PERFORMANCE WORK STATEMENT
REMOVE EXISTING FLOOR COVERING AND INSTALL NEW FLOOR TILE
AT
BUILDING 7532
LACKLAND AIR FORCE BASE
1. SCOPE OF WORK. Contractor shall provide all personnel, equipment, tools, materials, labor, transportation, and supervision necessary to remove existing carpet and base board material in the dining room area and install new composite floor tile squares and base board material in the Amigo Inn dining facility in building 7532, 2215 Andrews Ave, Lackland AFB, TX 78236. The estimated square footage of dining area is 4700 Sq Ft. The facility has been tested and is asbestos free.
All activities shall be coordinated with the customer point of contact stated in the purchase order. All work shall be accomplished in accordance with product manufacturer’s recommendations and industry standards and meet all municipal, state, and federal codes, standards, and guidelines.
2. SPECIFIC TASKS.
a. This is an operational facility. The contractor will start this project on any day Monday through Sunday. All work will be performed between the hours of 7:00 PM and 4:00 AM CST.
b. Due to time constraints and time needed for materials to cure, dry, and settle, the new tile can be installed in sections, if needed. The contractor must submit a written plan to the customer point of contact (and receive approval) describing how they plan to accomplish this task and how the room will be sectioned off. Patrons will be in the facility starting on Monday mornings; if a section cannot be used, then it must be partitioned off by the contractor prior to the facility opening for business.
c. Remove existing carpet and all wall base board material in the dining room area from the facility and transport and dispose of the old materials off of the installation. Remove existing adhesive residue from the floor so that a minimum of 80% of the overall area of the original substrate is exposed. Old floor covering may be present under the existing carpet.
d. Tile Design Pattern. Two colors of tiles will be laid: primary color (70% floor coverage) will be Sandrift White, 51858, and secondary accent color (30% floor coverage) will be Cottage Tan, 51830. Tiles will be laid so that the secondary colored tiles serve as an accent color. Accent colored tile will not set side by side or touch one another. Customer point of contact must approve proposed contractor design.
e. Contractor will have a Material Safety Data Sheet MSDS on hand and available for any and all chemicals, adhesives etc. that are used for this project. MSDS must be available for viewing at all times during the length of this project.
f. The contractor will move all dining room booths, tables, chairs necessary at the start of each work shift to accomplish the removal of existing floor covering and base board material. At the conclusion of work each morning, all dining room tables, booths and chairs must be put back to their original location and readied for use in all areas that can be used which do not have tile or other components that are curing, drying, settling, etc. All tables, booths, and chairs must be free of dust and debris and all floors must have a clean appearance.
g. Contractor will provide 3 cases of the new floor tiles (each color) to the government at the conclusion of the job. These tiles are required to provide future preventative maintenance.
3. WARRANTY. Installation shall be warranted from defect for one year and all commercial warranties shall apply. Warranty shall begin the day of completion and acceptance by 802 FSS/FSVF.
