This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
(ii) This solicitation is issued as a Request for Quotation (RFQ).

(vi) Description of Requirement:
The Ephraim Ranger District-The designated office, is located at 540 N Main Street, Ephraim
UT and consists of approximately 6,200 square feet of useable space and the Warehouse of approximately 360 square feet.
The entry is at ground level, in the front and the back of the office building. The main office building consists of the reception area; two restrooms, 14 offices, a computer room, a break room, two conference rooms and a hallway. This area contains both carpeted and resilient floor areas. There are sinks and cabinets with counters in the break room and the large conference room.
The warehouse hall area and restrooms, at the same address will be cleaned two times per week May thru Oct and one time per week from November thru April. The cleaning at the warehouse will include all requirements as listed in Daily (every cleaning), weekly, monthly, and four times a year requirements. The Sanpete Ranger District warehouse hallway/restrooms, consists of approximately
286 square feet of useable space.
The bathrooms, hall, shower stalls, and dressing rooms of the warehouse will require periodic cleaning as listed in the schedule. The same services will be required as in the District Office. The areas to be cleaned, total approximately 286 square feet. The rest of the building will not be the responsibility of the contractor. There is a Janitor closet in the hallway where water is available, and supplies may be stored.
There are two bathrooms that have a painted concrete floor. The square footage is approximately 118
(each bathroom is 7.8' by 7.6'). There are two shower stalls. The square footage is approximately
20 (each shower stall is 3.2' by 3.2').
There are two dressing rooms adjacent to the shower stalls that have a painted concrete floor. The square footage is approximately 62 (each dressing room is 6.7' by 4.7').
There is a hallway separating the bathrooms and the shower dressing rooms. It has a painted concrete floor. The square footage is approximately 86 (the hall is 16.1' by 5.3').
Government Furnished Property
GOVERNMENT FURNISHED PROPERTY
The following supplies will be furnished by the Government and provided to the Contractor as needed for use in the performance of this contract. This property shall be used and maintained by the contractor in accordance with the provisions to the “Government-Furnished Property” FAR clause contained in this contract:
a. Keys for to access buildings. Keys furnished to the Contractor shall be returned upon termination of the contract, and shall not be shared or duplicated. Contractor shall immediately notify the COR if any keys are lost.
b. Dumpster for refuse on site.
Contractor Furnished Supplies, Materials and Equipment
The contractor shall furnish all cleaning supplies and equipment. Supplies to be provided are: hand soap, paper towels, toilet paper, toilet seat covers, window cleaner, light bulbs, and fluorescent tubes, plastic trash bags, waxes, cleaners and any necessary cleaning supplies to complete the contract requirements. All cleaning supplies will have a disinfectant in it. Fluorescent tube must be "cool white super saver" tubes and supplied for all offices. A surplus of supplies such as toilet paper, paper towels, light bulbs, fluorescent tubes, Windex, and trash bags, will be kept on hand in case of emergencies, in the main offices and at the warehouse. Carpet cleaning, vacuuming, and resilient floor cleaning equipment and supplies necessary to adequately meet the cleaning specifications and perform this contract according to all the terms, are also to be supplied by the contractor. No flammable or explosive liquids such as gasoline will be stored. The Contractor shall submit a list of cleaning products to be used to the Contracting Officer’s Representative (COR), for approval at the pre-work conference and at any time that the approved product is replaced with a different product.
The contractor shall provide toilet tissue, hand soap, paper hand towels, plastic trash bags, paper bags and all other supplies necessary to perform the required services as stated herein. Supplies shall meet the existing equipment manufacturer’s specifications.
The Contractor will be permitted to store small quantities of Contractor-furnished supplies and equipment in the Government building. Supplies furnished by the Contractor shall be maintained in a neat, orderly manner by the Contractor when stored at a Government Facility within the designated area.
