Pricing:
Any resultant contract awarded in response to this solicitation is considered a service-type contract. However, because some of the issued task orders will include minor or ancillary construction activities, both the Service Contract Act and Davis-Bacon wage determinations are included in the solicitation and any subsequent contract.
The contract line item (CLIN) pricing shaded in GREEN should be priced in accordance with the applicable Davis-Bacon wage determination.
The un-shaded CLIN pricing should be priced in accordance with the applicable Service Contract Act wage determination.
The Contractor-offered pricing is considered the maximum price for each CLIN. When task orders are competed, the Contractor can elect to offer less than the maximum CLIN price; however, this CLIN reduction is only in effect for that specific task order offer; and does not constitute a contractual change to original CLIN price.
The offeror’s prices for all line items should include all applicable overhead, profit and all burden costs such as, insurance, support costs, compliance with environmental laws, taxes, and protection of government property. If the work will be sub-contracted the price proposed should be inclusive of all costs involved with subcontracting the work.
The North American Industry Classification System (NAICS) code for this acquisition is 237310 and is set-aside for small businesses. Each period on the schedule is one year in length and the Base period begins on the date of contract award. Price will be evaluated by adding the total price for all 4 periods. Service rates and Davis Bacon Wage Determination apply to this acquisition.
BASIS OF AWARD: Evaluation factors other than price, when combined, are significantly more important than price in the award decision. Award(s) will be made to that offeror whose proposal contains the combination of those criteria offering the best value to the government. This will be determined by comparing differences in the value of non-cost features with differences in cost to the government. In making this comparison the government is more concerned with obtaining superior technical or management performance than with making an award at the lowest overall cost to the government. However, the government will not make an award at a significantly higher overall cost to the government to achieve slightly superior technical or management performance. Offers should be submitted initially on the most favorable terms, from a price and technical standpoint, which the Contractor can submit to the Government. The Government may, after evaluation of the proposals, conduct oral or written discussions as appropriate, however award may be made without discussions.
Failure to provide all information requested by the RFP may render the proposal as unacceptable.
Scope and Statement of Work
1.0 General Information:
The intent of this contract is to provide for the complete performance of road maintenance services on the Shawnee National Forest East Side (Hidden Springs Ranger District) and the Shawnee National Forest West Side (Mississippi Blufs Ranger District) in accordance with all terms, conditions, specifications,drawings, exhibits, schedules, maps, provisions and other requirements of the contract. Unless otherwise provided, the Contractor shall furnish all labor, materials, facilities, equipment, tools, supplies, supervision, transportation and incidentals required to perform the work. Measurement and payment for contract work will only be made for and under those pay items included in the Schedule of Items. All other work and materials will be considered as subsidiary or incidental to and included in the payment for pay items shown.
The proposed Contract is for a 12 month Base Period and three 12 month Option Periods. Except where noted, the quantities shown in the schedule are estimates and they apply to the Base Year as well as the Option Years.
Payment will be based on the Actual Quantity (AQ) the contractor completes and the government accepts.
1.1 Award of Contract:
The Government may award multiple contracts. At least one award may be made for each Ranger district identified below, or the Government may, at its discretion, make a single award.
Hidden Springs Ranger District Road Maintenance (East Side Contract) Mississippi Bluffs Ranger District Road Maintenance (West Side Contract)
Vendors may submit quotations for the Hidden Springs Ranger District or the Mississippi Bluffs Ranger District separately or they may provide a quotation for both Ranger Districts combined. To be considered for award, pricing for all items MUST be included in your quotation (either the Hidden Springs Ranger District or the Mississippi Bluffs Ranger District separately or both Ranger Districts combined)
Vendors shall enter a UNIT PRICE and AMOUNT for each PAY ITEM and TOTAL AMOUNTS for the BASE YEAR plus OPTION YEAR 1, OPTION YEAR 2, and OPTION YEAR 3 where indicated on Schedule of Items.
1.2 Contract Time
It is anticipated that the contract will be awarded on or about August 28, 2019 or as soon as otherwise possible.
It is also anticipated that the first Task Order shall be issued within 10 calendar days after the effective date of the Notice to Proceed.
All work shall be designated and specified in a Task Order. The attached Road Maintenance Schedules (Exhibits H & I) may be used as guides for when Task Orders will be issued and denote roads and mileage covered by this contract. The contractor shall not begin work until a Task Order has been issued. The contractor shall complete all work specified and designated within the timeframe identified in the Task Order.
