Please complete the table below indicating your discounted price to the Government for banking and storing the following number of NCBI units based on the break points listed (pricing bands). Please also include the racial/ethnic breakdown according to the break points. Do not complete the table for any range beyond what is in your proposal (e.g., if your proposal states you will bank 500 units in year 1, you should complete only the first and second columns for that year). Note that your discounted price may be different than your actual expenses for banking CBU. Additionally, if you would not accept a contract for less than the number of units specified in Table D.1, do not complete the corresponding columns in Table D.2 (e.g., if you proposed in Table D.1 to bank 290 CBU in Year 1 and would not accept a contract for 240 CBU in Year 1, do not complete the '0-250' column in Table D.2.).

**Two or more broad races (i.e., AI/AN, B/AA, Asian, NH/PI, or White/Caucasian). Note that Hispanic should not be considered a race.
***Note that NCBI collections in Year 1 may not begin until you are in full compliance with the contract requirements, including IRB approval of any changes to your informed consent documents as requested by the Project Officer. On average, banks do not begin NCBI collections until 2-3 months after the date of contract award. Please factor in this delay when projecting cord blood units in Year 1.