Contractor shall meet all OSHA requirements for storing cleaning compounds/chemicals if stored on
Government premises. If a piece of equipment needs repair or is determined unsafe by the COR or Safety
Officer, the Contractor shall immediately stop using the equipment. Defective equipment shall be repaired or replaced within 72 hours.
Biobased Products The Contractor must comply with Section 9002 of the Farm Security and Rural Investment Act of 2002
(FSRIA), Executive Order (EO) 13423, “Strengthening Federal Environmental, Energy, and Transportation
Management,” and the Federal Acquisition Regulation to provide bio-based products.
The Contractor shall utilize products and material made from bio-based materials (e.g., bio-based greases, bio-based hydraulic fluids, bio-based absorbents) to the maximum extent possible without jeopardizing the intended end use or detracting from the overall quality delivered to the end user. All supplies and materials shall be of a type and quality that conform to applicable Federal specifications and standards, and to the extent feasible and reasonable; include the exclusive use of biobased products. The
Contractor shall utilize the greatest number of biobased products available that meet applicable health and environment specifications. All supplies and materials to be used in the performance of work described herein are subject to the approval of the COR. The following is an example list of products that may be used in this contract for Janitorial Services for which bio-based products are available. The list is not all inclusive. It is desirable that vendors be able to supply the greatest number of bio-based products listed meeting the health and environmental specifications.

Each product submitted for use under this contract must be identified with at least one of these categories (some products may be listed under more than one category). Labeling shall be printed on all containers.

Bio-based products that are designated for preferred procurement under USDA’s Bio-Preferred program must meet the required minimum bio-based content as stated in the USDA Final Rule available at www.biopreferred.gov. The Contractor should provide data for their bio-based products such as bio-based content.
In addition to the bio-based products designated by the U.S. Department of Agriculture in the Bio-Preferred
Program, the Contractor is encouraged to use other bio-based products.
The Contractor shall submit with the initial proposal a complete list of bio-based products, indicating the name of the manufacturer, cost of each material, and the intended use of each of the materials that are to be used in carrying out the requirements of the contract. Additionally, the awarded Contractor on each anniversary date of the contract shall compile a complete list of bio-based products, including the information above, purchased to carry out the contract requirements. The Contractor shall list volume to be used and total cost for each individual product. This information will be used for reporting purposes.
The Contractor shall comply with the provisions at FAR 52.223-1, Bio-based Product Certification.
The Contractor shall comply with the clause at FAR 52.223-2, Affirmative Procurement of Bio based Products under Service and Construction Contracts.

Proposed bio-based custodial products which must be selected in accordance with the criteria included above.
At a minimum, must identify products by brand name for each of the product types.
NOTE: A Contractor may propose more than one product within a product category and/or propose a product or products addressing more than one product category.
Contractor Representative-The contractor shall provide a contract representative for the performance of the work. The name of this person and alternate(s) who shall act for the contractor when the contractor is absent shall be designated in writing to the contracting officer. The contractor representative or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contractor representative and alternate(s) must be able to read, write, speak, and understand English.
Government and Contractor Responsibilities:
Government Responsibilities - The Government will not be responsible for damage occasioned by fire, theft, and accident or otherwise, to the Contractor's supplies, materials, or equipment in storage. In addition, the
Government will not be responsible for damage to the Contractor's and his/her employee's personal belongings brought into the building.
Contractor Responsibilities - The Contractor shall not employ persons for work on the contract if such employee is considered by the Contracting Officer's Representative (COR) to be a potential threat to the health, safety, security, general well-being, or operation mission of the Agency.
a) The Contractor is responsible for loss, theft, damage, or destruction of any Government or personal items belonging or assigned to the occupant or user in any office or other rooms of the facilities when such acts can be proven, beyond a reasonable doubt, to be the fault of the Contractor or one of his/her employees, agents or subcontractors.
b) Anyone employed by the Contractor must be eighteen (18) years of age or older. No one under the age of 18 shall be permitted to work in the building or on the premises. The Contractor shall ensure that no one but the Contractor and/or his/her employees have access to the premises.
