About this file

This is a General Terms and Conditions document for Waukesha County Bid No. 2025049, issued November 11, 2025, establishing requirements for vehicle modification services for eligible older adults and adults with disabilities in Waukesha County, Wisconsin. The Aging and Disability Resource Center (ADRC) seeks to develop a list of preferred providers to deliver all labor and materials necessary for vehicle modifications on an as-needed basis. The County will award up to three vendors, with award determination based on the percentage discount bid off the NAGS price list plus flat rates for predetermined jobs, inclusive of service costs. Bidders must submit responses via Waukesha County's website in hard copy or electronic format; fax and email submissions are not accepted. The contract term is one year with options to extend for four additional one-year periods, contingent upon fund appropriation. Any requests for clarifications or changes must be submitted in writing at least five working days prior to the bid opening, and bidders are responsible for monitoring the Waukesha County Purchasing website for amendments.

Pricing structure requires vendors to charge customers 10 percent of the total modification cost, with the ADRC invoicing for the remaining 90 percent up to a maximum of $7,000; customers remain responsible for the initial 10 percent plus any amount exceeding $7,000. No additional costs are permitted for minimum orders, overhead, delivery, or fuel surcharges. Price adjustment requests must be submitted 60 days prior to contract year end and will be evaluated based on indexed cost changes. The County prefers payment via MasterCard P-Card with pricing inclusive of all associated fees; alternatively, payment will be made within 30 days of invoice receipt. Vendors must provide proof of insurance prior to service commencement, including workers' compensation with $100,000 limits per accident, commercial general liability with $500,000 minimums across multiple coverages, and automobile liability with $500,000 combined single limits. Funding sources include Wisconsin Department of Transportation grant funds supplemented by consumer contributions. The County reserves the right to cancel the contract with 30 days written notice for non-compliance and prohibits contract assignment without written consent.