2.0 Scope of Work:
2.1 Surface Replacement Schedule:
Activity Code 114 - Surface Replacement (not shown on the Road Maintenance Schedule (Exhibits H and I)) shall be furnished and accomplished in conjunction with Activity Code 109 - Blading Aggregate Surfaced Roads with quantities and grade of material as specified by Task Order from the CO.
2.2 Mowing Schedule:
See Exhibits H & I for a schedule of mowing. The following roads shall be mowed within two weeks prior to the Memorial Day and Labor Day Holidays to insure that all mowing is completed prior to Holiday activity unless otherwise scheduled by the CO.
Activity Code 401 - Mowing:
Hidden Springs Ranger District - Millstone Bluff No. 462; Bell Smith Spring No. 848; and Bell Smith Scenic No. 849, High Knob Road, No. 114; Garden of Gods Road, No. 115; Pounds Hollow, No. 134; Garden Rec Site, No. 163; Tower Rock Road, No. 575; and Whoopie Cat Road, No. 1736, are to be mowed two weeks prior to Memorial Day and Labor day Holidays. Stoneface Road, No. 1453; and Cadiz Campground, No. 598, are to be mowed two weeks prior to Labor Day Holiday.
Mississippi Bluffs Ranger District- Little Grand Canyon Road, No. 351; Johnson Creek Road, No. 724; John Spur Road, No. 786; and Turkey Bayou Road, No. 786R, are to be mowed two weeks prior to Memorial Day and Labor Day Holidays. Buttermilk Hill Road, No. 718 is to be mowed two weeks prior to Labor Day Holiday.
2.3 Location and Description:
See the attached Location Maps (Exhibits J, K, L, M, N, O, P, and Q) which identify locations of roads anticipated to require routine maintenance. These maps are not intended to depict all roads, but do represent a majority of the current Forest Service jurisdiction roads and similarly a majority of the roads for which work may be specified.
2.4 Damage and Repairs:
The Contractor shall be held responsible for any and all damage caused by his operations, including but not limited to damage to: utilities, roads, culverts, header walls, bridges, guardrail, signs, facilities, low water crossings, mail boxes, etc., without regard to ownership.
2.5 Weed and disease Prevention:
This work consists of washing and treating construction equipment to remove seeds, plants, and plant fragments from the equipment before the equipment is used on National Forest System lands.
Use a high pressure washing system. For work on National Forest System lands, use a washing system that traps all wash water and either stores it for removal from National Forest System lands or recycles the water for continued use. If the equipment recycles the water, provide adequate filters for seed removal. Dispose of the filter material and removed seeds in an approved manner. Do not mix soaps, detergents, or other chemicals with the wash water. For washing done at a commercial washing facility, use an approved facility.
Wash the sides, tops, and undercarriages of all construction equipment. Remove all seeds, plants, plant fragments, dirt, and debris from the construction equipment. Inspect the washed construction equipment, including the undercarriage, to ensure that the washing removed the dirt, debris, and seeds from the construction equipment. Rewash the construction equipment as necessary or as directed.
2.6 Safety and Health:
A. 29 CFR, Part 1910: Occupational Safety and Health Administration (OSHA) General Industry and Health Standards.
B. 29 CFR, Part 1926: OSHA Construction Industry Standards.
C. 40 CFR, Part 61: National Emission Standards for Hazardous Air Pollutants.
D. 40 CFR Part 261: Environmental Protection Agency (EPA) Characteristics of hazardous Waste.
E. CFR Part 761, EPA Polychlorinated Biphenyls (PCB’S), Manufacturing, Processing, Distribution in commerce and Use Prohibitions.
Acquisition of Publications: Referenced CFR publications may be purchased from the Superintendent of Documents, U.S.
Government Printing Office, Washington, D.C. 20402.
Prior to commencing work, representatives of the Contractor, including the Superintendent, shall meet with the Government to review the Contract's safety and health requirements. The Contractor's safety and health program shall be reviewed, and implementation of safety and health provisions pertinent to the Work shall be discussed.
Contractor Responsibility: The Contractor shall assume full responsibility and liability for compliance with all applicable codes, standards and regulations pertaining to the health and safety of personnel during execution of the Work. The Contractor shall have written safety and health programs in compliance with 29 CFR Parts 1910 and 1926.
Safety and Health Programs: The Contractor shall submit, for approval, copies of the project safety and health programs, as applicable to the work scope, or required as a result of the safety meeting, including but not necessarily limited to the following:
A. Occupational Noise Exposure.
B. Fall Protection.
C. Personnel Protective Equipment.
D. Control of Hazardous Energy.
E. Electrical Safety Related Work Practices.
F. Lead.
G. Respirator Protection.
H. Confined spaces.
Special facilities, devices, equipment and similar items used by the Contractor in execution of the Work shall comply with 29 CFR Part 1910, Subpart I and other applicable regulations.