c) The Contractor is also responsible for ensuring that his/her employees do not disturb papers on desks, open desk drawers or cabinets, or use Government telephones or equipment, except as authorized, in writing, by the Contracting Officer's Representative (COR).
d) The Contractor shall supply some type of identification that must be worn at all times while on the premises by the Contractor and his/her employees. (FAR 52.204-9 -- Personal Identity Verification of
Contractor Personnel)
e) The Contractor shall inform the Contracting Officer's Representative (COR) of any items in need of repair/replacement.
f) Key Control - the Contractor will establish and implement methods of ensuring that all keys issued by the Government are kept secure in the Contractor's possession and are not used by unauthorized persons.
A. Keys issued by the Government shall not be duplicated.
B. The Contractor will report the occurrence of a lost key to the contracting Officer no later than the next duty day.
C. The Contractor will be required to reimburse the Government for replacement of locks or re-keying as a result of the Contractor loss of keys. In the event a mate key is lost or duplicated, all locks and keys for that system will be replaced by the Government and the total cost deducted from the monthly payment due the Contractor.
D. It is the responsibility of the Contractor to prohibit the use of keys issued by the government to persons other than the Contractor's employees. It is also the responsibility of the Contractor's employees to prohibit the opening of locked areas by Contractor's employees to permit entrance of persons other than Contractor's employees engaged in the performance of assigned work in those areas.
g) Energy Conservation - Contractor shall turn on lights in rooms only during the time needed to perform the required work and shall ensure that all lights, except those required to remain on, are turned off in each room as it is completed.
h) Safety – The Contractor shall be responsible for instructing company employees in appropriate safety measures. Caution should be used when placing or use of mops, brooms, or any equipment in traffic lanes or other locations. The Contractor shall provide, place, and remove appropriate warning signs for wet or slippery floor areas caused by cleaning or waxing applications. Cloths, mops or brushes containing a residue to wax or other combustible material subject to spontaneous ignition shall be disposed of or stored outside the buildings.
Additional Services
Upon notification, the Contractor shall perform special event cleaning or other services not outlined in the
PWS that may be required in any building, area or room covered under this contract. The Contracting Officer shall order services by issuing the specifications for the required work task(s) to the contractor. Pricing and terms will be negotiated prior to issuance and signature of a modification to the contract. Contractor shall begin emergency work, as determined by the Contracting Officer, within one hour of notification, which may be verbal. The Contracting officer or designated Contracting Officer Representative (COR) may notify the
Contractor as soon as a special event requirement is known, but no less than 24 hours prior to the event.
Completion schedule shall be determined for each event. Payment for emergency or special event cleaning services will be made by submission of an invoice and will be separate from the monthly billing under this contract.
Work Schedule - Cleaning services are to be performed as required in the frequency schedule shown below, and during the following tome periods;
Between hours of 6:00 PM and 8:00 AM Monday, Tuesday, Wednesday, Thursday.
Weekend cleaning will be between 4:30 PM Friday and 8:00 AM Monday, not to interfere with any
Government business.
Contractor will provide the Government with a work schedule for weekly, monthly, biweekly, quarterly and semi-annual work to be performed.
The contractor will not be responsible to clean on the following holidays, or the day the holiday is observed.

Unforeseen Building Closures – When an unforeseen building closure occurs on a regularly scheduled day of work, the Government shall have the following options:
a. To require the Contractor to perform work necessary to sustain operations on the following normal duty day.
b. To forgo the work and reduce payment due the Contractor accordingly for work not performed.
