MARKET RESEARCH QUESTIONNAIRE

Mobile MRI, CT & Ultrasound Services FCC Petersburg, VA

The Federal Bureau of Prisons is conducting market research in compliance with Federal Acquisition Regulation, Part 10. The service will encompass the provision for Mobile MRI, CT & Ultrasound Services. The service will be provided for the prisoners held in the custody of the Department of Justice, Federal Bureau of Prisons, the Federal Correctional Complex (FCI), Petersburg, OH located at 1060 River Road, Hopewell, Virginia 23860.

Mobile MRI, CT & Ultrasound Services will be performed in a designated area behind the gate determined by the Health Services Department at FCC Petersburg. Performance of this service is defined within the attached Statement of Work. Please see for more details.

STATEMENT OF WORK

SUBJECT:

The Federal Bureau of Prisons, FCC Petersburg, Virginia, intends to award a single firm-fixed price type contract to responsible entity for the provision of Magnetic Resonance Imaging (MRI), Computerized Tomography Scan (CT) and Ultrasound. These service will be performed on-site at the Federal Correctional Complex Petersburg (there known as FCC Petersburg) located in Hopewell, Virginia. These services will be provided to the inmate population incarcerated at the FCC Petersburg.

Place of Performance:

Service on this contract will be provided at the Federal Correctional Complex Petersburg, 1060 River Road, Hopewell, Virginia 23860.

Health Requirements:

The contractor shall provide an individual whom is free of communicable diseases. Any individual who subsequently develops a medical condition which might reasonably be expected to place other workers, inmates, or public risk shall be immediately removed from affected activities. In any such case, the contractor shall be responsible, at no additional cost to the institution, for securing the services of any replacement personnel required to perform this contract.

Institution Security:

The contractor must agree to adhere to all regulation prescribed by the institution for safety, custody, and conduct of inmate patients. FCC staff shall be responsible for maintaining security of inmates, as determined by the Chief Correctional Services Supervisor their designee. The custody of Federal inmates remains the primary responsibility of the FCC.

The contractor will also be requires to complete a four hour institution Familiarization Program, as required by policy. Additionally, security related annual refresher training may be required.

Time spent for security related training will be reimbursed to the contractor at the pro-rated session contracted rate.

Prospective contractor will be inquired to comply with the following security investigative procedures:

(1) National Crime Information Center (NCIC) check;

(2) Declaration of Federal Employment (OF-306);

(3) FD-258 (fingerprint check);

(4) Law Enforcement Agency checks;

(5) Vouchering of Employers;

(6) Employment Resume;

(7) Completed Contractor Pre-employment Questionnaire;

(8) Release of Information;

(9) Questionnaire for Sensitive Positions (SF-85 for Low Risk, SF-85P for above low risk);

(10) Credit Check, if applicable;

(11) Urinalysis, if applicable.

Prospective contractor are advised during the solicitation or quotation phase that a urinalysis test is required (for detection of marijuana and other drug usage). If the proposed contract is with a company and the individual(s) assigned to perform the work test(s) positive, the individual(s) shall be excluded from performing the contract and the contractor shall provide acceptable replacement personnel subject to the same security requirements. If the proposed contract is with an individual, the contract award may not be made until a negative test is received.

Inmate Management:

Provision of services under this contract will require supervised contact with inmates. The contractor will not be responsible for the management of inmates except to ensure that those inmates involved in activities within the contractor’s scope of work shall abide by the rules, in effect, to ensure the safety and well-being of all participants.

The contractor will report any infraction of rules and regulations by any inmate to the Health Services Administrator or Contract Monitor (CM).

Schedule:

A session is defined as a site visit. The Contractor will normally provide 4 sessions per month.

Work hours will normally be between 7:00 am-3:30pm, Monday- Friday, excluding Federal Holidays. Based upon previously required services, an estimated number of 12 scan days per year will be performed for MRI/CT and 12 site days per year for Ultrasound at flat rates per visit. Previously required services determined the total of inmates to be scheduled during site visits at the mutual agreement between the agency and contractor, and based primarily on the number of approved consultation requests and exam due dates.

Supplies and Equipment:

The Contractor will supply MRI, CT and ultrasound units, film and all supplies needed for contrast administration and film processing. Any other supplies required for the performance of this contract will be provided by the Government, through the Health Services Department.

Scope of Work:

The contractor shall perform MRI, CT and ultrasound services which include, but is not limited to the following:

The Contractor will provide a dedicated Certified Technologist (one for the MRI and one for the CT)and a driver(one for the MRI and one for the CT) to come into the institution. The Contractor will provide a dedicated Certified Ultrasound Technologist. All personnel entering this institution must pass a security clearance prior to entering the facility, (security clearance takes about 30days). The Contractor will adhere to all regulations prescribed by the institution for safety, custody, and conduct of inmates. The institution will provide staff to perform patient/inmate management. The following must also be adhered to:

a) FDA Certification for the radiological equipment

b) Technologist operating equipment must be licensed and board certified in the according discipline

c) Radiologist providing interpretation services must provide a copy of current license and board certifications in Radiology, and specific evidence of training, education, or experience interpretation of MRI and CT scans.

d) All licenses, certifications, and proof of insurance.

Additional information:

This is a contractual agreement and not an appointment. All duties within this Work Statement shall be performed in accordance with the standards and method generally accepted with the contractors own expertise. The contractor shall not be responsible for the direct supervision of inmates. The contractor will not be under the direct supervision of the Government. However, the contract work will be monitored for performance according to the terms of the solicitation/contract.

Evaluation Factors:

The following factors will be used to evaluate offers. Price and past performance. The offer received that represents the best value to the government, including price, will be selected.

Payment terms:

The contractor shall receive payment for service once each month. Payment terms shall be Net 30 Days. All payments shall be made via Electronic Funds Transfer. The contractor is advised that the government cannot guarantee payment on a certain day of the month and that is the contractor‘s responsibility to ensure that payment has been received.
