QUALITY ASSURANCE SURVEILLANCE PLAN INTRODUCTION:
This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate Contractor actions while implementing this contract. It is designed to provide an effective surveillance method of monitoring
Contractor performance for each listed objective in the contract.
The QASP provides a systematic method to evaluate the services the Contractor is required to furnish.
This QASP is based on the premise that the government desires to maintain a quality standard in operating this contract to provide the optimum service.
The Contractor, and not the government, is responsible for management and quality control actions to meet the terms of the contract. The role of the government is quality assurance to ensure contract standards are achieved.
In this contract the quality control program is the driver for product quality. The Contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a "self-correcting" contract is to ensure that the quality control program approved at the beginning of the contract provides the measures needed to lead the Contractor to success.
Once the quality control program is approved, careful application of the process and standards presented in the remainder of this document will ensure a robust quality assurance program.
Basic Services.
Crosswalks, walkways, handicapped accessibility areas, parking area, roadway’s including entire road of
71 White Mountain Drive, all approaches, electric vehicle gates and manual pedestrian gates shall be kept clear of snow and ice and maintained safely for visitor and employee use in accordance with the time requirements listed above. Late and/or non-performance of the services will not be acceptable to the White
Mountain National Forest. Performance threshold:
SURVEILLANCE: The COR will receive complaints from Forest Service personnel and pass them to the
CO for correction, if applicable.
STANDARD: The standard is two or less customer complaints per month in each area. The COR shall notify the CO for appropriate action in accordance with FAR 52.212.4, Contract Terms and Conditions-
Commercial Items or the appropriate Inspection of Services clause, if any of the above service areas exceed two customer complaints.
PROCEDURES: The COR will consider the customer complaint valid upon receipt from the customer. The
Contractor will be given two hours after verbal notification to correct the unacceptable performance A defect will not be recorded if proper and timely correction of the unacceptable condition(s) is accomplished.
If the number of valid complaints not corrected by the Contractor exceed two in a given month, the COR shall inform the CO, who may impose penalties or terminate the contract for convenience or cause, as allowable by law.
Performance Threshold: 100% of the time.
QUALITY ASSURANCE SURVEILLANCE PLAN:
1. PURPOSE
This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following:
What will be monitored.
How monitoring will take place.
Who will conduct the monitoring.
How monitoring efforts and results will be documented.
This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.
This QASP is a “living document” and the Government may review and revise it on a regular basis.
However, the Government shall coordinate changes with the contractor. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.
2. GOVERNMENT ROLES AND RESPONSIBILITIES
The following personnel shall oversee and coordinate surveillance activities.
a. CO - The CO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and shall safeguard the interests of the United States in the contractual relationship. The CO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.