Work Standards - The contractor shall exercise care in work methods to prevent damage to any office equipment, windows, materials on desks, etc. All work under this contract shall be performed in a skillful and workmanlike manner. The Contracting Officer may, in writing require the Contractor to remove from work, any employee the Contracting Officer deems to be incompetent, careless, or otherwise objectionable for theft, possession and/or removal of materials, supplies, equipment, or any government-owned property, or for unsatisfactory performance. The Contractor shall be responsible for maintaining satisfactory standards of employee competency, conduct, appearance, and integrity, and shall be responsible for taking such disciplinary action with respect to their employees as may be necessary. The Contractor shall prohibit their employees from disturbing papers on desks, opening desk drawers or cabinets, or using telephones or office equipment provided for official Government use.
SERVICE DELIVERY SUMMARY
Daily Services- (To be performed Monday, Tuesday, Wednesday, Thursday, and one day between
Friday-Sunday).
a) Restrooms
1. Floors shall be swept daily. Floors are to be spot cleaned to remove all obvious surface dirt.
2. Water closets and urinals shall be washed and sanitized. No rust or encrustation shall remain.
Traps shall be maintained free from odor at all times. Washbasins and counters shall be cleaned.
3. Mirrors, shelving, dispensers, and chromium fixtures shall be damp wiped and polished.
4. Surfaces. All other surfaces shall be spot cleaned and horizontal surfaces dusted.
5. Dispensers and receptacles. Paper towel waste receptacles shall be emptied and towel soap and toilet paper dispensers serviced prior to occupant's official starting time. Sanitary napkin receptacles shall be emptied, cleaned, disinfected, and provided with new bag liner daily.
6. Ephraim Warehouse Restrooms. All daily services above shall be performed during scheduled cleaning of the restrooms at the warehouse.
7. Ephraim Warehouse Dressing Rooms/Showers shall be washed and sanitized. No rust or encrustation shall be remaining. Traps shall be maintained free from odor at all times.
b) Room Cleaning
Office areas, coffee rooms, and conference rooms, at the main office, shall receive the following daily cleaning:
1. Wastebaskets shall be emptied and wastepaper and trash removed to main deposal area.
2. Floors. The tiled floors areas are to be swept and damp mopped to remove all obvious dirt, dust, and litter.
3. Carpet. The full-carpeted area is to be spot vacuumed to remove all obvious surface dirt from traffic areas and from under and around furniture.
4. Washbasins. In the coffee room and the janitor room area shall be cleaned and the counter damp-wiped and dried, as necessary, to keep them in a clean condition. Paper towels shall be supplied where towel holders are provided.
5. Dust Horizontal surfaces of tables, filing cabinets, and bookcase sections shall be dusted to remove all obvious dust, litter, or dirt.
6. Janitor's Closets. To be maintained in a sanitary and clean condition daily. MSDS sheets will be posted where cleaning supplies are stored.
c) Lobby Front Entrance, and Corridors
1. The front entrance floor shall be swept and damp mopped.
2. All carpeted floors shall be vacuumed.
3. All display cases in the front entrance and lobby shall be dusted and glass washed and dried.
4. The receptionist desk and counter shall be dusted, and glass washed and dried.
5. All wastebaskets shall be emptied, and trash removed to main outside dump- ster.
6. Drinking fountains shall be cleaned and wiped dry.
d) Waste Collection and Removal - Daily
All refuse, trash, and garbage and shredded paper shall be collected and removed from both buildings and placed in the dumpster provided at each location. Plastic wastebasket liners shall be used in all wastebaskets and replaced weekly or more frequently if needed. If the dumpster is full, contractor must bag or box refuse and leave beside the dumpster so trash does not blow away.
The refuse, trash, and garbage and shredded paper shall be collected and removed from the warehouse office and restrooms each time cleaning is performed as scheduled.
Weekly Services - (To be performed at any time between 4:30 PM Friday and 8:00 AM Sunday as long as not interfering with work activiti.es when the front office is open.
A. Restrooms
1. Restroom floors shall be wet mopped (scrubbed if needed) in the offices weekly, and in the warehouse (each time the restrooms are scheduled for cleaning).
B. Room Cleaning
1. Dust horizontal surfaces of files, chairs, tables, book cases, window sills and other furniture with a treated dust cloth. Glass desk-tops shall be cleaned with glass cleaner.
