PERFORMANCE RISK ASSESSMENT QUESTIONAIRE

Please provide your candid response to the following questions. The information that you provide will be used in the awarding of federal contracts. Therefore, it is important that your information be as factual and accurate as possible. If you do not have the appropriate knowledge of or experience with the company in question, please forward this questionnaire to the person who does. Please return within three (3) days to Reginald Brown via e-mail reginald.brown@fsis.usda.gov. Evaluators: Please do not send a copy of your evaluations to the requesting vendor. Thank you.

PAST PERFORMANCE INFORMATION

The offeror identified below is submitting a proposal to the USDA, Food Safety and Inspection Service for ISO Standard 17025, 17034, and 17043 for the Accredited Laboratory Program (ALP). Please provide your assessment of the extent of relevant experience and past performance as requested by the following questionnaire. Space is provided for comments (additional pages may be used if desired). Comments would be particularly appreciated when applicable. A Rating Guideline is attached for your use in assigning a performance level rating for each of the factors in Section V.