The Contractor shall take all necessary precautions to prevent injury to the public, occupants, or damage to property of others.
Dispose of solid, liquid and gaseous contaminants in accordance with local codes, laws, ordinances and regulations.
Comply with applicable federal, state and local noise control laws, ordinances and regulations, including but not limited to 29 CFR 1910.95 and 29 CFR 1926.52
2.7 Public Safety and Traffic Control:
The Contractor shall exercise caution and care while pursuing the work to prevent unnecessary conflict with or potential hazards to road users, private property and public property.
When working in roadways the Contractor shall post temporary warning signs on each end of the section of road for which road maintenance activities are being performed. Signs shall be of the size, quantity and colors as required in the current edition of the "Manual on Uniform Traffic Control Devices" (MUTCD). These signs shall be moved as needed to properly delineate and identify the section of road being maintained. These signs shall be in place only when road equipment or workers are actually performing operations. All slow moving equipment shall have a reflectorized "slow moving vehicle" emblem and flashing caution lamp(s) appropriately mounted and visible to vehicles approaching from the front, back, or sides. All equipment shall have operational back-up alarms and other equipment as needed to comply with OSHA requirements. All equipment will be inspected for compliance with the equipment requirements of this contract prior to contract award and periodically during the contract.
The Contractor shall not close any road without prior approval from the COR except as may be necessary for short duration activities during which all applicable traffic control must be in place. The contractor agrees to re-open a road immediately if during a short duration road closure any emergency response personnel need to pass.
Work shall be so scheduled such that at the end of each day the road is passable for the type of traffic normally using the road. No material stockpiles, soil or gravel berms or soil or gravel windrows shall be left overnight within the traveled way or shoulder. During daily activities, material stockpiles and equipment placed in or adjacent to the traveled way shall be properly signed or illuminated consistent with the MUTCD.
2.8 Notification:
The Contractor shall notify the CO at least 48 hours prior to beginning work on the project.
2.9 Payment, Accomplishment Records and Reports:
The contractor shall deliver to the CO all information including but not limited to weight tickets, receipts, reports, submittals or quality control prior to or concurrent with an invoice or request for payment. Payment for work, materials or services in dispute shall be withheld until resolution is reached. The contractor shall not invoice or request payment for work, materials or services known to be in dispute.
3.0 Definitions:
A. Quality Assurance. Those actions taken by the government to check goods or services to determine that they meet the requirements of the SOW.
B. Quality Assurance Surveillance Plan. An organized written document used for quality assurance surveillance. The document contains information about how the government will assess performance, checklists, and may contain decision tables.
C. Quality Control. Those actions taken by a contractor to control the production of goods or services so that they will meet the requirements of the SOW.
D. (Acceptable) Service. A job performed to the standard and within the acceptable quality level. The contractor must do the specific job, and meet the standard, and meet the acceptable quality level before one can say that performance has been acceptable and that he/she should be paid.
E. Standard. An acknowledged measure of comparison.
F. Replace. Remove existing damaged material, dispose of properly and replace with new materials and/or equipment, as noted.
4.0 Government Furnished Property/Services/Materials/Items:
Maintenance Activity descriptions will be provided with the contract documents. There are no other Government Furnished Items with this contract.
5.0 Contract Requirements:
A. Task Orders: All work shall be initiated by a Task Order issued by the Contracting Officer. Task Orders will reference Maintenance Activities to be performed. No work is to occur until a Task Order is received by the Contractor.
B. Final Road Surface: Shall vary by the Maintenance Level of the roadway.
1. Maintenance Levels 1 and 2: Final surface shall be maintained to drain and prevent erosion of the surface. Emphasis shall be on maintaining the cross slope and lead off ditches. These roads are for high clearance vehicle traffic.
2. Maintenance Levels 3, 4 Gravel Surface: Final surface shall be maintained to drain; prevent erosion; cleaning the ditch line and lead off ditches; cleaning culverts; removal of ruts, holes, washboards and other surface irregularities to provide a smooth traveling surface; mowing the shoulders and remove brush overhanging the roadway. These roads are for passenger car traffic.
3. Maintenance Levels 3, 4 and 5 Oil and Chip Surface: Final surface for these roads shall be to clean ditch lines and culverts, mow the shoulders and remove brush overhanging the road way.

