About this file
This is an Anti-Human Trafficking Affidavit required by the State of Florida for the Annual Bid for Liquid Carbon Dioxide contract opportunity. The affidavit document serves as a mandatory attestation that must be executed, renewed, or extended with every contract in compliance with Florida Statute 787.06(13). The undersigned party certifies under penalty of perjury that their entity does not use coercion in obtaining labor or services. The affidavit defines coercion across seven specific categories: use or threat of physical force, unlawful restraint or confinement, exploitative debt practices involving pledged labor, confiscation or withholding of immigration or government documents, threats of financial harm, fraud or deception, and provision of controlled substances for exploitation purposes. The document also defines labor as work of economic or financial value and services as acts performed at the behest or supervision of another, including forced marriage, servitude, and organ removal.
The affidavit requires signature, printed name, title, entity name, and date from the authorized representative of the bidding organization. No pricing terms, set-asides, incumbents, or other financial considerations are specified within this compliance document, as it serves solely as a certification mechanism to ensure contractor compliance with Florida's anti-human trafficking statutes and does not address the substantive terms of the liquid carbon dioxide procurement.