2. Floors. All full tile areas, all front/rear entries with tile, and coffee rooms are to be swept with a treated dust mop to remove all dirt, dust, and litter.
3. Carpet. The full carpeted areas and front/rear entries shall be vacuumed to remove all dirt, dust, and litter.
4. Mats. Any mats in the front and rear entrances shall be removed from the building and vacuumed to remove excess dirt.
5. Wastepaper receptacles. Plastic liners will be replaced weekly or more often if needed.
Monthly Services
A. Restrooms
1. Restroom wastepaper receptacles in all offices and warehouse shall be damp wiped inside and out.
1. Walls, Partitions, doors in all offices and warehouse shall be damp wiped.
2. All vinyl tile floors shall be scrubbed and buffed.
3. Peaks showers shall be scrubbed and sanitized.
B. Room cleaning, entrances, lobby and corridors. Warehouse, conference rooms, and hallways
1. Walls shall be spot cleaned.
2. Glass in doors, pictures, and bookcases shall be cleaned.
3. Vinyl tile or tile floors shall be scrubbed (with clean water each time) and buffed to a high luster.
4.Wastepaper receptacles - damp wipe inside and out.
Quarterly Services - (December, March, June, September)
A. Room Cleaning
1. Dusting. All surfaces in the building approximately 70 inches or more from the floor shall be cleaned by dusting or vacuuming, including light fixtures.
B. Floor Cleaning
1. Tiled Flooring. All areas having tiled flooring shall be stripped, waxed, and buffed. Use non-slip floor wax. This applies only to the areas that require waxing. This includes the floors into the restrooms and hallways at the warehouse. The computer room floor must be scrubbed, stripped, and waxed without using power equipment and work must be scheduled with the Government.
Semi-Annual Services - (November and May)
A. All light fixtures shall be washed
B. Windows. All windows at all facilities will be washed inside and out.
C. Blinds. All blinds at all facilities will be dusted, washed, and dried.
Once a Year Services -(to be done in May)
A. Carpets and Mats. All carpeted areas will have desks and chairs moved and vacuumed well then put back in place. All carpets and mats will be shampooed.
Services to be Performed as Required
a) Carpet. Stains shall be removed from all carpets in the buildings as needed.
b) Entrances. Front and rear entrances shall be kept clean to provide a food appearance. Sites that have glass entrances and front lobby areas with glass shall be kept clean to provide a good appearance. All glass will be washed as needed.
c) Other Interior Surfaces. Door and window ledges, heating, and air conditioning grills, fire apparatus, and doors shall be dusted.
d) Windows shall be closed and all exterior doors closed and locked. Alarm set each day after janitorial services are performed at locations with alarms.
e) Light Bulbs and fluorescent lamps shall be replaced as needed.
CONTRACTOR'S SUBMITTALS
A. Schedule- Submit a schedule at least 1 month in advance of required work showing dates, "weekly", "monthly", quarterly", and "semi-annual" work will be performed. Inform the
Government of schedule changes at least one week in advance.
B. MSDS Sheets - Provide MSDS sheet Schedule - Submit a schedule at least 1 month in advance of required work showing dates, "weekly", "monthly", "quarterly", and "semi-annual" work will be performed for all materials used or stored in Government Offices.
C. Cleaning Product Information - Manufacturer's or supplier's information describing the contents and recommended uses for each cleaning product used.
D. Personnel - Provide and keep current a written list of the names of all contractors' employees performing duties under this contract. This shall be in accordance with:
Standard Procedures The facility may never have more than five (5) valid customer complaints. The Contracting Officer's
Representative shall notify the Contracting Officer for appropriate action in accordance with FAR
52.212-4, Contract Terms and Conditions-Commercial Items (JAN 2017) or the appropriate Inspection of
Services clause, if any of the above service areas exceed two customer complaints.