C. Blading Aggregate Surfaced Roads: Blading operations shall be conducted when and where identified on the Task Order and shall include removal of ruts, potholes, corrugations and bumps with a scarifier. All suitable dislodged aggregate shall be smoothly redistributed over the entire travelway to provide proper cross slope or crown (2% to 4% slope). Blading area shall be from outside edge of shoulder to outside edge of shoulder on existing road surface and shall include turnouts, curve widening and turnarounds.
D. Surface Replacement, Aggregate Surfaced Roads: Normal operation in this activity is replacement of surface aggregate that is lost due to surface failure, traffic wear or erosion. Work to be performed under this activity includes existing surface preparation, furnishing aggregate and spreading aggregate with a finish blading. Finish blading of surface replacement is included in this activity and will not be paid for separately under Activity Code 109. The Contractor shall have the capability and may be required at times to deliver surface replacement aggregate and perform motor grader finish blading at the same time when designated and specified by Task Order from the CO.
In areas where activity 301 is required, aggregate placement will not be started until cleaning and reshaping of ditches is completed unless otherwise approved by the CO.
E. Stone Riprap Placement: Normal operation in this activity is placement of riprap for bank protection, slope protection, drainage structures, and erosion control. Work to be performed under this activity includes providing and placing riprap by the method called for, either machine placed or end dumped. Culvert pipe placement and earthwork are not included in this item and shall be paid for under appropriate activity code. In areas where activity 301 or 802 is required, riprap placement will not be started until those activities have been completed unless otherwise approved by the CO.
F. Clean and Reshape Ditches: Ditches shall be maintained in their original location and to the design depth or to a minimum depth of 12 inches below the road shoulder and to the shape and grade required to insure drainage of the roadway. All ditches shall be maintained free of loose sod and other loose material on the slope between the bottom of the ditch and the road shoulder. Handwork may be required to properly connect ditch flow lines to drainage structure inlets/outlets and is considered a part of this activity. The Contractor is to perform Activity Code 301 work for all ditches on roads where Activity Code 109 is to be performed.
G. Hand Cleaning of Culverts: Cleaning of culverts is removing silt, slough and debris from the culvert interior, catch basin, inlet and outlet ditches by a laborer using a shovel or other hand tools.
The work is performed to provide unobstructed flow to, through and away from the culvert minimizing road prism erosion and other resource damage. This activity includes maintenance of box culverts.
H. Mowing: Mowing shall consist of cutting grass and small woody plant growth (12 inches in diameter or less at mower height) within the travelway and on both sides of roads including mowing around road signs and appurtenances.
I. Brushing: Brush cutting shall consist of cutting and removing all living and dead vegetation and brush having a diameter of 6 inches or less at stump height with hand tools and a mechanical arm brusher attached to a piece of heavy equipment.
J. Bridge Maintenance: Bridge maintenance includes preventative maintenance work necessary to prolong the life of the structure. Preventative maintenance work under this activity is limited to cleaning the decks, cleaning drain holes, cleaning bridge seats, cleaning pile caps, cleaning beams, clearing debris and brush from under the bridge and from within the stream channel; and removing any visual obstructions from signs and object markers (i.e., brush, vegetation).
K. Rubber Tire Backhoe with Operator and Ground Person: Equipment rented under this activity is for, but not limited to, unexpected work such as culvert repair, road bed repair and land slide removal.
L. Motor Grader with Operator: Equipment rented under this activity is for, but not limited to, unexpected work such as road surface repair, cleaning ditches and pothole scarification.
M. Excavator with Operator: Equipment rented under this activity is for, but not limited to, unexpected work such as road surface repair, cleaning ditches, clearing, and culvert installation.
N. Dozer with Operator: Equipment rented under this activity is for, but not limited to, unexpected work such as road surface repair, cleaning ditches, clearing, and culvert installation.
O. Damages: Contractor shall be responsible for repairing all damage caused by their equipment or employees to Forest Service property at their expense. Any ruts or other damage to the ground shall be raked smooth and seeded and mulched.
1. Seed: Shall be a lawn mix commercially available.
2. Mulch: Shall be certified weed and seed free straw.
6.0 Requirements guidelines.
A. Equipment Requirements
1. Motor Grader: The equipment shall be a motor grader with a minimum weight of 20,000 pounds and a minimum SAE horsepower of 85. The equipment shall be equipped with a scarifier with all teeth attached and in working order.
2. Mower: Equipment used shall consist of a wheeled tractor with a minimum net horsepower of 35 and a bushhog.
3. Brushing: Equipment used shall consist of hand tools, a mechanical arm brusher attached to a motor grader, wheeled tractor, or other power equipment as approved by the COR.
