall survey all facilities listed for any suspected asbestos containing materials
(ACMs) and Lead Based Paint (LBP). The Contractor will determine the number of samples to perform a complete analysis of ACM and LBP. Unless otherwise specified, the Contractor shall be responsible for all materials, labor, equipment, supervision, and incidentals required to perform the Requirements/Tasks and Specifications associated with this Statement of Work.
ACM/LBP Survey
Task 1: Asbestos Survey Services
The Contractor shall perform the following tasks:
1) The Contractor shall mobilize to the Site to perform the survey.
2) The survey shall be performed by a professional accredited as an asbestos inspector in accordance with the Asbestos Hazard Emergency Response Act (AHERA) regulations, Title 40 Code of Federal Regulations (CFR), Part 763, Subpart E, Appendix C.
3) The purpose of the survey shall be to locate, inspect, assess, and sample all suspect ACM materials where required. Identify all ACM as to type, condition, percent of asbestos content, and quantity. The inspection shall be performed in accordance with the requirements of 40 CFR Part 61, Subpart M, National Emission Standards for Hazardous
Air Pollutants (NESHAP) and following the AHERA sampling protocol.
4) Standard Survey Components
a) Review existing facility plan in Appendix A and inspection report in Appendix B for reference to asbestos potentially used in construction and recent inspection.
b) Physically inspect and list all suspect materials throughout the designated facility and/or facility components.
c) Sample all suspect materials and submit the bulk samples to an accredited third-party laboratory for asbestos analysis by Polarized Light Microscopy (PLM). Samples will be analyzed by layer. Layers will be determined by the laboratory. Do not assume any materials to be asbestos containing without approval from the COR.
i) Perform Point Count Analysis for all friable samples determined to be ≤5% asbestos-containing by PLM.
ii) For all non-friable organically bound materials (i.e. vinyl asbestos tile, etc.) that are determined PLM to be ≤1% asbestos-containing without approval from the
COR.
iii) All collected samples must be analyzed. Analysis for a homogeneous may not be stopped at the first positive unless specifically allowed by the COR. Report the number of samples in the survey report.
iv) The contractor shall determine the number and location of samples to be collected, up to 12 total samples for the 10 sites.
Task 2: Lead-Based Paint Survey Services
The Contractors shall survey designated facilities and/or facility components for lead-based paint. The tasks expected for the Contractor are listed below.
• The Contractor shall mobilize to the Site to perform the survey
• The survey shall be performed by an Environmental Protection Agency (EPA) accredited inspector and/or risk assessor for lead-based paint inspection in accordance with EPA regulations, Lead: requirements for Lead-based Paint
Activities Final Rule, Title 40, Part 745, Vol. 61 No. 169, pg. 45813, Section 227, subpart L, of the Code of Federal Register.
• The objective of the survey shall be to inventory, locate, and assess the existence of lead-based paint throughout the designated facility and/or facility components, condition, and lead content.
• XRF testing is not required.
• As a minimum, the following components shall be included in the performance of the survey:
o Review existing facility on a surface-by-surface plan and record all painted for reference to lead paint used on surface treatment.
o Throughout the designated facility and/or facility components, physically inspect materials for lead paint.
o Sample/analyze by TCLP for LBP o The locations of Lead-based paint within the building.
o The name of the Inspector/Risk Assessor performing the inspections and copies of current certificates.
o Classification of all surfaces into positive or negative (but not inconclusive) categories paint chip laboratory analysis.
o Specific information on the laboratory methodologies.
o Results of laboratory analysis including chain of custody.
• The contractor shall determine the number and location of samples to be collected, up to 12 total samples for the 10 sites.
ACM/LBP Survey Report
The Contractor shall submit two (2) separate, individual reports: Carson NF ACM and LBP
Survey Report and Gila NF ACM and LBP Survey Report. Contractor shall perform a Survey
