Description of Requirement

C.1 Scope of Work:

The interior of the Middle Fork Guard Station bunkhouse (MFGS bunkhouse) requires use of a respirator. 12” heaving has been observed in the kitchen flooring. Apparent mold was observed on most of the trim, flooring, doors, and anything organic inside the building. In the basement, no organic substrate was found without apparent mold. The bedrooms in the east addition contain minimal apparent mold. No apparent mold was observed in the attic, attic insulation, or spray foam in wall cavities.

Work will consist of removal of all mold hazards from facility that was impacted by water damage, and mitigation of further mold growth at the MFGS bunkhouse at 3600 Sinks Canyon Rd (Hwy 131) outside of Lander, Wyoming.

All work must comply with IICRC S520 Standard and Reference Guide for Professional Mold Remediation.

Work will include the following:

• Complete drying of the basement prior to work performed.

• Containment with negative air and proper bagging of contaminated materials will be required to minimize exposure to other buildings, people, and livestock on the premises.

• Demolition/removal of mold-contaminated materials, including drywall, insulation, flooring, subflooring, and majorly affected interior wood. Interior wood to be removed will include all cabinets. Removed material must be disposed of offsite by contractor.

• Minorly affected interior surfaces should be cleaned/treated rather than removed.

• Chemical Based cleaning/abrading of any non-removable wood/organic substrate will include interior doors, all door jams, exterior doors, and all wooden components of the windows.

• Surfaces exposed by demolition, including insulation, studs, subfloor, and joists are to be treated via sodium bicarbonate blasting or USFS approved alternative.

• Surfaces exposed by demolition are also to be sealed with USFS approved anti-fungal compound to limit possibility of future mold growth.

• Scrub the air with HEPA Vacuum/Air Scrubber following any dust producing cleaning/abrading to reduce airborne mold spores to an acceptable threshold.

• Building structure, exterior, roofing, and windows (excepting trim) must remain undamaged and stay with the structure. No lead-based paint or asbestos hazards exist at the job site.

• Follow-up air testing will be performed by following industry standards.

• Contractor is expected to leave job site free of any materials brought to the site by themselves or any subcontractors prior to closeout.

• Demolition/removal work in the basement will include removal of shower stalls, mirrors, contaminated plumbing fixtures, and all contaminated wood paneling. There is a section of stucco/expanded metal ceiling in the utility room that will require demolition.

• The wooden staircase is a select area that will need to remain in place and be remediated with chemical-based cleaning/abrading, followed by encapsulation.

Quality and effectiveness of the mold mitigation will be based on preliminary mold tests and post mold mitigation clearance testing following mold mitigation while containment is still in place, 2-5 days after the work is completed, to ensure mold was fully abated and safe conditions exist prior to renovations. Mitigation shall continue until mold levels are less than outdoor levels, below 14 spores per cubic foot, or below the level recommended by a certified laboratory on all samples taken within the facility. Air sampling was initially conducted using a Zefon Bio Pump Plus calibrated to 15 LPM with Air O Cell sampling media. Total air volume of 75L was collected per sample, at the following locations:

1. FS-01 Background

2. FS-02 Kitchen

3. FS-03 East Bedrooms

4. FS-04 Basement

All air samples analyzed showed elevated levels of mold including but not limited to Stachybotry. The initial air quality inspection report is attached.

A “Mold Remediation Project in Progress” sign will be placed at the job site egress along with a packet containing a chemical list, list of workers, consultant protocol, sign-in log, and floor plan. Emergency exists must be clearly marked. Personal protective equipment (PPE) shall be put on prior to entering the bunkhouse. Respirators should be wiped down with a disinfectant wipe following each use.

The following PPE is to be worn by all workers, supervisors, and others entering the MFGS bunkhouse:

- Half-face respirator, N-95 mask, or full-face respirator.

- Tyvek coveralls with a hood, ankles, and wrists taped if no elastic is present.

- Goggles if an N-95 or half-face respirator is used.

- Latex gloves under work gloves

- Work boots

- Hard hat if deemed necessary by FS representative.

There is to be no eating, smoking, or drinking in the containment area. PPE is to be always worn when inside the MFGS bunkhouse. Do NOT reuse disposable PPE. Dispose of disposable PPE (Tyvek overalls and latex gloves) after each use. Anti-microbials are to be used in accordance with manufacturer’s instructions. Critical barriers are to be installed with tape and adhesive (along with staples if necessary) using double 6-mil fire retardant polyethylene. Air scrubbers are to be ducted to the outside. They are to be maintained daily during the remediation project and cleaned in a contained area following the remediation. All used filters are to be disposed of in 6-mil debris bags. HEPA vacuums are to be maintained throughout the course of the remediation. Bags should be changed when nearing capacity and are to be disposed of in 6-mil waste bags prior to leaving contained areas. All work will be conducted in accordance with the FP-14, the Federal Acquisition Regulations (FAR), and the Forest Service Supplemental Specifications (FSSS).

Use of Premise:

The Middle Fork Guard Station serves as seasonal human and horse housing for the Washakie Ranger District of the Shoshone National Forest. The staging areas will need to be shared between the contractor and Forest Service employees. Always keep driveways, loading areas, and entrances serving premises clear and available to Government, Government's employees, and emergency vehicles.

Schedule deliveries to minimize use of driveways and entrances. Schedule deliveries to minimize space and time requirements for storage of materials and equipment on-site.

The contractor has use of the site’s electricity, sinks, toilets, and heating. The building will not be occupied during remediation, so utility services may be interrupted. However, local shutoffs should be used to prevent interrupting service to trailers and corrals on property.