4. Rubber Tire Backhoe: The rubber tired backhoe shall have a minimum net 56 horsepower and be equipped with a minimum 14 feet reach and a minimum 24 inch bucket.
5. Excavator: The equipment shall be a minimum of a 15 metric ton tracked excavator.
6. Dozer: The dozer shall have a minimum SAE 85 horsepower and be equipped with a 6-way blade.
B. Maintenance Activity Requirements: Maintenance Activities descriptions will be provided.
C. Code Compliance: The contractor is required to make repairs following all applicable Maintenance
Activities, national, state and local codes, including but not limited to:
1. Occupational Health and Safety Administration (OHSA)
D. Site Cleanup and Disposal:
1. All construction debris and litter shall be removed from the job site at the end of the day.
Contractor shall keep all litter and debris picked up during the day.
2. Contractor shall not allow fluids from the maintenance equipment to be released on roads or adjacent surfaces.
E. Protection of Forests, Parks and Federal Lands:
1. Contractor shall inform and educate the supervisor(s) on the job and each individual worker that any snake encountered must not be killed or harmed. The timber rattlesnake is legally protected under the Illinois Endangered Species Protection Act, and a violation is a Class A misdemeanor.
(Class A misdemeanors carry a fine of up to $1,000 and up to 364 days in jail.) Contractor is to notify the CO of any timber rattlesnake sighting.
2. If Federal, State or Forest listed threatened or endangered plants, animals, or cultural resource sites are discovered within the roadway by the Contractor, then the CO and COR shall be notified immediately and immediately discontinue any further work in the vicinity of the discovery. Work may be terminated for the convenience of the government as determined by the Contracting Officer. The COR will, within 24 hours of notification, mark the area where work may no longer be required.

DIVISION 100 - ROAD SURFACE ACTIVITIES
ACTIVITY CODE 109 – GRADING (MOTOR GRADER)
Maintenance Requirements
Blading operations shall be conducted as required and shall include removal of ruts, potholes, corrugations and bumps with a scarifier. All suitable dislodged aggregate shall be smoothly redistributed over the entire travelway to provide proper cross slope or crown (2% to 4% slope). Blading area shall be from outside edge of shoulder to outside edge of shoulder on existing road surface and shall include turnouts, curve widenings and turnarounds.
Suitable moisture content of surface aggregate during blading shall be from five to fifteen percent by weight. The Contractor is responsible to insure that blading operations are performed within the specified moisture content limits. All suitable aggregate which has been worked onto road shoulder by traffic or maintenance activities shall be reclaimed onto the travelway. A definite, continuous and smooth line between aggregate surface and road shoulder shall be obtained.
Grass or leaf litter deposited within the travelled way shall be smoothly spread within the travelway or along the shoulder after movement sufficient to sift out surface aggregate. In some instances, hauling of excess grass or leaf litter will be required to remove this material from the road surface simultaneous to blading operations. Disposal sites shall be locations approved by the COR. The maximum haul distance required is 1 mile, undercutting of the road surface at the shoulder line and leaving material berms is unacceptable. No material shall be allowed to remain on shoulders or in ditch lines as a result of the contractor's operation.
Minimum blading work shall be five passes with the grader on any road section. Passes will not exceed 1 mile intervals along any length of road and at any one time without the approval of the COR. Maximum blading speed shall be such that surface aggregate rolls and mixes from the angled blade of the motor grader and such that bouncing of the motor grader and the creation of a corrugated road surface from blading operations is prevented. Loose rocks protruding 4 inches or more above the graded surface shall be removed from the travelway. It is permissible to waste these rocks on Forest Service land, beyond the roadside mowing limits and away from drainage ditches as approved by the COR.
Travelways (aggregate surface) shall be maintained over the full existing width. Dips and swales within the travelway designed to control surface drainage shall be maintained. Mailbox turnouts along roads shall also be maintained as part of the road. Blading and handwork may be required to maintain mailbox turnouts. Mailboxes shall be protected during maintenance activities.
The Contractor will be held responsible for any damage caused by his blading operations to utilities, the road or existing associated road structures such as culverts, header walls or bridges.
Identification
Roads to be bladed on a schedule are shown on the attached Project Site Maps and Schedule (See Exhibits H - Q). Refer to Exhibit A for Definition of Road Structure terms and Exhibit D & G for Grading Typicals.
Method of Measurement
Measurement shall be made to the nearest tenth (0.1) mile, measured along the centerline of the road.
Basis of Payment
Payment shall be made for all work in this activity inspected and deemed acceptable by the CO in accordance with the following pay item and shown in the Schedule of Items.

