PERFORMANCE WORK STATEMENT / QUALITY ASSURANCE PLAN

1. Roadbed
1.1 Grading Produce a safe, smooth, drained, and lasting road surface free of potholes, ruts, berms, washboard, and organic soil and debris while recovering any useable existing aggregate. This work also includes the reconditioning of drainage dips, turn-outs, and turnarounds. All work to be done in an environmentally sound manner.
Maintain per road surface typical or original shape
Traveled way free of loose rocks 3” or larger in any dimension
Surface free of potholes, ruts, berms, washboard, and organic soil
Recover any usable existing aggregate
Road Safe to drive within speed limit over 100% of traveled way; Surface shaped with proper crown or slope as well as can be accomplished utilizing native surface material over 100% of length.
Leave Seven or fewer loose rocks 3” or larger in two dimensions on traveled way within a mile 100% of the time
Surface prepared according to Quality Control Plan 100% of time; 100% of potholes with a depth greater than 1.5” removed; Leave Seven or fewer visible potholes within a mile 100% of the time; Leave Less than 300’ containing berms, and/or washboard, and/or organic soil within a mile 100% of the time
Recover 95% of usable existing aggregate that has been worked onto the road shoulder and take extra care to prevent wasting usable existing aggregate over shoulder
Drive the road;
Inspection for adherence to Quality Control Plan; Visual Inspection;
Physical measurements
Positive/Negative performance to be reported to NIH.

2. Drainage
2.1 Culvert
Cleaning
Inspect and clean culverts to maintain free flow
Removal of blockages, debris, and obstructions including barrel, inlet/outlet basins on culverts 3’ or smaller in diameter.
Inspect all culverts, flag
95% Free flowing to maximum attainable flow to, through, and away from culvert on culverts 3’ or smaller in diameter; flag and report 100% of larger pipes needing cleaning to

2.2 Ditches Clean ditches to