Applicable OSHA and environmental laws and landfill policies will apply for waste disposal.

Precautions must be used in transportation and disposal of waste material.

The contractor may apply for a Forest Service permit to have a self-contained camper on site. Only utilities available will be electricity, and water, dependent on weather. Occupancy of the building is not permissible.

C.2 Project Location:

C.5 Contract Time:

Work shall be completed no later than 60 calendar days after effective date of NOTICE TO PROCEED, subject to such extensions as may be authorized. Anticipated work will begin 14 days after award.

C.6 Work Plan and Schedule:

The Contractor shall provide a work plan and schedule that demonstrates how the work will be completed and in what time frame at the pre work meeting. No work shall start until the work plan and schedule is approved by the CO.

C.7 Quality Control and Materials:

Government inspections are for the sole benefit of the Government and does not relieve the contractor of responsibility for providing adequate quality control measures. The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with specifications. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services or materials.

(d) Comptroller General Examination of Record. Upon contract closeout for contracts utilizing Simplified Acquisition Procedures (SAP) according to

FAR 13:

(a) If unobligated funds in the amount of $1000 or less remain on the contract, the Contracting Officer (CO) shall issue a unilateral modification for de-obligation. The contractor will receive a copy of the modification but will not be required to provide a signature. The CO shall immediately proceed with contract closeout upon completion of the period of performance, receipt and acceptance of supplies or services, and final payment.

(b) If unobligated funds of more than $1000 remain on the contract, the CO shall issue a bilateral modification for de-obligation. The contractor will receive a copy of the modification and will be required to provide a signature. 452.236-74 Control of Erosion, Sedimentation, and Pollution (NOV 1996)

(a) Operations shall be scheduled and conducted to minimize erosion of soils and to prevent silting and muddying of streams, rivers, irrigation systems, and impoundments (lakes, reservoirs, etc.).

(b) Pollutants such as fuels, lubricants, bitumen’s, raw sewage, and other harmful materials shall not be discharged on the ground; into or nearby rivers, streams, or impoundments; or into natural or man-made channels. Wash water or waste from concrete or aggregate operations shall not be allowed to enter live streams prior to treatment by filtration, settling, or other means sufficient to reduce the sediment content to not more than that of the stream into which it is discharged.

(c) Mechanized equipment shall not be operated in flowing streams without written approval by the Contracting Officer.

452.237-74 Key Personnel (FEB 1988)

(a) The Contractor shall assign to this contract the following key personnel: Project Manager

(b) During the first ninety (90) days of performance, the Contractor shall make no substitutions of key personnel unless the substitution is necessitated by illness, death, or termination of employment. The Contractor shall notify the Contracting Officer within 15 calendar days after the occurrence of any of these events and provide the information required by paragraph (c) below. After the initial 90-day period, the Contractor shall submit the information required by paragraph (c) to the Contracting Officer at least 15 days prior to making any permanent substitutions.

(c) The Contractor shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, complete resumes for the proposed substitutes, and any additional information requested by the Contracting Officer. Proposed substitutes should have comparable qualifications to those of the persons being replaced. The Contracting Officer will notify the Contractor within 15 calendar days after receipt of all required information of the decision on substitutions. The contract will be modified to reflect any approved changes of key personnel.

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Past Performance Technical Price

Technical Factors. Contractor shall submit a valid IICRC S520 Standard and Reference Guide for Professional Mold Remediation. Certificates will be evaluated on a meets/doesn't meet criteria.

Past Performance: The Government will evaluate the Offeror’s past performance on same or similar work* and will consider information submitted by the Offeror, obtained from references or obtained by the Government from other sources that is relevant to the following elements:

(i) Quality of Service.

(ii) Timeliness of Performance.

In evaluating past performance, the Government may consider the quantity and relevance of the work for which past performance data is collected, the currency and relevance of the information collected, the context of the data and the general trends of Offeror’s performance.

In accordance with Federal Acquisition Regulations, Offerors that have no record of past performance will receive a neutral rating.

The Government will also consider past performance information provided for predecessor companies and personnel who have relevant experience.

Reporting Systems (CPARS) ratings, and the Federal Awardee Performance and Integrity Information System (FAPIIS). If an offeror has no record of past performance ratings in the above system, this factor will be treated as neutral, resulting in an acceptable rating. The failure of an offeror to meet the requirements of a factor will result in an unacceptable rating.

An award will not be made to any offeror who receives an Unacceptable Past Performance rating:

Acceptable Offeror clearly meets the requirements of the solicitation.

Unacceptable Offeror does not clearly meet the requirements of the solicitation.

Price: The Government may award a contract on the basis of initial offers received, without discussions. Therefore, each initial offer should contain the offeror’s best terms from a price and technical standpoint.

The Government reserves the right to make an award to other than the lowest priced offeror or to the offeror with the highest non-price evaluation if the Contracting Officer determines that to do so would result in the best value to the Government. Proposals that do not address one or more criteria or subfactors, may not be considered further for award.

All items on the Schedule of Items must be priced in order to be considered for award.

(b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

52.217-5 Evaluation of Options. (July 1990)

Except when it is determined in accordance with FAR 17.206(b) not to be in the Government’s best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the option(s).

52.237-1 Site Visit (APR 1984)

Offerors or quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award.

(a) The use in this solicitation of any Federal Acquisition Regulation (48 CFR Chapter 1) provision with an authorized deviation is indicated by the addition of "(DEVIATION)" after the date of the provision.

(b) The use in this solicitation of any Agriculture Acquisition Regulation (48 CFR Chapter 4) provision with an authorized deviation is indicated by the addition of "(DEVIATION)" after the name of the regulation.