ACTIVITY CODE 114 - SURFACE REPLACEMENT, AGGREGATE SURFACED ROADS
Normal operation in this activity is replacement of surface aggregate that is lost due to surface failure, traffic wear or erosion. Work to be performed under this activity includes existing surface preparation, furnishing aggregate and spreading aggregate with a finish blading. Finish blading of surface replacement is included in this activity and will not be paid for separately under Activity Code 109. The Contractor shall have the capability and may be required at times to deliver surface replacement aggregate and perform motor grader finish blading at the same time when designated and specified in the applicable Task Order. In areas where activity 301 is required, aggregate placement will not be started until cleaning and reshaping of ditches is completed unless otherwise approved by the CO.
Remove, by cutting out, all damaged areas of surface and work suitable material smoothly over the entire travelway width while disposing of unsuitable material as approved by the CO. Prevent disturbing base or subgrade material that is in satisfactory condition. Deposit new aggregate in the section being worked to achieve grade, thickness and template as specified in the applicable Task Order. Spread material evenly across the travelway and finish blade entire area receiving surface replacement aggregate in accordance with Activity Code 109. No excess or unwanted material shall be allowed to remain on shoulders or in ditch lines as a result of the Contractor's operations.
Trucks hauling aggregate material shall comply with weight limits posted on the roads and bridges that are used for haul and for truck weight limits as permitted by the State of Illinois Department of Transportation.
Materials
Course aggregate and agriculture lime shall be from an approved source and meet Illinois Department of Transportation quality and gradations CA-1, CA-5, CA-6 or CA-10 or as otherwise specified in the applicable Task Order.
The specific sections to be resurfaced with the quantity (in tons) and gradation of aggregate that is required for each section will be specified in the applicable Task Order.
Measurement shall be made to the nearest one-tenth (0.1) of a ton. The Contractor shall supply weight tickets for each Task Order (utilizing State of Illinois certified scales) showing aggregate quality, gradation, source, gross, tare and net weights.

Payment shall be made for all work in this activity inspected and deemed acceptable by the CO in accordance with the following pay item and shown in the Schedule of Items.

ACTIVITY CODE 115 – STONE RIPRAP PLACEMENT
Normal operation in this activity is placement of riprap for bank protection, slope protection, drainage structures, and erosion control. Work to be performed under this activity includes providing and placing riprap by the method called for, either machine placed or end dumped. Culvert pipe placement and earthwork are not included in this item and shall be paid for under appropriate activity code. The contractor shall have the capability and may be required at times to deliver and place riprap when designated and specified in the applicable Task Order. In areas where activity 301 or 802 is required, riprap placement will not be started until those activities have been completed unless otherwise approved by the
CO.
Machine Placed: Place riprap to its full thickness in one operation to avoid displacing the underlying material. Do not place riprap material by methods that cause segregation or damage to the prepared surface. Place or rearrange individual rocks by mechanical or manual methods to obtain a compact uniform blanket with a reasonably smooth surface.
End Dumped: Dump riprap to its full thickness in one operation. Avoid displacing the underlying material. Distribute larger rocks throughout the mass of stone. Obtain a uniformly thick blanket with a reasonably smooth surface.
Trucks hauling riprap material shall comply with weight limits posted on the roads and bridges that are used for haul and for truck weight limits as permitted by the State of Illinois Department of Transportation.
Materials
Stone Riprap shall be from an approved source and meet Illinois Department of Transportation specifications for quality and gradations for number 3, 4, 5 or pit-run supplied riprap as specified by the Work Order from COR.
The specific sections where riprap is to be placed and the quantity (in tons) and gradation of riprap that is required for each section will be specified in the applicable Task OrderR.
Measurement shall be made to the nearest one-tenth (0.1) of a ton. The Contractor shall supply weight tickets for each Task Order (utilizing State of Illinois certified scales) showing aggregate quality, gradation, source, gross, tare and net weights.

ACTIVITY CODE 150 – INCIDENTAL GRADING (MOTOR GRADER)
This activity has the same maintenance requirements as Activity Code 109 with the exception that Activity Codes 301 and 305 do not apply here. This is strictly for the grading portion of the work.
Roads to be bladed shall be identified on the task order by road number and the miles of road to be graded. Refer to Exhibit A for Definition of Road Structure terms and Exhibit D & G for Grading Typicals.
Measurement shall be made to the nearest tenth (0.1) mile, measured along the centerline of the road.

