About this file

This is a Special Requirements/Instructions document for a Request for Qualifications (RFQ) issued by Harris County, Texas for Professional Women's Health Services as a Concessionaire for the Access2Health Channelview Wellness Pod Complex serving Precinct 2 constituents. The RFQ requires vendors to demonstrate a minimum of five years of relevant experience in domestic violence survivorship and mental health services, with specific track record in East Harris County. Submissions must be formatted in six numbered sections (Transmittal Letter, Scope of Service/Narrative, Organizational Information/Qualifications/Experience, Legal Documents, Questionnaire, and Miscellaneous) and limited to twenty pages excluding resumes. Vendors must submit responses through Bonfire or hardcopy to 1111 Fannin, 12th Floor, Houston, Texas 77002 by the deadline specified on the Bonfire platform. The RFQ evaluation process is governed by Texas Government Code 2254.003, with award made to the most qualified vendor based on demonstrated competence. Harris County reserves the right to conduct vendor presentations, site visits, and negotiations with selected vendors prior to final contract execution, which requires Commissioners Court approval.

Evaluation criteria weight professional qualifications and experience at 25 percent, quality and comprehensiveness of proposed methodology at 30 percent, project team qualifications at 25 percent, and verifiable references at 20 percent. Pricing is not included in the RFQ submission and will be negotiated separately after qualifications evaluation; all services must be priced reasonably and affordably given that Precinct 2 has the highest rate of uninsured constituents with the lowest median income. Vendors must complete Form 1295 (Certificate of Interested Parties), maintain current Texas registration and Franchise Tax Identification Numbers, obtain background checks at vendor expense, provide proof of insurance, and comply with HIPAA and OSHA bloodborne pathogen regulations. Harris County expects a single award but reserves the right to award to multiple vendors and may terminate negotiations or service at any time. The awarded vendor is responsible for all toll and parking fees and must return the SmartPod facility in equal condition upon contract termination.
