Quality is calculated using a formula multiplying the ratio of satisfactory above-ground trees to planted trees by the ratio of satisfactory dug trees to total dug trees, expressed as a percentage. The Government will conduct its own surveillance inspections at variable intensities and compare results to contractor plots; if results differ by 5% or less, payment is based on contractor results, otherwise government results apply.
Performance thresholds establish 90% as the target quality standard and 80% as the minimum acceptable quality. For subitems scoring 80-90%, contractors may request replanting if seedling stock is available. Subitems below 80% are rejected with no initial payment; contractors may replant once if approved by the Contracting Officer and with seedling availability, bearing all reinspection costs. Failure to reach 80% quality within two consecutive workdays of written notice may result in contract termination and default. Reinspection requests must be submitted in writing within three calendar days; if a second inspection varies less than 5% from the first, the contractor pays reinspection costs.

QUALITY ASSURANCE SURVEILLANCE PLAN
Willamette NF, 2026 Middle Fork Spring and Fall Tree Planting
CONTRACTOR QUALITY CONTROL PLAN
A. Quality Control Plan
The Contractor shall provide a Quality Control Plan (QCP) as part of this Technical Proposal. The Plan must demonstrate how the Contractor shall assure that quality of performance meets the objectives and requirements of the contract. Be specific about how the non-planting employees (Foreman and Inspector) will work together to provide the necessary oversight to recognize and correct deficiencies, in real-time, to verify planting quality both above ground and below ground.
Additionally, the QCP shall specify:
- How quality will be monitored to ensure performance standards are met.
- How the work will be supervised.
- How results of the monitoring will result in quality performance.
- Identify the personnel responsible for performing quality control.
B. Contractor Quality Control Inspection
The Contractor shall assure themselves that performance meets contract specifications, in accordance with FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, paragraph (b), prior to requesting the Government to inspect for payment or acceptance. The Contractor shall contact the Contracting Officer upon completion of a work area. The Contracting Officer may observe the Contractor’s inspection at any time and shall otherwise have unlimited access to the inspection data.
ADDITIONAL INSPECTION AND ACCEPTANCE REQUIREMENTS
The Contractor may be put on notice of non-compliance (or other contract action taken) for failure to conduct plot inspections in accordance with the QCP. The Contractor’s inspector is not permitted to modify plot deficiencies nor influence the planting quality by indicating upcoming plot locations to the planters.
In the event the Contractor falsifies inspection results as determined by the Contracting Officer, the Government may declare the Contractor to be in default of the contract. Additionally, the Government may refer such falsification to the U. S. Attorney’s Office for possible prosecution pursuant to 18 U.S.C. 1001 (Falsification of Government Documents) or 18 U.S.C. 287 (Fraud).
CONTRACTOR INSPECTION PROCEDURES
Planting Quality Assurance Plots: the Contractor will be required to take representative sample plots (at 1/50th acre) of the unit to verify planting quality both above ground and below ground using the R6 Standard plot procedure (Exhibit G) and will record results using the R6-2400-113 Planting Inspection Sheet (Exhibit H):
1. Determine total number of plots required for each unit based on a minimum of one 1/50th acre plot per two acres of planting work (1% sample intensity). A minimum of 3 plots is required for all units, regardless of size.
2. Establish plot centers with easy to see flagging labeled with plot number, date, and initials.
3. Determine slope percent and adjust plot radius accordingly.
4. Determine plantable and unplantable spots, number of planted trees, species mix, micrositing, site preparation, planting depth, stem position, spacing, damage to trees, and firmness. Check for contract compliance.
5. Determine number of trees to inspect underground and dig for root placement and other underground requirements.
6. Calculate quality for individual planting units based on the following formula. Calculations are based on planted trees. Under no circumstances will quality be averaged among multiple items or sub-items.

GOVERNMENT QUALITY ASSURANCE SURVEILLANCE PLAN
The Government will monitor progress to assure the Contractor is providing the quality performance indicated in their Quality Control Plan (QCP) and that work meets contract performance standards. Surveillance will consist of visual observations of the entire operation including the Contractor’s inspection procedure and sample plots to determine compliance with the specifications. A portion of the Government’s plots will be reinspection of plots installed by the Contractor. Plots will be located throughout the planting area to obtain a representative sample of the work. The Government may choose to sample at higher or lower intensities based on visual observations. Each contract unit will be inspected separately, and inspection results will not be averaged with those of other contract units. The Contractor may observe plot procedures while they are underway.
Upon completion of the entire unit, the Government will compare results with Contractor plot sheets to determine acceptability. If the Government’s and the Contractor’s inspection results are within 5% of each other, pay will be calculated based on the Contractor’s plot results. If there is a difference of greater than 5% between inspection results, the Government’s plot results will be used to calculate payment. Payment must be calculated based on a minimum sample intensity of 1%.
The Government may choose to accept the Contractor’s inspection results regardless of a greater than 5% difference in quality, if all the following conditions are met:
1. The Government sampled at less than a 1% intensity.
2. The Government’s visual inspections and plot results indicate acceptable quality.
3. The Government has found the Contractor’s inspection procedures to be acceptable based on visual observations as well as reinspection of plots.
UNSATISFACTORY PERFORMANCE
If the planting quality for any subitem falls below 90 percent and remains above 80 percent:
1. Contractor may be permitted to replant the subitem in order to achieve a higher planting quality percentage. Replanting will be subject to availability of seedling stock and shall be requested in writing by Contractor. Following completion of replanting, a new inspection shall be made by Contractor.
Payment will be based on the new inspection.

2. Payment may be made as a percentage of the unit price (see Section 5.2.6 Payment). When inspection results are below 90 percent and excess trees constitute any part of the deficiency, payment will not be made until the deficiency is corrected by Contractor.
Minimum acceptance quality is 80 percent. If the planting quality for any subitem falls below 80 percent:
1. Work on the subitem will be rejected and no payment made.
2. Contractor may replant the subitem, only if additional supplies of suitable seedlings are available and rework is approved by the Contracting Officer. Only one replant will be permitted. Payment will be based on a new inspection made after replanting. Contractor shall bear the cost of all reinspections after replanting.
3. The Government will immediately notify Contractor in writing and order improvement of the quality of future work. If the quality of future work is not raised above 80 percent within two consecutive workdays after receipt of notice, Contractor’s right to proceed may be terminated and the contract considered in default. Repeated failure to perform work at or above 80 percent acceptable performance shall also be considered reason for contract termination and default action.
REWORK AND REINSPECTION AFTER REWORK
If original inspection results are unacceptable to Contractor and a second inspection is requested without rework, the same inspection procedure will be used following a different plotline. If the second inspection show less than a 5 percent variance from the first inspection, the Contractor shall pay the cost of the second inspection, and results of the second inspection will be used in determining payment. Reinspection requests shall be in writing and submitted within three calendar days of the original inspection notification.
When inspection results are below 80 percent and the deficiency is correctable, payment will not be made until the deficiency has been corrected. Reinspection after rework will be made in the same manner as the first inspection but on different plot lines. The Contractor shall pay for inspections necessitated by the rework.