DIVISION 300 - DRAINAGE ACTIVITIES
ACTIVITY CODE 301 - CLEAN AND RESHAPE DITCHES
Ditches shall be maintained in their original location and to the design depth or to a minimum depth of 12 inches below the road shoulder and to the shape and grade required to insure drainage of the roadway. All ditches shall be maintained free of loose sod and other loose material on the slope between the bottom of the ditch and the road shoulder. Repair to any utilities, drainage structures (i.e., culverts, etc.) and road appurtenances damaged by the Contractor during the ditching operation shall be at the Contractor's expense. Handwork may be required to properly connect ditch flow lines to drainage structure inlets/outlets and is considered a part of this activity. The Contractor is to perform Activity Code 301 work for all ditches on roads where Activity Code 109 is to be performed.
Road Ditches - All suitable material (having the same character as the road surface) that is excavated shall be reclaimed and used within the travelway as approved by the CO. Unsuitable material shall be bladed out of the ditch and onto the backslope or excavated and hauled to a disposal site as approved by the CO. When backslope conditions are such that the motor grader can be safely operated on the backslope, this excavated material shall be feathered up onto the backslope.
The feathered material shall be bladed up onto the backslope in a manner that does not create berms or piles of material.
A maximum of two passes of the motor grader is required for the feathering work. The CO may designate areas that shall not receive feathering. The Contractor shall be responsible for the expense of road repair resulting from reclaiming onto the roadbed any unsuitable materials excavated from ditches.
Tail and Lead-Off Ditches - Existing ditches that are silted in or closed with other debris shall be opened to provide drainage away from the roadway. Unsuitable material excavated from tail and lead-off ditches shall not be reclaimed onto the roadbed. The Contractor shall be responsible for the expense of road repair resulting from reclaiming onto the roadbed any unsuitable materials excavated from tail and lead-off ditches.
Cleaning and reshaping of ditches shall be coordinated, planned and executed with Activity Code 109, Blading, Aggregate Surfaced Roads to blade the traveled way when suitable material excavated from ditches is utilized in the roadbed for the purpose of mixing suitable material in with the surface aggregate.
Road ditches and lead-off ditches shall be identified and maintained by the Contractor during the blading operation under Activity Code 109. Identification and maintenance of ditches shall be done in such a manner as to insure proper drainage of roadway.
Cleaning and reshaping of ditches will not be measured. It is considered a subsidiary item to Activity Code 109.
Payment shall be made under Activity Code 109 after this activity is inspected and deemed acceptable by the CO.
ACTIVITY CODE 305 - HAND CLEANING OF CULVERTS
Cleaning of culverts is removing silt, slough and debris from the culvert interior, catch basin, inlet and outlet ditches (refer to Exhibit B, Typical Culvert Section) by a laborer using a shovel or other hand tools. The work is performed to provide unobstructed flow to, through and away from the culvert minimizing road prism erosion and other resource damage. This activity includes maintenance of box culverts.
Unsuitable material removed shall be placed outside of the roadway such that it will not re-enter the culvert, catch basin or ditches. Unsuitable material removed from culverts shall not be reclaimed onto the roadbed. The Contractor shall be responsible for the expense of road repair resulting from reclaiming onto the roadbed any unsuitable materials excavated from culverts, catch basins or ditches.
Culverts shall be identified and maintained by the Contractor during the blading operation under Activity Code 109.
Hand cleaning of culverts will not be measured. It is considered a subsidiary item to Activity Code 109.
Payment shall be made under Activity Code 109 after this activity is inspected and deemed acceptable by the CO.
ACTIVITY CODE 310 – CULVERT REPLACEMENT/INSTALLATION
Maintenance Requirements
Installation of a culvert is the excavation and setting of a new culvert in a new location or placing an extension on an existing pipe. Replacement of a culvert is removing the existing drainage structure, disposal of the debris off National Forest lands and installation of a new culvert in the existing location. Work shall include providing all equipment, labor, materials other than the culvert, all excavation, removal of existing culvert, preparation of the bedding, setting the new pipe, compacted embankment and replacement of surfacing. Included is 15 ton of IL-CA-06 aggregate to be used in the bedding, setting, embankment and road surface repair. Contractor is responsible for utility line location prior to starting work. Government shall supply all culverts, bands and extensions needed.
Replacement culvert pipes shall be placed in the same location and at the same skew as the original, unless designated otherwise. Location and skew for new installations shall be identified on the ground prior to starting work. Bedding for pipes 48” diameter or equivalent and larger shall be approved by the CO prior to placement of backfill.
Unsuitable material removed shall be placed outside of the roadway such that it will not re-enter the culvert, catch basin or ditches. Unsuitable material removed from culverts shall not be reclaimed onto the roadbed. The Contractor shall be responsible for the expense of road repair resulting from reclaiming onto the roadbed any unsuitable materials excavated from culverts, catch basins or ditches.
