About this file
This is an Insurance Requirements Checklist prepared by the Solid Waste Authority of Palm Beach County, Florida for ITB 26-405, covering Asphaltic Concrete Repairs at the North County Transfer Station. The project involves milling and resurfacing approximately 20,000 square yards of existing roadway, full depth restoration of 2,525 square yards, installation of new curbs, striping, and signage. The work must be performed by a FDOT pre-qualified, licensed Florida contractor meeting minimum project experience requirements. All labor, materials, equipment, and incidentals necessary to complete the construction work as detailed in the solicitation documents must be provided by the selected contractor.
The vendor insurance checklist specifies mandatory coverage limits including Commercial General Liability with $1,000,000 per occurrence and $2,000,000 general aggregate, Automobile Liability at $1,000,000 combined single limit, Workers' Compensation at statutory limits, and Umbrella/Excess Liability at $2,000,000 per occurrence. The Solid Waste Authority of Palm Beach County, its officers, employees, and agents must be named as Additional Insured on applicable policies using ISO forms CG2010 and CG2026, with a Waiver of Subrogation endorsement (CG2404) required. Contractors must provide a current certificate of insurance before accessing SWA property or commencing work, and the prime firm is responsible for ensuring all employees and subcontractors comply with insurance requirements. The Authority has established a three percent (3%) goal for Small Business Enterprise subcontracting participation on this project.