The COR will inspect all work tasks, Twice monthly. The inspection is required to ensure Contractor compliance with the appropriate paragraphs of the SOW each time the service{s) is/are performed and record results of inspection noting the date and time of the inspection. If an inspection indicates unacceptable performance, the COR will notify the contract manager of Quality Control Inspector of the deficiencies. The Contractor shall be given 24 hours, after notification to correct the unacceptable performance.
Contractor Personnel Qualifications
The contractor shall provide qualified personnel. Contract personnel shall refrain from familiarizing themselves with or disturbing any books, papers, or other materials in any of the offices. Only the
Contractor or employees of the Contractor will be allowed in the building or on the grounds of the
Manti-La Sal National Forest covered in this contract. The Contractor shall notify the Government, by email or telephone, immediately when employees quit or new employees are hired before they start working in any Coconino facility. Children and pets are not allowed in the building or surrounding area.
Exceptions must be approved by Government representative. Any equipment in the building including but not limited to, radios, typewriters, computers, faxes or copy machines shall not be used by the contractor or the contractor's employees.
Security
A. All outside doors shall remain locked while contractor performs work and contractor shall assure that all entrance gates, doors, and windows are locked after services are perform d and the security of the building is maintained.
B. Extinguish all unnecessary lights.
C. Ensure that the coolers and heaters are turned off or turned down as designated.
D. Ensure that all entrance doors and windows are closed and locked after services are performed and the that security of the building is maintained.
E. Ensure that any cleanser or cleaning solution used in the performance of janitorial services does not cause damage to Government property.
F. Janitor closet areas shall be kept clean, neat, and odor free, remove all trash and soiled mop water daily.
Performance Requirements Standards
Performance requirement standards are described in the Performance Requirements Summary Tables above for daily, weekly, monthly, and semi annual work. The Contractor is expected to ensure services are provided or performed to the appropriate standards through the Contractor's Quality
Control Plan. Except as stated in the summary tables, services required by this contract are generally of such a nature that nonconforming or incomplete performance disclosed by Government inspection is not subject to correction by re-performance or late performance, and will result in a deduction being made to the contract price for the month.
Inspection Procedure
Inspection of the office buildings to determine the acceptability of the workmanship will be made by the Government and any deficiencies encountered will be called to the attention of the
Contractor. Correction of all deficiencies will be made prior to re-inspection and acceptance of work by the Government in conformance with the work standards. Inspections will be done at a minimum of once a month or more frequently if needed.
Any Government personnel or visiting public may observe unacceptable services, incomplete work or required services not performed and may contact the CO or the COR to communicate a complaint.
Customer complaints will be substantiated by the COR for validity. Should the COR determine that the complaint is valid, the COR will inform the Contractor orally and in writing within 48 hours of validation of the complaint. Refer to the Performance Requirements Summary Tables for remedy for non-conforming services.
A. Acceptance - Workmanship shall be of an acceptable quality such as would be found in a business office visited by the general public and meet the acceptable quality level (AQL) listed in summary.
B. Payments - Payment will be made monthly for work meeting or exceeding the AQL completed and accepted by the Government covering the previous month's service. Upon acceptance, the Government will pay monthly the appropriate amount determined by the occurrence of the services as stated in the Schedule of Items.
C. Deductions - anytime the performance of services falls below the AQL, a deduction may be made for services that cannot be re-performed or poorly performed work. For documented poor performance a reduction of I0%-30% may be reduced on the invoice for the applicable line items depending upon the severity of the poor performance. For non-performance, no payment will be issued.
Definitions Acceptable Quality Level (AQL). The maximum allowable deviation from acceptable performance that may occur before the Government will invoke payment deduction. An AQL does not allow a contractor knowingly to offer defective services, but limits reduced payment to circumstances in which defective performance results in a measurable reduction in the value of services rendered.
Defective Service: A unit of service which contains one or more defects, or nonconformance with specified requirements.