This work is performed to provide unobstructed flow to, through and away from the culvert minimizing road prism erosion and other resource damage.
Culverts shall be identified on the task order by road number and milepost, as well as marked on the ground.
Culvert replacement or installation shall be measured per nearest 2 foot increment in length of the pipe.
Payment shall be made under Activity Code 310 after this activity is inspected and deemed acceptable by the CO in accordance with the following pay item and shown in the Schedule of Items.

DIVISION 400 - ROADWAY ACTIVITIES
ACTIVITY CODE 401 - MOWING
Mowing shall consist of cutting grass and small woody plant growth (2 inches in diameter or less at mower height) within the travelway and on both sides of roads including mowing around road signs and appurtenances.
It is the responsibility of the Contractor to remove fallen limbs, downed trees and other debris which would prevent the mower from meeting the height and width requirements in this specification.
Mowing operations shall include mowing from the travelway to 5 feet horizontally beyond any ditches or 10 feet horizontally out past the travelway and to a height of 2 inches to 4 inches (refer to Exhibit F). When fence or tree lines are encountered, then they will be considered the edge of the mowing limits. More than one pass with the mower may be required to obtain the minimum width. Any additional mowing for safe sight distance at road intersections or blind curves as designated by the CO is considered incidental and a part of this activity.
All rocks larger than 4 inches, all woody material larger than 1 inch in diameter and 24 inches in length and any sharp object which could cause tire damage shall be removed from the travelway as part of this activity.
The contractor shall remove grass and weeds growing either on or within 12 inches of the bridge or any bridge features, including guardrails. Grass removal at the base of bridge features and guardrail shall be accomplished with a “weed eater” cutter or similar apparatus. Grass shall be cut down flush to grade.
Roads to be mowed are shown on the attached Project Site Maps, Exhibits H & I Road Maintenance Schedule. Work limits for the mowing operation are shown in Exhibit F, Roadside Mowing Detail.
Equipment Specification
Mowing shall consist of a wheeled tractor with a minimum net horsepower of 35 and a bush hog.
Measurement shall be made to the nearest tenth (0.1) of a mile, measured along the center line of the road. Each side of the road will not be measured separately.
Payment shall be made for all work in this activity inspected and deemed acceptable by the CO in accordance with the
401 Mowing Mile
DIVISION 400 - ROADWAY ACTIVITIES
ACTIVITY CODE 402 - BRUSH CUTTING; MACHINE & HAND
Brush cutting shall consist of cutting and removing all living and dead vegetation and brush having a diameter of 6 inches or less at stump height with hand tools and a mechanical arm brusher attached to a piece of heavy equipment. Brushing shall be performed with the use of a mechanical arm brusher or by handwork.
The mechanical arm brusher shall be attached to a motor grader, wheeled tractor, or other power equipment as approved by the CO. The mechanical arm brusher is intended to allow for the Contractor to drive along the side of the road and mechanically reach out cutting the brush.
Handwork shall be performed for limbing with the use of pruning saws, power saws, nippers, bow saws or cross-cuts.
Limbs shall be pruned as close as possible to tree trunks. Brush will be cut no higher than 3 inches above ground level.
Removed material shall be scattered outside the roadway. Disposal shall include removal of brushing debris from the roadbed and ditches, pruning scarred limbs, and breaking down all brushing debris below the level of grass growth to provide a roadside strip that will facilitate future mowing.
Removal of trees, limbs, brush, and obstructions shall be limited to those which hang over the roadway obstructing a driver's sight distance, those that are within 14 feet vertical clearance above and 10 feet on each side of the roadbed, or dead snags that could fall into the travelway as shown on Exhibit E. All fallen trees, limbs, or brush in the travelway or road ditch line shall also be placed outside of the roadway. On sharp curves, tree limbs shall be trimmed to a minimum width of 12 feet on inside of curves or as identified in the field by the CO for safe traffic sight distance.
Vegetation removed from bridge stream channel shall be disposed as to not interfere with stream channel flow. Trees over 8 inch diameter shall not be removed unless specified by CO. Brush or smaller diameter trees within 10 feet in all directions along portion of the bridge features shall be cut flush to the ground and removed.
The specific sections of road and bridges that require brush cutting will be designated and specified in the applicable Task Order.
Equipment Specification
Brushing shall consist of a mechanical arm brusher or loader mounted vertical sickle mower attached to a motor grader, wheeled tractor, or other power equipment as approved by the CO.
Measurement shall be made to the nearest tenth (0.1) mile, measured along the centerline of the road.
