That log book is to be maintained at the respective water systems' pump-houses in a manner that prevents the log books from becoming damaged by humidity and/or vermin.
4.1.2. Routine water systems evaluations. The contractor is to perform a visual evaluation of each water system during the site visits conducted as per paragraph 3.1.1 of this document. The purpose of these evaluations is to identify water system component malfunctions, failures, vandalism and/or sabotage. When any of those conditions are observed and/or suspected, that finding is to be recorded in the Log Book, and the COR is to be notified promptly. Components of the water systems include, but are not limited to:
the storage tanks, pump-house buildings, pumps, control systems, well-heads, valves, hose-bibs, backflow prevention devices, disinfectant equipment, and associated plumbing.
4.1.3. Routine bacteriological monitoring. All of the Group A water systems require 1 raw well sample and 2 routine distribution system samples to be collected quarterly for bacteriological monitoring. Each of the routine distribution system samples are to be collected from two different remote taps unless there is only one remote tap. Where a Group A system has 1 remote tap, the 2 quarterly routine distribution system samples are to be collected on two different days. The Group B water system requires 1 raw well sample and 1 routine distribution system sample to be collected quarterly for bacteriological monitoring.
The samples are to be analyzed by a State of Florida operated and/or approved laboratory. All of the routine bacteriological monitoring samples for any single water system are to be collected on one day, with the exception of a Group A water system that only has 1 remote tap. The results of the microbiological analysis shall be reported in terms of the presence or absence of total coliform. The contractor is to forward microbiological analysis results, including date and time that the contractor received notification of the results, to the COR. The COR is to be notified of any total coliform-positive (TC+) test results on the same day that the contractor receives such notification from the laboratory.
All TC+ sample results are to be tested for the presence of E. coli.
Whenever a sample analysis result is TC+ and/or E. coli-positive (EC+), the contractor is to notify DEP (for Group A systems) or DOH (for the Group B system) before the end of the day that the contractor was notified of the results. The contractor is to provide a copy of the notification sent to DEP
National Forests in Florida
Performance Statement of Work Osceola National Forest Water System Operation and/or DOH to the COR along with the date and time that the information was provided to DEP/DOH.
4.1.4. Annual Nitrate and Nitrite monitoring. Each system, of both groups, is to be sampled and analyzed annually for the presence of Nitrate and Nitrite as per the FAC's. The samples are to be collected in June of each year. The results are to be reported to the COR, DEP (for Group A water systems) and DOH (for Group B water systems) before the end of the first week in July.
4.2. Non-routine water system operation and maintenance work items. These work items are to be considered as Indefinite Delivery Indefinite Quantity (IDIQ) contract items. The contractor is to provide unit-costs for these work items.
When the work item has been completed successfully, the contractor is to submit an invoice for the work in IPP, separate from the work items in section 3.1.
4.2.1. Repeat Microbiological Monitoring Samples. All repeat microbiological monitoring samples are to be collected and analyzed as per the relevant FAC's. The contractor is to perform the following repeat samples within the time constraints described below and, in the FAC.
Group A Repeat Samples. Within 24 hours of learning of a TC+ routine sample result, at least 3 repeat samples are to be collected and submitted to the laboratory for analysis. One sample is to be collected from the location of the original TC+ sample. The second sample is to be collected within five service connections upstream of the TC+ sample collection location. The third sample is to be collected within five service connections downstream of the TC+ sample collection location. If the original TC+ sample was collected at the adjacent-to-the-final-service connection in the distribution system, an alternate representative location may be used instead.
Additional repeat samples are required to be collected for a water system in the month following a system having a TC+ routine sample. 3 additional samples are required for each TC+ sample, and an additional 3 samples for each repeat sample that is also TC+.
There are provisions under the Revised Total Coliform Rules for a water system to require increased bacteriological monitoring frequency. The DEP makes that determination. The contractor is to inform the COR promptly upon being made aware of such a determination. The contractor is not to begin sampling any water system at an increased frequency until instructed to by the COR.

Performance Statement of Work Osceola National Forest Water System Operation
Group B Repeat Samples. Whenever a routine sample is TC+, a repeat sample is to be collected before the end of the first business day following notification of the TC+ results. One repeat sample is to be collected for each TC+ sample.
There are provisions in FAC 64E-8 for increased monitoring frequency. The Department of Health makes that determination. The contractor is to inform the COR promptly upon such a determination being made. The contractor is not to begin sampling at an increased frequency until instructed to by the
COR.
4.2.2. Level 1 Assessments, as per the Revised Total Coliform Rule. Level 1 Assessments are performed on water systems when triggered by the criteria of the Revised Total Coliform Rule. If the DEP determines that a Level 1 Assessment has been triggered for one of the Group A water systems, the contractor will be required to perform that assessment within 30 days of the notification of that trigger. The contractor is to use the Level 1 Assessment form specified by the Revised Total Coliform Rule, submit it to DEP, and provide a copy to the COR with the date and time of submission to DEP.
The following trigger conditions for which Level 1 Assessments may be required.
• The public water system has 2 or more TC+ routine and/or repeat samples in the same month.
• The public water system fails to take every required repeat sample after any single TC+ sample result.
4.2.3. Clearance Sampling. Whenever repair work is performed on water system components, conditions that require clearance sampling before the water system can be returned to normal operation may occur. The contractor will be required to perform that clearance sampling when directed to by the COR.
4.2.4. Public Notification. Whenever a water system exhibits a condition(s) that warrants public notification, as per the FAC, the contractor is to notify the COR immediately. The contractor will write the appropriate notification and provide it to the COR, and then with COR approval, post the notification according to the methods described in the FAC and/or are provided by DEP.
