This document is a Reference Questionnaire for a federal contract opportunity. It provides a sample transmittal letter and questionnaire to be sent to references for a contractor submitting a proposal in response to Solicitation Number 12444224R0006 for "Prescribed Fire Support – National Forest & Grasslands in Texas" issued by the USDA Forest Service.

The questionnaire evaluates the contractor's past performance in areas such as quality of work, customer satisfaction, timeliness, and safety.