Performance Requirements Summary (PRS): Identifies the key service outputs of the contract that will be evaluated by the Government to assure contract performance standards are met by the contractor.
Quality Assurance. Those actions taken by the Government to assure services meet the requirements of the Performance Work Statement (PWS).
Contracting Officer’s Representative (COR): A Government person appointed by the contracting officer to help the contracting officer with contract administration and surveillance.
Government Inspector: A Government person responsible for surveillance of contractor performance.
Quality Assurance Surveillance Plan (QASP): An organized written document used for quality assurance surveillance. The document contains specific methods the Government will use to perform surveillance of the contractor.
Quality Control: Those actions taken by a contractor to control the performance of services so that they meet the requirements of the PWS.
Random Sampling: A sampling method in which each service output in a unit has an equal chance of being selected.
Material Safety Data Sheets (MSDS): Information that accompanies chemicals in the workplace.
MSDS includes details such as the risks, precautions, and first aid procedures associated with the chemical.
CONTRACT PERFORMANCE PERIOD
Ephraim Ranger District Janitorial Contract
Base period beginning October 1, 2023 – September 30, 2024
There is a possibility for 4 (four) option years:
Option Year 1 – October 1, 2024 – September 30, 2025
Option Year 2 – October 1, 2025 – September 30, 2026
Option Year 3 – October 1, 2026 – September 30, 2027
Option Year 4 – October 1, 2027 – September 30, 2028
Restroom: Restroom areas will be visually inspected to ensure that all Floors are swept daily. Floors are spot cleaned to remove all obvious surface dirt. Water closets and urinals are washed and sanitized. No rust or encrustations remain. Traps shall are maintained free from odor at all times. Washbasins and counters are cleaned. Mirrors, shelving, dispensers, and chromium fixtures are damp wiped and polished. All other surfaces are spot cleaned and horizontal surfaces dusted. Paper towel waste receptacles are emptied and towel soap and toilet paper dispensers serviced prior to occupant's official starting time. Sanitary napkin receptacles are emptied, cleaned, disinfected, and provided with new bag liner daily. Ephraim Warehouse
Restrooms. All daily services above are performed during scheduled cleaning of the restrooms at the warehouse. Ephraim Warehouse Dressing Rooms/Showers are washed and sanitized. No rust or encrustation remaining. Traps maintained free from odor at all times. Floors are wet mopped (scrubbed if needed) in the restrooms weekly, and in the warehouse each time the restrooms are scheduled for cleaning). Wastepaper receptacles in all restrooms and warehouse are damp wiped inside and out. Walls, Partitions, doors in all restrooms and warehouse are damp wiped. Peaks showers are scrubbed and sanitized.
Floors: Flooring throughout the facility will be visually inspected to ensure that the tiled floors are swept and damp mopped to remove all obvious dirt, dust, and litter. stripped, waxed, and buffed. Use non-slip floor wax.
This applies only to the areas that require waxing. This includes the floors into the restrooms and hallways at the warehouse. The computer room floor must be scrubbed, stripped, and waxed without using power equipment and work must be scheduled with the Government. All vinyl tile floors are scrubbed and buffed.
Carpets: Carpets and Mats. The full-carpeted area is spot vacuumed to remove all obvious surface dirt from traffic areas and from under and around furniture. All carpeted areas have had desks and chairs moved and vacuumed well then put back in place. All carpets and mats are shampooed.
Dusting: Horizontal surfaces will be visually inspected including tables, filing cabinets, bookcase sections, light fixtures, blinds, door and window ledges. Heating and air conditioning grills, fire apparatus, and doors to ensure that they are free of obvious dust, dirt, and cobwebs.
Glass: All interior glass surfaces will be inspected to ensure that the glass is clean and free of streaks and smudges. Glass surfaces include windows, plexiglass, mirrors, all display cases in the front entrance and lobby, he receptionist desk glass, glass desk-tops glass in doors, pictures, and bookcases, and front and rear entrances.
