About this file

This file is a Vendor Questions Worksheet for an Invitation to Negotiate (ITN) issued by the State of Michigan for the 2018 Transearch Dataset (ITN 200000000001). The document serves as a questionnaire that bidders must complete as part of their proposal submission. Rather than detailing specific products or services, quantities, or project timelines, this worksheet functions as a standardized form requiring bidders to provide comprehensive information about their company, qualifications, and compliance status. Bidders must identify a sole contact person and contract signatory, provide detailed company background information including legal structure and formation state, and disclose any history of debarment, litigation against Michigan, or contract defaults within the previous five years. The worksheet also requires bidders to describe partnerships and strategic relationships that would benefit the State and identify the physical address of the business location responsible for account management if awarded.

The worksheet includes several compliance certifications and eligibility requirements that bidders must affirm or deny. Bidders must certify their status as qualified disabled veteran-owned or service-disabled veteran-owned businesses if applicable, provide documentation of such status, and confirm they did not participate in developing the solicitation or evaluation process. Additional mandatory certifications address abusive labor practices, Michigan business tax compliance under Public Act 431 of 1984, Iran-linked business status under Public Act 517 of 2012, environmental compliance as a Clean Corporate Citizen, convict labor compliance, and State of Michigan tax payment obligations. Bidders must also authorize the State to verify all information provided in their registration and bidding documents. The worksheet explicitly states that failure to comply with participation restrictions or providing false certifications renders a bidder ineligible for contract award or subcontractor participation.
