VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K)

Acquisition Plan Action ID:VA245-18-AP-1616

1. Contracting Activity: Department of Veterans Affairs, VISN 05, Washington DC VA Medical Center

Station 688, has requested the purchase of DePuy Synthes screw sets for patient implant. The purchase request applicable to this acquisition is 688-18-1-035-2422.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The screw sets being purchased are manufactured by DePuy Synthes. These screw sets will be implanted into patients by an Orthopedic surgeon. DC VAMC has leased these screw sets in the past, but the VA has a new mandate that each facility must have 2 complete sets of these screw implants in stock at all times. The DePuy Synthes screw sets being purchased will also complete the screw sets on hand at this time at DC VAMC. The estimated amount of this purchase is $75,638.56.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: DePuy Synthes is the manufacturer of the screw sets at DC VAMC. Screw sets can’t be combined by different manufacturers and implanted into patients. There are no authorized resellers or distributors for this product. They must come from the manufacturer.

4. Description of market research conducted and results or statement why it was not conducted:

Market research was conducted. DePuy Synthes does not have a GSA or FSS Schedule. There is an IDIQ DLA contract (SPE2D017D0005) for these supplies; however it states it is for times of conflict or military need. A couple of FBO notices were found and I did some follow up with the contracting specialist/officer. The documentation of the DLA contract as well as the FBO notices and follow up emails are attached to the market research report.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