Drinking Fountains and Sinks: All drinking fountains and sinks will be visually inspected to ensure that they are clean, sanitized, free of water marks, and any other debris or encrustation.
Waste Baskets: All waste baskets shall be visually inspected to ensure that all trash is removed.
Lights: All interior lights will be inspected to see if the Contractor is replacing burnt-out bulbs.
GOVERNMENT REMEDIES
The CO shall follow FAR 52.249-8 Default (Fixed-Price Supply and Service), for Contractor’s failure to perform satisfactory services or failure to correct non-conforming services.
PERFORMANCE-BASED CONTRACTING
Quality will be measured based on the standards and objectives listed in Cleaning Frequency and Quality
Assurance section of this document.
IDENTIFICATION OF CONTRACTOR EMPLOYEES:
All contract personnel are required to identify themselves as Contractors. In any situations where their
Contractor status is not obvious to third parties, they must make their status known to avoid creating an impression that they are Government employees.

MEASUREMENT AND PAYMENT
Payment will be made monthly for the janitorial services that have been satisfactorily provided during that month. Work which does not meet specifications, and which is not corrected will be handled in accordance with

(xii) 52.217-8 Option to Extend Services (Nov 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 3 days of the expiration of the contract.
(xiii) 52.217-9 Option to Extend the Term of the Contract (Mar 2000)
(a)The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the
Government to an extension.
(b)If the Government exercises this option, the extended contract shall be considered to include this option clause.

(xv) 452.204-70 Modification for Contract Closeout (DEVIATION JUL 2022) Upon contract closeout for contracts utilizing Simplified Acquisition Procedures (SAP) according to FAR 13:
(a) If unobligated funds in the amount of $1000 or less remain on the contract, the Contracting Officer
(CO) shall issue a unilateral modification for de-obligation. The contractor will receive a copy of the modification but will not be required to provide a signature. The CO shall immediately proceed with contract closeout upon completion of the period of performance, receipt and acceptance of supplies or services, and final payment.
(b) If unobligated funds of more than $1000 remain on the contract, the CO shall issue a bilateral modification for de-obligation. The contractor will receive a copy of the modification and will be required to provide a signature. (The CO may also request a Release of Claims be completed by the contractor, although not required for contract and orders using SAP procedures.) If the bilateral modification and Release of Claims are not returned to the CO within 60 days, the CO shall release the modification as unilateral and proceed with contract closeout upon completion of the period of performance, receipt and acceptance of supplies or services, and final payment.

Personal Identity Verification of Contractor Employees
(a) The contractor shall comply with the personal identity verification (PIV) policies and procedures established by the Department of Agriculture (USDA) Directives 4620-002 series.
(b) Should the results of the PIV process require the exclusion of a contractor's employee; the
Contracting Officer will notify the contractor in writing.
(c) The contractor must appoint a representative to manage compliance with the PIV policies established by the USDA Directives 4620-002 series and to maintain a list of employees eligible for a USDA
LincPass required for performance of the work.
(d) The responsibility of maintaining a sufficient workforce remains with the contractor. Contractor employees may be barred by the Government from performance of work should they be found ineligible or to have lost eligibility for a USDA LincPass. Failure to maintain a sufficient workforce of employees eligible for a USDA LincPass may be grounds for termination of the contract.
(e) The contractor shall insert this clause in all subcontracts when the subcontractor is required to have routine unaccompanied physical access to a Federally-controlled facility and/or routine unaccompanied access to a Federally-controlled information system.

(f) The PIV Sponsor for this contract is a designated program point of contact, which in most cases is the
COR, unless otherwise specified in this contract. The PIV Sponsor will be available to receive contractor identity information from * (hours and days) to * (hours and days) at * (office address for registration). The Government will notify the contractor if there is a change in the PIV Sponsor, the office address, or the office hours for registration; however, it is the contractor's responsibility to meet all aspects of paragraphs (c), (d), and (e).
