About this file

This is a Drug-Free Workplace Certification Form required by the Hillsborough County Board of County Commissioners in Florida for vendors bidding on a one-time purchase of two Dodge taper bushed reducer gearboxes and four taper bushing kits, with a project estimate of $30,400.00. The form serves as a compliance document under Florida Statute 287.087, certifying that the vendor has implemented and maintains a drug-free workplace program. The document does not specify response dates, due dates, site visits, bidder meetings, or award dates, as it is a standard certification requirement rather than a solicitation document.

The form requires vendors to certify implementation of six specific workplace requirements: publishing a statement prohibiting controlled substance activity, informing employees of drug abuse dangers and company policy, providing copies of the statement to relevant employees, notifying employees of their obligation to report drug-related convictions within five days, imposing sanctions or requiring participation in rehabilitation programs for violating employees, and making good faith efforts to maintain workplace drug-free status. The vendor's authorized representative must sign and date the form as proof of compliance. No pricing information, set-asides, or funding details are contained in this certification document.
